Normal 0 false false false EN-US X-NONE AR-SA MicrosoftInternetExplorer4 Background: Preterm delivery is a relevant public health problem since it is an important factor determinant of morbidity and the main reason for neonatal mortality. Many publications have pointed at clinical methods, biological, biochemical and ultrasonographic markers, which applied in combination or on their own, aim at predicting spontaneous preterm delivery. Transvaginal sonography is an effective method for the study of uterine cervix during pregnancy since it permits to assess the cervical morphology and biometry in detail with a high degree of reliability. The aim of this study was to evaluate the association between spontaneous preterm delivery (SPTD) before 35 and 37 weeks of gestational age, in high risk population and the measurement of the cervix length and cervical funneling.

Methods: A prospective cohort of 200 women carrying high risk pregnancies were evaluated by transvaginal sonography between 14th and 28th weeks of gestation.
Results: Cervical length less than 18mm before 35th and 37th weeks of gestational age and the presence of cervical funneling presented a satistically significant association with spontaneous preterm delivery before 35 weeks. The cervical length with less 18 mm demonstrated a strong association with SPTD before 37 weeks (p<0.005 OR=92.15) and before 35 weeks' gestation (p<0.001 OR=32.33). The logistic regression analysis suggested the cervical length with less than 18mm as the only variable that revealed satistically significance association with SPTD.
Conclusion: The results seem to indicate that the assessment of cervical length is an important ultrasound marker for predicting spontaneous preterm delivery.

Abstract

Background: Preterm delivery is a relevant public health problem. The aim of this study was to evaluate the association between spontaneous preterm delivery (SPTD) before 35 and 37 weeks of gestational age and the measurement of the cervix length, cervical funneling and Cervical Gland Area (CGA), in high risk pregnant population.
Methods: A prospective cohort of 200 women carrying high risk pregnancies was evaluated by transvaginal sonography between 14th and 28th gestational weeks. The data were analyzed using statistical methods. A multiple linear regression model was estimated in order to examine the relationship between the gestational age at delivery and the cervical markers. A multiple logistic regression was estimated in order to analyze the factors associated to spontaneous preterm delivery and the transvaginal sonographic markers.
Results: Cervical length less than 18 mm and the presence of cervical funneling presented a statistically significant association with spontaneous preterm delivery before 35 weeks. The nondetection of Cervical Gland Area demonstrated a strong association with spontaneous preterm delivery before (p=0.0001, OR=169.1, CI=2.6-3.1) and 35th and 37th gestational week (p=0.001, OR=115, CI=2.12-3.5). The multiple logistic regression analysis suggested the non-detection of CGA as the only variable to reveal statistically significance association with spontaneous preterm delivery.
Conclusion: Based on results of present study the absence of cervical gland area (CGA) can be a new and important ultrasound marker for predicting spontaneous preterm delivery and needs to confirm with future multicenter investigations.

Abstract

Background: Bronchiolitis is one of the most common diseases of the lower respiratory tract in infants. Vitamin D has been shown to be protective against lower respiratory infections; however, there are limited and contradictory results in relation to serum vitamin D level and the incidence of bronchiolitis in children. The aim of this study was to compare serum levels of this vitamin in healthy infants and infants with acute bronchiolitis.
Methods: This case-control study conducted at Abuzar hospital in Ahvaz city, during October to March of 2014. Three groups of 45 eligible infants including control, low and high intensity of bronchiolitis enrolled to study. The severity of bronchiolitis classified according to scores derived from the respiratory distress assessment index. One to eight scores considered as low intensity and 9 to 17 scores considered as high intensity of bronchiolitis. Subsequently, 3 ml of venous blood sample were taken from them and the serum levels of 25(OH)D were measured by using an enzymatic kit.
Results: 60 (44.4%) infants were girls. The mean age of the control, low and high intensity of bronchiolitis groups were 11.2±5.2, 10±5.8 and 9.8±4.7 months, respectively (P=0.1). The mean of 25(OH)D concentrations in the control, low and high intensity bronchiolitis groups were 28.3±19.4, 17.7±11.7 and 13.6±5.7 nm/l, respectively. There was a significant difference in levels of 25(OH)D between the control-low intensity bronchiolitis groups (P=0.001) and the control-high intensity bronchiolitis groups (P=0.002), this difference was not significant between the two groups of bronchiolitis. There was a direct and significant correlation between serum level of 25(OH)D and age (r=0.2, P=0.005), breast milk consumption (r=0.3, P=0.001), and vitamin D supplementation (r=0.6, P=0.000).
Conclusion: In the present study, levels of 25(OH)D were significantly lower in infants with bronchiolitis than control group. In addition, 25(OH)D levels did not affect the severity of bronchiolitis.

Abstract

Background: Polycystic ovary syndrome (PCOS) is the most common endocrine disorder in reproductive age women. The prevalence of PCOS among Iranian women is 14.6% based on the Rotterdam definition. PCOS can increase the risk of chronic metabolic complications. Metabolic syndrome (MetS) is one of the most important of them. Insulin resistance is fundamental in PCOS and MetS pathophysiology. MetS is a collection of chronic metabolic derangements, which promotes the risk of serious diseases such as cardiovascular disturbances and diabetes. The quality of diet is inversely correlated with obesity, which is one of the consequences of PCOS and it is a risk factor for metabolic syndrome. As the first study, the present study aimed to compare the dietary intake of macronutrients in PCOS women with and without MetS.
Methods: In this nested case-control study, the convenience sampling method was used to select participants. The case group included 14 PCOS patients with MetS and the control group included 28 PCOS patients without MetS. The investigated macronutrients included fats (total fat, saturated fatty acids, mono and polyunsaturated fats, trans fatty acids), carbohydrate, protein, and fibers (total and soluble). The dietary intake assessment was carried out by a 168-item semi-quantitative food frequency questionnaire (FFQ). This study was performed on Arash Women's Hospital under support of Tarbiat Modares University, Tehran, Iran, from August 2014 to September 2015.
Results: Dietary intake of total fat was significantly higher in PCOS women with MetS than the control group (P<0.001, 146.62±45.17 vs. 59.91±32.79 g/d). Dietary intake of saturated fatty acids (P=0.004, 11.44±11.08 vs. 19.73±6.76 g/d), monounsaturated fats (P<0.001, 14.15±12.66 vs. 26.16±7.76 g/d), polyunsaturated fats (P=0.002, 9.02±9.18 vs. 17.22±6.45 g/d), carbohydrate (P<0.001, 182.34±121.08 vs. 365.64±77.11 g/d), protein (P=0.001, 42.74±42.85 vs. 78.06±24.04 g/d), total fiber (P<0.001, 17.77±15.09 vs. 35.97±22.64 g/d) was significantly lower in PCOS women with MetS than the control group. All results are expressed in terms of grams consumed per day.
Conclusion: Our results demonstrated that dietary intake of macronutrients was significantly different in polycystic ovary syndrome patients with and without metabolic syndrome.

Abstract

Background: With the rapid advancement of minimally invasive surgical techniques, laparoscopic cholecystectomy has become one of the most frequently performed procedures worldwide. Despite its benefits, such as reduced postoperative complications and faster recovery, effective pain management remains a critical challenge. Postoperative pain can significantly impact patient recovery, hospital stay duration, and overall satisfaction. Given the limitations and side effects of opioid-based analgesics, there is growing interest in non-opioid alternatives for pain control following laparoscopic cholecystectomy. This narrative review evaluates the efficacy of various non-opioid analgesics in managing postoperative pain in patients undergoing this procedure.
Methods: A comprehensive literature search was conducted across multiple electronic databases, including Scopus, PubMed, Science Direct, SID, Magiran, and Google Scholar, using both Persian and English keywords. English equivalents were "Pain" and "Laparoscopic Cholecystectomy." Studies discussing pharmacological interventions for postoperative pain management in laparoscopic cholecystectomy were included.
Results: The review identified a wide range of non-opioid analgesics used for pain control in laparoscopic cholecystectomy patients. These included: Systemic medications (intravenous/oral) were magnesium sulfate, ketamine, ketorolac, paracetamol, ibuprofen, pregabalin, gabapentin, acetazolamide, N-acetylcysteine, dexamethasone, clonidine, celecoxib, and diclofenac. Local anesthetics & adjuvants were used intraperitoneally like bupivacaine, lidocaine, and hydrocortisone. Administration routes varied, with most drugs given intravenously, intraperitoneally, or preemptively (e.g., celecoxib and clonidine before surgery). Some studies highlighted the benefits of multimodal analgesia, combining different drug classes to enhance pain relief while minimizing side effects.
Conclusion: Despite the availability of numerous non-opioid analgesics, no single drug has been universally endorsed as the gold standard for postoperative pain management in laparoscopic cholecystectomy. Current evidence suggests that the choice of analgesic should be individualized, considering patient factors (e.g., comorbidities, pain tolerance) and the anesthesiologist’s expertise. Multimodal approaches, combining different drug classes, may offer superior pain control with fewer adverse effects compared to monotherapy. Further high-quality randomized trials are needed to establish standardized protocols and optimize pain management strategies for these patients.
