Abstract
Background: Clindamycin is a suitable antibiotic for treatment of skin and soft tissue infections. Moreover, it can suppress toxin production in many pathogenic bacteria such as S. aureus. There are two mechanisms of resistance in this antibiotic. Constitutive resistance can be detected by standard disk diffusion method but in the case of inducible resistance, D-test should be carried out. The main aim of this study is to determine prevalence of clindamycin inducible resistance among methicillin resistant and susceptible isolates of S. aureus isolated from different clinical samples.
Methods: A total of 87 clinical isolates from clinical samples were collected. Methicillin resistance was determined using standard disk diffusion method. Subsequently, D-test was carried out according to CLSI guideline. Presence of the sea gene (enterotoxin A) was detected by PCR using specific primers.
Results: Out of 87 isolates, 18(20.7%) were clindamycin inducible resistant while constitutive resistance was detected among 21(24.1%) isolates. The 95% Confidence intervals for the proportion of inducible clindamycin resistance among clinical isolates of S. aureus was 12.2% to 29.2%. The inducible phenotype in MRSA isolates was more common than that of MSSA isolates (33.3% vs 5.1%).Significant differences were found between prevalence of inducible clindamycin resistance and type of infection (p=0.045). Importantly, there was a significant correlation between sea gene and the constitutive/inducible resistance (p<0.0001).
Conclusions: Due to the high prevalence of clindamycin inducible resistance among clinical isolates of S. aureus, we recommend D-test to avoid treatment failure.

Abstract
Background: Staphylococcus aureus is a major foodborne pathogen throughout the world. Enterotoxins and toxic shock syndrome toxin-1 are important virulence factors and as pyrogenic toxin superantigens have profound effects on the ir host. Thus circulation of TSST1 producing S.aureus among people and food chain is a worrying issue. The present paper was conducted to study Prevalence of tst, entC, entA and entA/C genes in staphylococcus aureus strains isolated from different foods.
Methods: Over 1040 food samples have been analyzed differentially according to Iran national standard (number= 1194) for S.aureus identification. After DNA extraction, PCR reactions were carried out by reference strain as positive control, adequate primers.
Results: At present study, prevalence of foodstuffs contaminated by S.aureus isolates was about 9.5% (100 strains). Of 25% of isolates producing entC, 28% (seven strains) had tst gene at the same time and of 8% of isolates producing entA, 12.5% (one strain) were positive for tst genes simultaneously. Altogether of 9% isolates producing combination of entC and entA, 44.4% (four strains) were also producer of tst gene.
Conclusion: Prevalence of TSST1 producing strains in combination with enterotoxin genes is considerable especially with entC and A plus C. On the other hand, circulation of these isolates in humans, animals, foods and environment has hazardous effect for general public health.

Background: Nocardiosis is a rare and potentially
life-threatening infection caused by several species of the Nocardia genus. The objective of this study was to develop and evaluate a rapid and new method to clinically identify relevant Nocardia species. Rapid and accurate diagnosis of Nocardia species is essential
for the treatment of severe infections and prevention of cerebral abscess.
Methods: One hundred and eighty patients, 103 (57.22%) male and 77 (42.78%) female, with severe symptomatic
pulmonary infection were studied in the course of a 12-month period in Dr.
Shariati Teaching Hospital affiliated to Tehran University of Medical Sciences
in 2010. The specimens were cultured and identified
using microbiological and biochemical tests. Polymerase chain reaction (PCR) was used to directly identify the organism in the
broncoalveolar lavage samples collected from the patients. NG1 and NG2 primers were used to amplify a Nocardia genus-specific 598-bp fragment of 16S rRNA.
Results: Nineteen samples (10.56%) were positive with PCR and 5 samples (2.78%) with conventional methods. All
samples with positive cultures were also positive by PCR.
Conclusion: The results of this study showed that PCR has a high
sensitivity and accuracy for the detection of Nocardia compared with
culture and biochemical tests. Considering the rapidity, precision, high
sensitivity and specificity of molecular techniques, use of these techniques is
suggested in conjunction with conventional methods for the detection of Nocardia phenotypes in clinical laboratories and research centers.

Abstract
Background: Nowadays, Human adipocyte-derived stem cells (hADSCs) has been widely used in tissue engineering because of its unique features such as extraction from more sources, more easily and non-invasive extraction methods. In order to increase cell-cell interactions, similar to embryonic pre-cartilage condensation, the use of three dimensional (3D) high-density cell culture systems such as Pellet and Micromass that simulates optimal condensation in chondrogenesis in vivo is necessary. Also, these culture systems provide the proper diffusion of nutrients. Aggrecan is a proteoglycan and one of the important components of extracellular matrix of cartilage tissue that plays an important role in the organization of the extracellular matrix. The high concentrations of aggrecan produces the osmotic properties that is necessary to normal tissue function of cartilage. In current study, Aggrecan gene expression was investigated during chondrogenesis of hADSCs in two Pellet and Micromass culture systems.
Methods: This experimental study was done in Department of Anatomical Sciences Department of Faculty Medical in Isfahan University of Medical Sciences, Iran, from April 2013 to January 2015. First, the abdominal adipose tissue was obtained from three patients after obtaining written consent during their liposuction surgeries. ADSCs were extracted by mechanical and enzymatic methods and were cultured in monolayer culture. Then, in order to induction of chondrogenic differentiation, 5×105 cells of third passage (P3) were transferred to three-dimensional culture systems Pellet and Micromass containing chondrogenic mediums in experimental groups of 7 and 14 days. The evaluation of aggrecan gene expression was performed by real-time PCR technique.
Results: Gene expression analysis revealed that aggrecan was significantly increased in micromass culture at day 14 compared to Pellet culture at days 14 and 7 (P≤0.01). Also, aggrecan was significantly increased in Micromass culture at day 7 compared to Pellet culture at day 7 (P≤0.05).
Conclusion: Due to higher expression of aggrecan gene in Micromass culture compared to Pellet culture, this system may be more efficient than Pellet culture in synthesis of aggrecan in chondrogenic differentiation of ADSCs.

Abstract
Background: The approach of medical ethics training courses at Tehran University of Medical Sciences to change the attitude and promote medical ethics knowledge of learners has challenges. This study aims to identify the challenges in teaching medical ethics at Tehran University of Medical Sciences.
Methods: This is a qualitative study with semi-structured interviews conducted in April 2018 at Tehran University of Medical Sciences. Using purposive sampling, 23 participants were selected from the clinicians and faculty members of medical ethics and medical students. Data were analyzed by the content analysis method.
Results: The challenges of teaching medical ethics from the participant's point of view are classified into three themes: 1- Hidden curriculum 2- Necessity of completing medical ethics education program 3- Executive and managerial macro factors. The results showed that medical ethics training courses are not effective for changing students' behavior and their moral decision-making. There are substantial shortcomings in the current curriculum in terms of content, format, and implementation that make it unresponsive to ethical needs and concerns. Teaching medical ethics should be turned into a longitudinal theme. The duties and missions of the Department of Medical Ethics at the university are not well understood. The Department does not act as a strong executor and supervisor of medical ethics in interaction with higher authorities to pursue the requirements of effective ethics education and to ensure the implementation of ethical codes. There is no effective mechanism for evaluating the ethical performance of activists and students and giving feedback to them. There is insufficient organizational support for students' complaints and reports about the misconduct and unethical behavior of faculty or staff.

Conclusion: The effectiveness and efficiency of medical ethics courses to change the behavior and attitude of learners are not acceptable. Dealing with the existing challenges requires the efforts of the Medical Ethics Department to make maximum use of available resources and interact effectively with other academic departments.

Abstract
Background: Non-traumatic bone fractures are considered a pathologic condition with various etiologies, including cancer metastases, osteoporosis, and long-term corticosteroid consumption, which can affect people's quality of life. The main aim of this study was to investigate the etiology of pathological bone fractures in patients who were referred to the Hospital.
Methods: In this cross-sectional study, all patients with non-traumatic pathological fractures were included from March 2015 to February 2019 at Imam Khomeini hospital in Urmia city. Data analysis was calculated by chi-square test using SPSS.ver.17.
Results: A total of 168 patients participated in this study, of which 108 patients (64.3%) were female, and 60 patients (35.7%) were male with a mean age of 58.6±21.1 with a minimum and maximum age of 7 and 90 years old, respectively. According to the results, the most common etiologies consist of osteoporosis (33.3%), metastatic carcinoma (28.6%), and primary tumor (11.3%), respectively. Regarding the anatomical site involved, vertebrae (57.1%) and the neck of the proximal femur (27.4%) were detected as the most frequent sites with fractures, respectively. Moreover, a significant relationship was found between the fracture etiologies and demographics data (age and gender) (p<0.001). In this regard, osteoporosis and metastatic carcinoma were reported as the most common fracture etiologies in females and males. In addition, intertrochanteric-subtrochanteric (7.7%), distal femur (4.8%), intertrochanteric (1.2%), and acetabulum (1.2%) were considered other sites of fracture with less frequency. It is worth noting that for patients with ages less than fifty years old (<50 years old), commonly reported fracture etiology was the primary osteosarcoma, while in patients with ages more than fifty (>50 years old), osteoporosis was defined as the main fracture etiology.
Conclusion: Together, the present study results showed that osteoporosis and metastatic carcinoma were the most critical etiology of fractures, and there was a statistical correlation between demographic characteristics (such as gender and age) and pathological fracture etiologies. According to the statistical results, the most common sites exposed for fractures were also the vertebral and proximal femur.

Abstract
Background: Salmonella enterica subspecies enterica serotype Enteritidis (S. Enteritidis) is one of the leading causes of food-borne infections associated with the consumption of contaminated food products of animal origin in humans. gastroenteritis due to Salmonella is usually a self-limiting disease and does not require antibiotic therapy. However, antibiotic treatment for salmonellosis may be lifesaving for patients with severe infections.The objective of the present study was to examine antimicrobial resistance and determine its genetic basis in recently isolated S.Enteritidis strains.
Methods: During this study, in a cross-sectional descriptive study, 44 isolates of Salmonella enteritidis from human sources were investigated between September 2021 and September 2022.. After identification of the isolates using phenotypic and molecular methods by Multoplex-PCR, antibiotic resistance testing was performed according CLSI 2023. The strains were examined for the presence of qnrA,qnrB,qnrS and gyrA resistance genes by PCR.
Results: In a cross-sectional descriptive study, 44 isolates of Salmonella Enteritidis from human sources were investigated between September 2022 and September 2023. 100% of the strains were sensitive to imipenem and meropenem, and the sensitivity to the antibiotics ceftriaxone, ceftazidime, and cefotaxime were 93.2%, 90.9%, and 94.1%, respectively. 81.8% of isolates were sensitive to cotrimoxazole, sensitivity to ampicillin was 84.1%. Only 9.1% of isolates were sensitive to ciprofloxacin. Based on MIC results, 16 isolates had MIC between 0.002 and 0.064 and were placed in the sensitive area. 28 isolates had MIC between 0.125 and 0.5 and were placed in the area of reduced sensitivity. None of the strains resistant to disk diffusion method were resistant to MIC method. qnrA, qnrB and qnrS genes were not observed among ciprofloxacin resistant strains. All nalidixic acid resistant strains had gyrA gene.
Conclusion: In general, it was shown in this study that the resistance to the fluoroquinolone family is increasing among Salmonella Enteritidis isolates. On the other hand, we see a decrease in the sensitivity and prevalence of strains resistant to broad-spectrum cephalosporins among serovar Enteritidis, which is the drug of choice for extraintestinal infections.

Abstract
Background: The prevalence of cesarean section (CS) has recently increased dramatically. Cesarean scar defect (CSD) is one of the most common complications of CS. This study aims to investigate and compare the depth and volume of the CSD in symptomatic patients with and without uterine adenomyosis.
Methods: This retrospective case-control study was done from November 2020 to November 2022 at our referral women's hospital. The patients who underwent sonohysterography with a previous history of one or two CS for at least one year ago and chief complaints of abnormal uterine bleeding were included in our study. The patients with myometrial or endometrial pathology were excluded from the study. The symptomatic patients who met the inclusion criteria were divided into two groups: with adenomyosis (case) and without adenomyosis (control). The demographic data, medical history, and sonohysterography findings were obtained from the patient’s documents and archived images. In sonohysterography, CSD features, including the length, depth, width, volume, RMT (Remained Myometrial Thickness), AMT (Adjacent Myometrial Thickness), and RMT/AMT were measured. To analyze the variables, t-tests, and chi-square were used.
Results: Among 310 symptomatic women with a history of previous cesarean section underwent sonohysterography, 160 patients met the inclusion criteria (case group with adenomyosis: 82 patients and control group without adenomyosis: 78 patients). The chief complaints were post-menstrual bleeding (43.8%), prolonged bleeding (34.3%), and intermenstrual bleeding (21.9%). In the case group, 25.6% had CSD with RMT<2mm, but in the control group (without adenomyosis), only 6.4% had RMT<2mm. CSD depth and volume were significantly larger in patients with underlying adenomyosis (P-value of 0.002 and 0.038, respectively). Also, the mean RMT and RMT/AMT ratio were significantly lower in the case group (P-values of 0.004 and 0.00, respectively).

Conclusion: Our study demonstrated a strong association between the presence of adenomyosis and larger CSDs. To establish a causative relationship, we suggest a prospective cohort study to follow up the patients and compare the evolution of CSD in patients with and without adenomyosis.
