Abstract

Different aspects of the effect of trauma on the health status of populations have been studied annually 35 millions deaths, 3.5 millions disabilities and a sum of 500 billions $ cost. Evaluating different modes of prevention, a comprehensive study was carried out in the US during the 7th and 8th decades, introducing the TRISS method for the purpose of comparing the outcomes of trauma cases among multiple centers. This study has been carried out during 1996-97 in there hospitals of Tehran. After entering the hospital, all trauma cases were traced by trained interviewers until leaving the hospital. Based on the TRISS model, probability of survival was calculated for each patient and compared with the results of the MTOS study. Among 4863 cases evaluated, 300 (6.2%) deaths were observed, which is significantly more than the expected number (222 deaths, 2.7%) calculated under TRISS model. This difference may indicate a difference in the quality of trauma care.

Abstract

The problem of AIDS had not been recognized up to 1981 and in 1984 it was found that HIV virus is factor of this disease. For the time beings AIDS/HIV infection has chanched to a pandemic and cases affected to it are now reported from all over the world. In our country the first case was reported in 1987. Regarding the importance of disease and recognition of its prevention and transmission routes, based on disease epidemiology in country, this survey was conducted. This study was performed in 1999-2000 and is retrospective descriptive study. The main purpose of study is epidemiology of AIDS in Iran from beginning until now. Total information of cases of AIDS/HIV from beginning in Iran and all performed activities were obtained from ministry of health center for disease control-AIDS Dept. 95 files existing in the archives of Imam Khomeini hospital related to AIDS were also studied. 1953 cases of HIV+ have been reported until March of year 2000. At the same time 250 cases of AIDS have been reported which 215 of them died. Among transmission routes, in AIDS disease the most common way of transmission was transfusion of infected blood or its components but in HIV+ case the most one was drug injection. With respect to age, the higher and lower rate of affected people were in 30-39 and 0-4 ranges years respectively. The sex percentage in AIDS affected and HIV+ persons were 90.8% and 9.2% 95.2% and 4.8% male and female respectively. 130 from 1953 HIV+ cases had travel to abroad.

Abstract

This descriptive study is carried out to determine the effectiveness of family planning programmes in Iran for years 1991-96. The study revealed the following outlines: Contraceptives prevalence rates increased from 42% to 67% during years 1991-96. Zaro couples years of protection increased for norplant, injectable contraceptive, IUD, vasectomy and tubectomy, but for candom it is decreased in year 1996 (Comparing to earlier years of the study). A decrease in use failure rate of contraceptives was observed during the period of the study.

Abstract

Introduction: For the time being we have considered that the myocardial infarction is an increasing event in Islamic Republic of Iran and there are many procedures and methods which can help us to diminish the number of death from this ongoing event. The main aim of this research is to determine the survival rate in those patients who have had acute myocardial infarction and the association of it with different variables.

Methods and Materials: The present research is a descriptive case-series study which evaluates the 100 cases of acute myocardial infarction who had been admitted in Tehran Emam Khomeini Hospital during the year 1999.

Results: The mean age of patients was 57 years. The peak of attack rates was in spring and autumn. Investigating of the past history of these patients reviled that 41 percent had been smokers, 63.5 percent have had the history of previous ischemic heart disease, 41 percent have had hyper cholestrolemia, 34 percent had hypertension, 18 percent had diabetes mellitus, 9 percent had mitral rigurgitation and 9 percent had heart block. The Survival rate in our study has been calculated 68 percent in first 28 days of disease.

Conclusion: In our study we concluded that there is significant correlation between survival rate and past history of hypertension, ischemic heart disease, tobacco smoking and clip classification.

Abstract

Background: Brain hypothermia by reducing the temperature of the cerebrospinal fluid is done by a cooling pad in the thoracic region and protect brain from the ischemic injuries. Along with the spinal cord, the brain is an essential partner in the central nervous system, and similarly, it is surrounded and protected from the bony skull and from shock by cerebrospinal fluid. The brain analyzes information that is both internal and external to the body, transforms the information into sensations, and stores them as memories. So in this study we investigated the brain hypothermia by finite element modeling.

Methods: To investigate this phenomenon, in this study a numerical model of the head with respect to the structure of brain tissue and its contribution to heat transfer is presented in the fluid lab of the Amirkabir University of Tehran in January of 2016. In this model, Pennes's bioheat equation and finite element analysis has been used to predict temperature distribution in the brain tissue. The model geometry is designed in two state without considering the ventricles of the brain that are involved in the production of cerebrospinal fluid and with considering cerebrospinal fluid. So, in the second case, the cerebrospinal flow is considered as a heat transfer factor.

Results: We concluded that with cooling about 5 °C, in the first model without considering the ventricles, the gray matter temperature is reduced by about 4 °C and there is no change in white matter temperature. In the second model temperature distribution became more asymmetric. The temperature reduced about 3 °C in the corners. However, the temperature reduction at the edge of brain tissue and near cerebrospinal fluid were about 0.5 °C.

Conclusion: It was observed that in the case of ischemia, the temperature drop was higher than normal. So, during brain injuries to prevent serious damage, the brain metabolism can be reduced by cooling the spinal fluid.
