Abstract
The cyclical changes in the gonadotropins stimulate ovarian follicular development either to the ovulatory stage or to undergo atresia. One such intrafollicular factor may be inter-cellular communication via gap junctions. We have examined the effects of two agents (retinoids and alkanols), known to disrupt or uncouple gap junction, on FSH-stimulated LH receptor induction and progestrone synthesis in granulosa cells. Granulosa cells from diethylstilbestrol-treated immature rats were isolated and cultured in serum-free medium containing either FSH (15 ng/ml) or FSH and estradiol (30 nM), various doses (0.1-1000 nM) of either retinoic acid were added to the cultures at the time of seeding. Additional cultures containing the same concentrations of either FSH, or FSH and estradiol, were treated with 0.01-10 nM heptanol or octanol. The results of this study showed that: 1) Retinol, at all of the concentrations tested, had no effect on either FSH-stimulated LH receptor induction or progesterone accumulation by the granulosa cells. 2) Retinoic acid suppressed both LH receptor induction and progesterone accumulation by the cells. 3) Heptanol and octanol suppressed LH receptor induction but did not have inhibitory effect on the progesterone accumulation.

Abstract
The discovery of antifertility activity of gossypol led the scientists and researchers to the development of compounds with antifertility activity that can be used as male contraceptive. It is obvious that without the contribution and sense of responsibility in men, the population control programs and family planning will have no considerable success. In the present study, I have tried to evaluate two newly synthesized analog of diaminopyrimidines compounds with the chemical name of 2,4-diamino-5 (3,4-dichlorophenyl) 6-alcheloxymethylpyrimidine (ethyoxy and isobothyloxy) on male rats. This study indicates that this compounds without any toxic effects, significantly decrease the serum testosterone, sperm mortility (SM), percent of viable sperms, daily sperm production (DSP), epididimal sperm reserve (ESR) and fertility rats

Abstract
In this research anxiety and depression and their association with 32 different factors in students of Hamedan University of Medical Sciences were measured. Levels of anxiety and depression were assessed by State-Trait Anxiety Inventory (from 2, Trait) and Beck Depression Inventory. A questionnaire made of demographic data, educational and noneducational activities, the presence of medical or psychiatric disorders (over last year) and students' feelings about relationships with their parents as well as educational and financial matters was given to the subjects. Seventy percent of all of the university students completed the two inventories plus questionnaire. Twenty-one percent had a number of 18 or more (the cut-off point for depression) in Beck inventory and 33.4% had a number of 51 or more (the cut off point for anxiety) in State-Trait Anxiety Inventory (from 2, Trait). Differents levels of depression and anxiety and associated variables in each group were determined and discussed.

Abstract
Due to the important role of the trace elements (i.e. zinc and copper) in the biochemical processes of body and the effects of exogenous and endogenous hormones in distribution, absorption and excretion of these elements, we decided to determine the rate of the zinc and copper content of serum in women taking (LD) OCPs. Our finding indicates that the rate of the zinc content decreased significantly (P<0.001), whereas the copper content of women serum during this period showed a significant increase (P<0.001). In regard to previous months, the process of increasing were lasting for six months. Therefore, we would like to suggest that the serum content of these two elements should be monitored during the period of medication with OCPs, to avoid the deleterious effect of flactuation rates of these elements in serum.

Abstract
The poll was designed to let the management system of the university know about the opinions, criticisms and suggestions of the faculty members and create a sense of cooperation and promote the level of relationships. In November and December 1997, short after the new Chancellor of the University was selected, the poll was conducted by the new board to reach the above goals. In this survey, all the faculty members of the University were questioned about the problems they faced in the University. A questionnaire of 23 questions in two pages was given to them and was returned by mail. About 47.5% of the questionnaire were returned. Analysis of the results showed that a significant majority (93%) of the respondents believed that Tehran University should be the best University of Medical Sciences in Iran, and that it dose have the capability to be so, however, a majority (66%) of them claimed that it is not so at the present. A significant majority of the respondents believed that education and research are the two most important duties of the University, and the health, treatment, and cultural duties rank next. Except for the Medicine and Health Faculties others were not satisfied with their status at the University. The satisfaction of the faculty members with educational and research performance of the faculties was questioned the results varied in different faculties. Dissatisfaction with research status was most obvious. Forty-six and 25 percent of the staff in the faculty of medicine were satisfied with educational and research performance of their faculty, respectively. The teaching staff were also questioned about their problems the most important of which was shown to be that of convenience. More than 90% of respondents stated that opinion poll is an appropriate way to relate the management of the University to the faculty members and emphasized on holding periodic sessions with the Chancellor of the University and personal visits to him.

Abstract
Increase in the rate of elderly population have persuaded the people and governments of the most developed countries to evaluated and confront the complexity of social, economic, and health issues involved. Increase in the number of aging people is commonly called the aging of society. Currently, in the countries of Western Europe (Germany, France, England, Sweden), the figure of the people over 60 years old comprises 12-15 percent of total population, and according to the forecasting estimate will reach to 25% up to the year 2020. In our country, based on censuses that have been carried out during the years of 1355, 1365, 1375 this figure is 5.12%, 5.25% and 7.67%, respectively. The number of 60 years old persons or over under the supervision of Tehran University of Medical Sciences was 135379 persons, in the year 1376. Due to these demographic changes that is arising from the phenomenon of aging of society, a considerable increase in the incidence of chronic diseases and health requirement of the elderly living in the society happens. This can be translated in terms of more demands in the affiliated fields of nursing, health care and treatment. This study has been proposed to clarify and evaluate the afore mentioned requitements. In this study, the following results can be concluded. Contrary to physical and psychological problems of adulthood, only 25% were aware of assisting economic, health and treatment institution that were available to them. These people were usually suffering from a single to multiple ailments that happen concurrently and ladies were complaining more in this respect. The interesting point in this study is the answer to the question of most basic problems of their life. Many elderly mentioned financial problems the most influential factor in their life. The figures concerning the responses to the question of basic problems of elderly for economic and physical problems were 37.7% and 18.3%, respectively.

Abstract
We studied fertility indices of Iranian and non-Iranian populations which were covered by 23 Rays health houses. This cross-sectional study aimed to prove the effect of increasing number of non-Iranian immigrants on health indices of the area covered by Tehran university of medical sciences and health services, consequently giving attention to specific health programs. The data collection method was census of permanent residents of the villages by means of vital horoscopes. 8494 of 27611 persons were non-Iranian. Dependency ratio and natural growth rate was 75%, 1.14% in Iranians and 98%, 1.74% in non-Iranians, respectively. Mortality in non-Iranians was 1.24 times more than Iranians. General fertility rate in Iranians was 60 and in non-Iranians was 110 live births in 1000 women of child bearing age. Total fertility rate in Iranians was 1.73 and in non-Iranians 3.27 neonates for each woman. Age-specific fertility rates were absolutely higher in non-Iranians. Gross eproductive rate was 0.87 in Iranians and 1.59 in non-Iranians. Mean number of girls born by a non-Iranian mother will be double times of an Iranian mother (P<0.01). Modern contraceptive methods coverage was 70% in Iranian couples and less than 40% in non-Iranian couples. 96.94% of Iranian deliveries and 70.9% of non-Iranian deliveries were safe (P<0.01).

Abstract
A role of adenosine in pathogenesis of syndrome X has recently been postulated. Previous studies suggest that aminophylline (An adenosine receptor blocker) improves exercise tolerance in this disorder. Present study was performed to examine the efficacy of Theophylline in syndrome X and to determine the incidence of cardiovascular risk factors and clinical, ECG and angiographic findings in these patients. 31 patients were studied. The first 16 patients received Theophylline. Other 15 cases received placebo. Exercise tolerance test was repeated after 1-2 weeks. Patients who received Theophylline had increase in rate-pressure product at peak exercise and decrease in symptoms during exercise, but stage of st depression, global and peak st depression, exercise tolerance and persistence of st depression more than 3 minutes in recovery and symptoms of patients in general were not significant different. Theophylline has a favorable effect on exercise, reduces chest pain and rate-pressure product at peak exercise in patients with syndrome X, but not on other variables of exercise test.

Abstract
Concomitant use of lithium and E.C.T has always been accused to cause delirium in patients receiving such a combination. In this study incidence of delirium in patients who receive lithium+E.C.T. concurrently has been compared with those who have been treated with E.C.T. only. Of 49 patients who had Bipolar Mood Disorder (B.M.D.) 1 disorder (manic episode) 24 were given E.C.T.+lithium and 25 were treated with E.C.T. Only, 3 patients of the first group and 2 patients of the second group developed delirium. The difference between two groups was not statistically significant. Another finding was that all cases of delirium developed in patients who were above 35 years old (P value=0.001). These findings show that combination of E.C.T. and Lithium may not be so harmful as it was once considered. On the other hand it could be concluded that increased age may be a risk factor for delirium in such a combination.

Abstract
The healthy city is one that is designed to achieve optimal echological, social, and physical specifications for residents of the community that are developing constantly, and that a comprehensive different intrasector cooperation, resources and existing utilities are developed so that an environment of mutual cooperation and assistance for every individual civilian are provided. An environment to maximize their potential ability. One of the specific goals of healthy city, is the development of public knowledge, and cooperative involvement in social health and hygienic programs. The project of healthy city is carried in the Koyee Sizdah Aban, Rey city for the first time. This article has compared the studies carried in the Koyee Sizdah Aban and the other one that in carried in the district of Dowlat Abad, in regard to above-mentioned points. In this study the rate of knowledge, attitude, and the practice of four hundred of 15-49 years old women within the limits of healthy city, was compared with other four hundred women residing in the district of Dowlat Abad in regard to hygienic programs. According to the result of this study the mean percent of knowledge in the healthy cities women were significant in relation to other group. The rate of attitude toward the hygienic problems in the health city and the district of Dowlat Abad did not show a significant differences. The rate of practice of the women of healthy city with 95% of confidence was more than the women of Dowlat Abad. This study clarified that literacy of women and their spouse have positive effect on their knowledge and practice in regard to hygienic problems, but there is not meaningful relation between profession and age of women in regard to the knowledge and practice.

Abstract
Backgroud: Measles, as one of the most contagious diseases, because of the risks of mortality, mental and physical disabilities which may afflict on the patients, has always been so important to the scientists and researchers. With respect to burden of diseases, numerous measures have been taken to control and eliminate measles. In our country, vaccination is considered as the most important strategy to control the measles, however, the surveillance on the patients is on the second place. Screening (Active & passive), prompt and appropriate control of measles in high risk areas can help to control the disease.
Materials and Methods: There are 1375283 people with a variety of demographic traits who live in southern Tehran. Southern Tehran covering 10th, 11th, 16th, 17th, 19th divisions of Tehran municipality consists of high risk areas. Because of too many immigrants, overcrowded areas, malnutrition and poverty, the south of Tehran is considered as one of high risk areas, predisposed to measles epidemics. So in 1999, a cross-sectional study was done.
Results: On 230 subjects suspected to be measles in south of Tehran, 147 subjects were diagnosed as measles by para clinic evidence, 121 subjects of which were men. Most of these were of the age 20 to 24 years old who lived in the eleventh part of Tehran municipality area. The majority of patients were found among soldiers who lived in dormitories in Horr garrison and military training college located on the above-mentioned area. Unfortunately, most of them did not have reliable history of vaccination. Most of these patients had been in close contact with the colleagues who were measled (61.2 percent). The rate of measles incidence during winter soars at most.
Conclusion: The most important factor to be researched is that, the age of these patients differs much from what is expected, due to unknown reason.

Abstract
Maxillary sinus atelectasis is a rare and relatively unknown disease. Most patients present with non-sinonasel complaints. Negative intracavitary pressure due to ostial obstruction is suggested as the most probable etiology. Two patients with sinus atelectasis are presented in this paper: Case 1 presented with enophthalmus and maxillary sinus opacity and atelectasis in imaging. Endoscopic uncinectomy and midle antrostomy was done for this patient. Case 2 presented with mid-facial deformity and depression. Caldwel-Luc procedure with inferior meatal antrostomy was done. In 1 year follow up, they were asymptomatic and with no deterioration of facial deformity.

Abstract
Background: Cuffed pharyngeal tube is a device designed for ventilation of anesthetized patients. CPT has advantages over face mask including: maintaining of face mask can be difficult and boring after several minutes and mobility of the practitioner is reduced due to involvement of hands. Mask pressure can cause soft tissue and nerve damage around the nose. Anesthetic gas leakage from the mask adds to the operation room pollution. In difficult intubation CPT can be life-saving.
Materials and Methods: In our study CPT was compared with endotracheal tube (ET) in anesthetized patients. A scoring system for evaluating ventilation of patients was designed using symmetric chest wall motion during ventilation with anesthesia bag and sensing lung compliance through it, auscultation of breathing sounds, oscilation of bag with breathing and peripheral oxygen saturation by pulse oxymetry. Respiratory complications (pulmonary aspiration, Iaryngospasm and bronchospasm, nausea and vomiting) were looked for during anesthesia.
Results: The results showed that CPT was successful as ET in ventilation of spontaneously breathing patients and incidence of respiratory complications with CPT was no more than ET. Airway resistance was significantly greater with CPT than ET (P<0.05). Patients with ET had significantly greater incidence of sore throat than with CPT (P<0.05).
Conclusion: Thus we concluded that CPT can be used for ventilation of anesthetized patients not predisposed to pulmonary aspiration and whose peak airway pressure does not exceed 20-25 CmH2O.

Abstract
Introduction: Succinylcholine is a depolarizing muscle relaxant, which has been used extensively in anesthesia to produce muscle relaxation. The present study was done with the aim of comparing Fasciculation and postoperative myalgia after injection of two different doses of Succinylcholine (1.5 mg/kg and 3 mg/kg).
Materials and Methods: In this study which has been conducted in the form of Randomized double blind study in Tehran Shariati Hospital in the year 2000, 50 male patients in two groups (25 in each) with ASA I&II, age between 18 to 65, were candidate for an elective surgical repair of inguinal hernia received a standard anesthetic technique, including one of the two doses of Succinylcholine.
Results: After collecting data it has been considered that the average difference of two respective groups is meaningful in terms of fasciculation (P<0.05), but not in terms of myalgia (P>0.05).
Conclusion: we came to the conclusion that, firstly, the percentage of myalgia after the injection of succinylcholine is totally low and with the increase of the dose of medication from 1.5 mg/kg to 3 mg/kg the very percentage does not change considerably. Secondly, regarding fasciculation the increase of the dose of medication from 1.5 mg/kg to 3 mg/kg causes the decrease of the average limit of fasciculation. This decrease is very obvious in server fasciculation. As a matter of fact no relationship was found between decrease in fasciculation and change in myalgia.

Abstract
Background: In this study, the results of "Urethral Advancement & Glanuloplasty" in repairing anterior hypospadias were considered. Hypospadias is a common urethral anomaly in boys. More than 65 percent of hypospadias cases are anterior (glanular, coronal and distal penil shaft). More than two hundreds original techniques have been applied to correct hypospadias. Each of these techniques has some complications. The most common complications of hypospadias repairing are fistula and meatal stenosis.
Materials and Methods: A total of 74 boys with anterior hypospadias underwent the procedure of "Urethral Advancement and Glanuloplasty" (UAGP) in our medical centers from March 1994 to April 1999. The procedure included: degloving, correction of chordee, urethral mobilization and glans plasty. Cosmetic results were excellent in most patients. There was no fistula, and also meatal stenosis was not observed after applying V-Flap of the glans.
Results & Conclusions: In a 1 - 6 year follow - up (mean 3.15 years, SD ± 1.79) the results functionally and cosmetically, were satisfactory in all cases with no long-tem complication or chordee.

Abstract
Unstable angina (UA) is situated in a spectrum with myocardial infarction (MI) at one end of it and stable angina at the other end. To determine the clinical and paraclinical characteristics of unstable angina this study was designed.
Materials and Methods: Two hundred patients with definite UA who were hospitalized at cardiac care units (CCU) in Yazd were enrolled in this cross- sectional study. Clinical and para-clinical characteristics including class of severity of angina, clinical circumstances, drugs, risk factors, changes in ECG, and patient's condition at the time of leaving hospital were evaluated.
Results& Conclusions: The average of age of patients was 61.85 years and 57% of them were female and 43% were male. The most prevalent risk factor among men was smoking (48.8%) and among women was hypertension (62.8%). According to Braunwald suggestion, patients were divided into 3 classes based on the severity of their diseases. Frequency distribution of patients in classes I,H and III were 24.5%, 4% and 71.5%, respectively. Normal ECG, was observed in 24.5% of subjects at the time of hospitalization and in 67% of patients when leaving hospitals. Frequency distribution based on the risk stratification was 22.5%, 58.5% and 19% in the low, medium and high risk groups, respectively. During hospitalization, acute myocardial infarction (AMI) was seen in 3.5% of individuals that majority of them belonged to class III of angina.

Abstract
Obstructive cholestasis is associated with overproduction of endogenous opioids (EOP), nitric oxide (NO), and cytokins in the blood streams. Therefore we investigated the relationship between obstructive cholestasis and function of germ cells in adult male rats.
Material and Methods: To study this, we used three groups of animals: No-surgery, Sham-surgery, and surgical ligation of the bile duct. After 3 weeks all animal were killed by ether, serum concentrations of FSH, LH and testosterone were determined by Radioimmunoassay, apoptosis was evaluated by DNA fragmentation detected by in situ terminal deoxynucloetidyl Transfrase-mediated dUTP nike end labeling (TUNEL).
Results: The mean of FSH level in cholestatic, control and sham groups were 13.22+ 1.038, 18.14+ 1.276, and 16.92+ 1.072 ng/ml, respectively. The mean of LH level in cholestatic, control and sham groups were 0.83 + 0.21, 2.058 ± 0.26, and 1.84 + 0.17 ng/ml, respectively. In addition, the mean of testosterone level in cholestatic, control and sham groups were 1.52 ± 0.16, 2.41 ± 0.18, and 2.31 + 0.14 ng/ml, respectively. The results of this study were indicated that serum FSH, LH and testosterone were significantly lower in cholestatic than control and sham groups (p=0.0195, P= 0.0029, and P=0.0023, respectively). However there was no significant difference in apoptotic index between all of groups (P=0.195). The apoptotic index in cholestatic, control and sham rats were 9.897± 1.374, 7.086 + 0.91, and 7.729 + 1.101, respectively.
Conclusion: These findings have been shown which as obstructive cholestasis was decreased the levels of serum gonadotropins and testosterone but it has no significant effector testicular germinal cells apoptosis.

Abstract
Background: The results of studies about relationship between depression and positive signs in schizophrenia are controversial and clarifying the nature of this association may be difficult. The aim of present study is to investigate relationship between depressive symptoms and positive signs, in acute phase of patients with chronic schizophrenia, who have been admitted in Roozbeh Hospital.
Materials and Methods: Assessments were performed using the Beck depression Inventory for depression (subjectively) and positive and negative syndrome scale (PANSS) for psychotic symptoms.
Results: The results demonstrated a significant correlation between depressive symptoms and positive signs in these patients. Also, a significant correlation existed between depression and these items: delusions, hallucinatory behavior, excitement, hostility.
Conclusion: depressive symptoms and positive signs in schizophrenia may have a common underlying pathophysiological origin.

Abstract
Background: The damaging effect of exposure to loud noise on the structures of internal ear has been studied from many years ago. The effect of drill-generated noise on the non-operated ear is one of the problems that are less discussed. This study aims to evaluate the hearing loss that occurs in the non-operated ear in patients who undergo mastoidectomy by use of PTA (pure tone audiometery) and DPOAE (distortion product otoacoustic emission).
Materials and Methods: In this descriptive- analytic study, 49 patients who underwent mastoidectomy in Amir-Alam and Iman Khomeini hospitals, were evaluated for noise trauma generated by drilling on the non-operated ear. Patients were between 9-55 years old (mean age= 29) and they consisted of 25 males and 24 females. The patient's preoperative state were measured by PTA and DPOAE on the opposite ear also the postoperative state were re-evaluated by PTA and DPOAE.
Results: As a result, in 4 cases a significant change in PTA thresholds were observed, while two of them showed recovery after one month. Seven cases showed significant change in DPOAE amplitudes, of whom five cases recovered later.
Conclusion: In conclusion, the drill-induced hearing loss on the non-operated ear occurs in a few patients and it is often reversible.

Abstract
Background: Regarding the complications of chronic intractable epilepsy, the presence of respectable lesions in many these patients that can be diagnosed with noninvasive sensitive techniques such as MRI and SPECT and the unrecognized significance of epilepsy surgery in our country, we have decided to review the management of medically intractable epilepsy in patients, who underwent epilepsy surgery in neurosurgery department of Loghman Hakim hospital between 1997-2003.
Materials and Methods: In this study we retrospectively review 30 cases of medically intractable epilepsy that had underwent epilepsy surgery. All patients before surgery were investigated with brain MRI, brain SPECT, EEG and IQ test. Type of surgery was determined by MRI, SPECT and EEG findings. Pre - and postoperative seizure frequency and surgery complications studied. Seizure control was measured with Engel criteria.
Results: Patients mean age was 22.4 years. Three cases (10%) were females that all underwent temporal mesial lobectomy. In 18 cases (60%) there were concordant brain lesion with seizure origin that 9 cases (30%) underwent mesial temporal lobectomy and remainder 9 cases (30%) underwent lesionectomy.other12 cases (40%) that have uncertain brain lesion but suffer from drop attack due to one or combination of atonic, tonic, tonic clonic, clonic, myoclonic, absence or clonic underwent anterior callosotomy. patients that underwent mesial temporal lobectomy, anterior callosotomy and lesionectomy were seizure-free in 77.7%, 58.3% and 55.5% of cases respectively.
Conclusions: Provided to correct patient selection for epilepsy surgery we can manage intractable epilepsy properly. Regarding to the complication of intractable epilepsy, acceptable epilepsy surgery results and available sensitive noninvasive diagnostic techniques such as MRI in our country, epilepsy surgery should be considered seriously in our country and promoted.
