Abstract

Background: Pre-eclampsia is characterized by hypertension development and proteinuria during pregnancy. Hypertension disorder is a leading cause of maternal and fetal morbidity and death in worldwide. Although the pathophysiology of hypertension during pregnancy is unclear, but there is consensus that early diagnosis and aggressive treatment is warranted to prevent complicated to both fetus and mother. The changes of serum trace elements during pregnancy are paramount important to predict and good understanding the situation of patients. The aim of this study was about this issue.

Materials and Methods: In a case-control study we investigated the possible differences in the level of serum calcium, phosphorus and total protein in 55 healthy pregnant and 52 pregnant with hypertensive disease at 32-40 weeks of gestational age during the recent two years in Loghman hospital of Tehran. Some information such as age, blood group, parity number and blood pressure was taken from patients by a questionnaire.

Results: The case population consisted of 22 sever preeclampsia, 15 mild preeclampsia, 8 eclampsia, and 7 chronic hypertensive. The mean serum calcium concentration (mg/dl) was 9.180.74 in control group, 8.810.9 in mild preeclampsia, 7.850.38 in sever preeclampsia, 7.83 0.47 in eclampsia, 8.91 0.3 in chronic blood pressure. The mean serum phosphorus (mg/dl) level observed, 4.27 in sever preeclampsia, 3.74 in eclampsia, 3.59 in mild eclampsia, 4.09 in chronic blood pressure, and 3.43 in control pregnant women. The mean serum total protein concentration level in sever preeclampsia and eclampsia was 5.46 and 5.04 mg/dl respectively.

Conclusion: In conclusion, sever preeclampsia and eclampsia are associated with decreased level of calcium, total protein, and increased concentration of phosphorus.

Abstract

Background: Degenerative spondylolisthesis is a common disease of the lumbar spine especially in older ones. The disease represents a challenge to the treating physician. At present, for those patients that deteriorate clinically, there are many proposed algorithms for the surgical treatment. This before and after study was undertaken to assess the surgical results of decompression and instrumented posterolateral fusion in these patients.

Methods: The study population consisted of 23 patients who had undergone no prior surgery for degenerative spondylolisthesis on the lumbar spine. These patients were treated by decompression, bilateral posterolateral fusion, and segmental (pedicle screw) instrumentation with mean follow-up of 29 months (range, 13-73 months). Finally, The clinical results were evaluated for all patients by means of an Oswestry Disability Index (ODI) version 2.1, the Henderson's functional capacity, and persistence of leg symptoms, low back pain or claudication. Mann-Whitney and Chi-Square tests were used to assess the average values and comparison, respectively.

Results: Henderson's functional capacity at the last visit session was excellent in 14 (60.9%), good in 7 (30.4%), fair in 2 (8.7%) cases. ODI decreased from 72.2% (50-88%) preoperatively to 14.4% (0-54%) at the latest follow-up visit. A history of leg pain or claudication was correlated significantly with the amount of decline in ODI score and Henderson's functional capacity (p<0.05).

Conclusion: In spite of limited number of our patients, decompressive surgery plus instrumented posterolateral fusion is a safe, reliable, and satisfactory procedure for treating degenerative lumbar spondylolisthesis. This procedure may be done when conservative treatment was failed and psychological problems can be ruled out.

Abstract

Background: Evaluation of diastolic dysfunction and its relationship with TIMI frame count in patients with stable coronary artery disease referred to Ayatollah Taleghani Hospital in Tehran in the years 2010-2011 is the purpose of this study.
Methods: Patients were divided into two groups: case and control. Both groups had chronic angina. Patient information on check list has been studies based on data provided by angiography and echocardiography methods.
Results: Cases had significantly higher values of corrected TIMI frame count (TFC) for the left anterior desending artery (LAD), TFC for Circumflex artery (Cx) & Right coronary artery (RCA), (mean TFC P<0.001). Conventional echocardiography showed significantly lower maximal peak systolic velocity (sm) (cm/s), Correlation of early diastolic velocity at myocardial segments (Em) & peak systolic velocity at myocardial segments (Sm) with mean TFC in all 3 vessels were significant (P=0/0001). Ratio of maximal early to late diastolic filling (E/A), in patient with TFC≥ 21 was 0/7 & in patients with TFC≤21 was 1. DT (Deceleration time) had no significant correlation with TFC. Mean of isovolumetric relaxation time (IVRT) in cases was 91 m/s & in controls was 72 m/s Which was higher in patients with mean TFC ≥21. Correlation between mean of angiography and diastolic disfunction was significant.
Conclusion: According to this study diastolic dysfunction which estimated by echocardiography showed significant correlation with TIMI frame count in chronic stable angina patients. Due to simplicity, low cost, quality and reproducibility of this method, this will be helpful.

Abstract

Background: Considering nitric oxide (NO) has an important role in many biologic processes of cells and tissues such as in the digestive system and in this system act as a second messenger in pathological and physiological events in gastrointestinal region, in this study we investigated the effects of L-NG-Nitro arginine Methyl Ester (L-NAME) as the NO formation inhibitor on parietal cells of stomach in pregnant rats. Methods: Twenty four female rats were prepared and with eight weeks old and 200-250 g weight were used in this study. After matting of the female rats with the male rats, time of observing vaginal plaque considered as the zero day of pregnancy. Then the animals were divided into three groups of studying. Each group was containing eight rats. In this study, except the control group, the saline group received 2 ml/kg normal saline and experimental group received 20 ml/kg L-NAME interaperitoneally (IP), respectively on third, fourth, and fifth days of pregnancy for evaluation of its effects. On the 18th day of pregnancy, after anesthesia with ether, the animals were killed and dissected and the laparotomy was performed to separate the mother’s stomach. Then, the stomach was fixed in 10% formalin and after tissue passage, the sections were stained with Hematoxylin-Eosin (H&E). Then the changes of count and diameter in parietal cells were observed via light microscopy and Image Tools III. Results: Results of this study after analysis showed the significant changes in parietal cells count (mean 61.3±4.32) and its diameters (mean 16.12±1.18 µm) in L-NAME group in comparison to control and the sham groups in pregnant rats (P≤0.05). Conclusion: Results of this study showed L-NAME with effects on NO synthesis can reduce the count of parietal cells and increase its diameter in pregnant rats and has destructive effects on structure of stomach parietal cells in pregnancy rats.

Abstract

Background: Cytomegalovirus (CMV) stands out as a significant contributor to congenital viral infections, exhibiting a prevalence ranging from 40% to 90%. Cytomegalovirus has different effects on people, such that it appears without symptoms in people with a healthy immune system, but it leads to severe symptoms in infants and those with a weak immune system. To comprehend the disease's societal prevalence, seroepidemiological investigations are imperative. This study aims to ascertain the serum prevalence of cytomegalovirus and explore potential correlations between age, sex, and the prevalence of CMV in patients attending Shahid Motahari Clinic in Shiraz
Methods: A cross-sectional descriptive study was undertaken on 2,469 individuals who assessed their cytomegalovirus antibody levels between March 2019 and February 2023 at Shahid Motahari clinic in Shiraz. The ELISA method was employed to evaluate CMV, and the obtained data were analyzed using SPSS version 19 software. Chi-square tests and Fisher's exact test were applied to examine the correlation between positive antibody results and age as well as gender.
Results: Among the 2469 surveyed people, 658 people (26.65%) were men and 1811 people (73.35%) were women. Based on ELISA test, antibody titer was reported positive in 1157 people (46.9%), including 315 men and 842 women, and 1312 people (53.1%) had negative antibody titer. The highest number of positives is related to IgG, and in IgM, the test results are mostly negative. IgM prevalence showed no gender correlation but demonstrated a significant association with patient age. Meanwhile, IgG prevalence exhibited significant relationships with both age and gender
Conclusion: Considering that the city of Shiraz is considered as one of the centers of treatment in the country and a large number of patients from all parts of the country and even neighboring countries come to this city for treatment and especially for the purpose of organ transplantation; also with Considering the 46.9% prevalence of CMV infection in the region, it is recommended to use preventive methods such as vaccine and immunotherapy against CMV infection in patients

Abstract

Background: Vitamin D deficiency is common among pregnant women and can lead to maternal and fetal complications during pregnancy. This study was conducted with the aim of investigating the relationship between the mother's vitamin D serum level and the infant's anthropometric indices and neurodevelopment.
Methods: This prospective cohort study was conducted on a group of pregnant women who visited Imam Khomeini Hospital in Tehran from September to March 2020. Demographic information and anthropometric characteristics of the newborn were collected using the medical record. Blood samples were collected at the time of delivery to evaluate the mother's vitamin D status, and serum vitamin D levels were measured using standard laboratory procedures. The neurodevelopment of infants was assessed at the age of 6 months through the Ages and Stages Questionnaires (ASQ). Data analysis included using descriptive statistics to summarize maternal and neonatal characteristics. The relationships between maternal serum vitamin D levels, neonatal anthropometric indicators, and neurodevelopmental outcomes were examined through Spearman's rank correlation and Mann-Whitney U tests.
Results: In total, 123 pregnant women participated in this study, with a mean age of 31.41 years (±5.75). The mean birth weight, height, and head circumference of the neonates were 3208.33 grams (±706.05), 34.32 cm (±3.91), and 50.33 cm (±2.30), respectively. Statistical analysis showed no significant correlations between maternal serum vitamin D levels and the infants' weight (P=0.318), height (P=0.531), or head circumference (P=0.241). Furthermore, there was no significant association between maternal vitamin D levels and any of the assessed neurodevelopmental domains at six months (P > 0.05).
Conclusion:  While this study did not find a positive correlation between the mother's vitamin D serum level and the anthropometric indices and neurodevelopmental domains of infants at six months, it is important to interpret these results cautiously due to the study's limitations. Thus, it is advisable to conduct prospective cohort studies with large sample sizes in diverse demographics nationwide.
