Abstract

Background: Surgery and accurate removal of the brain tumor in the operating room and after opening the scalp is one of the major challenges for neurosurgeons due to the removal of skull pressure and displacement and deformation of the brain tissue. This displacement of the brain changes the location of the tumor relative to the MR image taken preoperatively.
Methods: This study, which is done from March to December 2019 in Tehran, is evaluated on the available database of RetroSpective Evaluation of Cerebral Tumors (RESECT) including pre-operative MR images, and intra-operative ultrasound from 22 patients with low-grade gliomas who underwent surgeries at St. Olavs University Hospital. This study is used for image registration of preoperative MR imaging and ultrasound imaging after resection of the skull to compensate for brain changes. By this method, we obtained a third image that resembles preoperative MR imaging but has the geometry of the brain shape changes. We used a combination of the two transformations named Affine and non-rigid Free Form Deformation (FFD) for hierarchically moving the pixels to compensate for global variations, and also nonlinear local and small variations. Also, by applying the mutual information function, we consider the entropy value as the criterion of similarity due to the non-similarity of the nature of the images. Also, Limited Broyden-Fletcher-Goldfarb-Shannon method is used for optimization.
Results: The results of the proposed method were presented on the available database of RetroSpective Evaluation of Cerebral Tumors (RESECT) including images of 22 patients with glioma type 2 tumors and evaluated based on 15 landmarks per patient and also mutual information criteria. The mean target registration error for affine, FFD and the proposed method are 46.19, 42.85 and 38.01, respectively. It was shown that the proposed method achieved high accuracy by combining the two transformations of affine and FFD compared to the separate use of each of the two models.
Conclusion: In image registration of preoperative MR and ultrasound images for compensation of brain shift, the combination of affine and FFD transformations had better results than the individual use of each of the transformations.

Abstract

  SARS-CoV-2 emerging from Wuhan, China is a member of the Coronaviridae family, which has so far infected and killed many people. The SARS-CoV-2 pandemic affected various aspects of life in Iran and Worldwide, and governments have imposed quarantines and travel bans on an unprecedented scale. The virus causes COVID-19, which can spread through close contact with the infected person, contaminated equipment, and suspended air droplets. The most common symptoms of the disease include fever, cough, shortness of breath, gastrointestinal symptoms, and diarrhea. In severe cases, the lung infection can occur, which causes Severe Acute Respiratory Syndrome that leads to ICU admission and even death.
  Besides, this infection can cause gastrointestinal, neurological, and renal impairments. Not merely, this new coronavirus has infected many more people worldwide in comparison to MERS and SARS, but also it has killed more people. Patients with underlying diseases such as hypertension, diabetes, respiratory problems, kidney disease, heart disease and Immunodeficiency are at higher risk of infection and potential death. Also, the risk of death and complication increases in older adults, while most of the infected children are asymptomatic. Some infected people may have mild or no symptoms but can still transmit the disease and spread it to others.
To diagnose COVID-19, serology tests, and level of ESR, CRP and other acute-phase reactants are helpful, whereas molecular tests, such as RT-PCR tests, that detect the virus’s genetic material are still the golden standard. Also, CT scan detects lung involvement; Ground-glass opacification, especially in lower lobes and subpleural region, is the most common CT characteristic, although it is not specific for COVID-19. Because the disease is difficult to diagnose, hard to prevent and challenging to treat, it has become a major concern for many countries. This review aims to gather existing information in the fields of virology, molecular pathogenesis, disease symptoms, epidemiology, clinical presentations, diagnosis, treatment, and the spread of the disease. This study also provides evidence-based prevention and treatment strategies for health policymakers, doctors, nurses, and practitioners in the field of public health, including researchers and students.

Abstract

Background: Breast cancer is the most common cancer in women and its treatment includes various surgeries. Breast-Q is a new patient-reported outcome instrument for breast surgery and it should be validated appropriately for clinical research. This study aimed to develop the Persian version of the Breast-Q and validate the reconstruction module to evaluate the quality of life and satisfaction of Iranian women with a previous history of breast reconstruction.
Methods: This study was descriptive and analytical cross-sectional. The study population consisted of women with a previous history of breast cancer and various breast surgeries referred to the outpatient clinic of Cancer Institute, Imam Khomeini Hospital from September 2017 to October 2019, using convenience sampling. After obtaining permission from the MAPI institute and translating all questionnaires into Persian, the validity, and reliability of the breast reconstruction module were evaluated. To confirm face validity, a questionnaire was given to patients to judge each item. To confirm the content validity of the questionnaire, two qualitative and quantitative methods were used. Content validity ratio (CVR) was calculated based on the expert opinion (Lawshe formula). Waltz & Bausell’s method was used to examine the content validity index (CVI). Finally, a new version of the questionnaire was assessed in 20 women who had undergone TRAM flap breast reconstruction at least 6 months ago.
Results: After translation of the questionnaire into Persian, and face and content validation process, the 116 items of the first questionnaire converted to 72 with an acceptable impact score greater than 1.5 and CVR and CVI. The internal validity of the breast reconstruction module was approved by Cronbach's alpha value of 0.94.
Conclusion: Considering the limitation of the specific questionnaires for various breast surgery procedures, the Breast-Q questionnaire can be an efficient tool for the evaluation of the quality of life and satisfaction in patients. Since the translation of the questionnaire has become valid and reliable, it can be used for future research by other researchers.

Abstract

Background: Cervical cavernous hemangioma is an extremely rare, benign lesions, and only a few cases are reported. Cervical cavernous Hemangiomas are characterized by an increase in the number of normal and abnormal veins. The majority of these lesions are superficial, often found in the head and neck area, but may also occur in the inner parts of the body. The occurrence of these lesions in the female reproductive tract, especially the the cervix is extremely rare. This disease should be considered as a differential diagnosis of abnormal uterine bleeding because of the low prevalence and importance of the diagnosis of this uncommon disease. The majority of cervical cavernous hemangiomas have been reported in women of childbearing age. Besides, although most lesions are symptomatic (mostly bleeding), diagnosis is often unlikely. They may cause abnormal vaginal bleeding in the form of menometrorrhagia and postcoital spotting. To date, fewer than 55 cases have been reported. This study aimed to report a rare case of cervical cavernous hemangioma.
Case Report: The patient was a 24-year-old woman who had referred to the outpatient clinic of Ghaem Hospital in September 2016 due to prolonged post coital bleeding after intercourse. At the time of the pelvic examination, posterior lip of cervix was very vascular and there was black color that had extend to the vagina. The patient underwent colposcopy. Following colposcopy, due to severe cervical bleeding, the vagina packed. The patient's bleeding was controlled with the pack after 24 hours. Cervical pathology revealed a cavernous hemangioma.
Conclusion: Cervical cavernous hemangioma is extremely rare. This should be considered in the differential diagnosis of patients with abnormal vaginal bleeding, especially those of childbearing age, and no clinical or radiologic findings such as leiomyoma, adenomyosis, or dysfunctional uterine bleeding. Pelvic examination is the first step in the management of the patient with vaginal bleeding. Increased awareness of this unusual cervical lesion can lead to early diagnosis and conservative treatment approaches. In most cases, hysterectomy is performed, but the patient with mild symptoms can be treated with conservative management. Increased awareness of this unusual cervical lesion can lead to early diagnosis and conservative treatment approaches.

Abstract

Background: Lymphoma is a malignant proliferation of lymphoid cells. External T-cell lymphoma is very rare. Lymphomas are neoplastic growth of lymphocytes caused by organisms like EBV, HIV, etc. This malignancy is classified as Hodgkin’s lymphoma and non-Hodgkin’s lymphoma. Oral lymphomas account for a small percentage of head and neck malignancies, and intraoral lymphoma is rare and mostly seen in Waldeyer's ring of immunocompromised patients.
Case presentation: A 67-year-old female was referred with a palatal wound that developed within two months. In medical history, she had a peptic ulcer since last year, and intestine lymphoma was diagnosed 4 months later. After that, her gastrointestinal lymphoma was treated with surgery and chemotherapy for 6 sessions. The oral examination revealed an endophytic ulcer with a granular surface in the left posterior part of the hard and soft palate and maxillary alveolar ridge. The lesion was under biopsy with clinical differential diagnosis of squamous cell carcinoma or metastatic lymphoma or deep fungal infection. Microscopic examination with hematoxylin and eosin (H&E) staining revealed malignant mononucleated lymphocytes and hyperchromatic, polymorphic, dysplastic cells. Also, acinic cells and muscular cells were destructed. Moreover, immunohistochemistry (IHC) analysis was positive for CD3, showing the presence of T lymphocytes. Therefore, diagnosis of metastasis of intestinal T-cell lymphoma to oral cavity was established. Because the patient had a history of intestinal lymphoma, she was referred to the oncologist for treatment. In further examinations, lung and liver metastasis were detected and the patient died on the second session of chemotherapy.
Conclusion: Malignant metastases to the oral cavity can occur in various forms, including swelling and ulcer, periodontal lesion and tooth luxation. Since, metastatic lesions in the oral cavity, could imitate the feature of inflammatory lesions, a thorough examination and taking history can be helpful in the early diagnosis and appropriate treatment planning to increase patients’ survival.

Abstract

Background: Systemic lupus erythematosus is a systemic autoimmune disease that affects almost all organs of the body, and viral infections are involved in its development and progression. The present study aimed to evaluate the serological status of some viral infections in patients with systemic lupus erythematosus and a healthy population.
Methods: This descriptive study conducted from May 2017 to April 2018 at Shiraz HIV/AIDS Research Center, Shiraz University of Medical Sciences, Shiraz, Iran on 70 patients with systemic lupus erythematosus and 70 healthy individuals who had no autoimmune diseases and were matched with the patient group for age and sex. All patients had active records and were routinely visited in rheumatology clinic of Hafez hospital, affiliated with Shiraz University of Medical Sciences. The evidence of active disease was assessed by the physicians of this practice according to the American College of Rheumatology criteria. Peripheral blood samples were collected in tubes containing EDTA and centrifuged at 3000 rpm for 5 min. The plasma of study participants was evaluated for HBsAg, HCVAb, HIVAb, EBV-VCA-IgG, and CMV-IgG using a commercially available ELISA kit.
Results: The seropositivity of CMV-IgG and EBV-VCA-IgG in the systemic lupus erythematosus group was 70 (100%) and 65 (92.9%), and in healthy individuals was 68 (97.1%) and 57 (81.4%), respectively. The prevalence of EBV-VCA-IgG in the systemic lupus erythematosus group was significantly higher than healthy ones (P=0.043). The optical density (OD) of CMV-IgG and EBV-VCA-IgG in patients with systemic lupus erythematosus was significantly higher than in healthy individuals (P<0.0001). All patients with systemic lupus erythematosus were negative for HBsAg and HIVAb, but HCVAb was detected in 1 (1.4%) patient.
Conclusion: Considering the higher frequency of EBV-VCA-IgG and the higher titer of antibodies against CMV and EBV in patient groups compared to healthy individuals group, it seems that periodical assessment of viral load in patients with systemic lupus erythematosus will be beneficial to prescribe medication by physicians if it is needed.

Abstract

Background: Diabetic foot infection is the most common complication of diabetic foot ulcer (DFU) leading to amputation. The treatment requires special attention to disease conditions, proper diagnosis, appropriate sampling for cultures, careful selection of antibiotics, rapid determination of the patient’s need for surgical intervention, and wound care. Clearly a systematic approach or, if possible, an evidence-based approach by using clinical guidelines about diabetic foot infections will lead to better outcomes. In this regard, this article aims to adopt the IWGDF clinical practice guideline on DFU infection.
Methods: In this study, the adoption of the international evidence-based clinical guidelines on the DFU infection was carried out using the ADAPTE methodology, which involved three stages of setup, adoption, and finalization. AGREE II tool was used to evaluate the quality of the selected clinical guideline.
Results: The results of this adopted clinical guideline according to the national facilities and access to equipment are described with 26 recommendations along with related reasons for the diagnosis and treatment of diabetic foot ulcer infections. The recommendation categorized into six different categories including Classification/ Diagnosis, Osteomyelitis, Assessing severity, Microbiological considerations, Surgical treatment, Antimicrobial therapy.
Conclusion: The mentioned recommendations in the diagnosis and treatment of diabetic foot ulcer infections extracted based on the methodology of systematic review studies. Actually the opinion of experts have been prepared and adjusted according to the limitations, equipment and facilities in the country. But in general, there are some challenges in diabetic foot infections should be considered as well. So monitoring the treatment of infection, optimal duration of antibiotic treatment, optimal treatment of infection in low-income countries, time and type of imaging studies, selection of medical or surgical treatments and hospitalization conditions are some of these challenges. However the definition and practical clinical application for the concept of bacterial bioburden of the wound for wound healing, evaluating the cost-effectiveness of new technologies in improving treatment and appropriate interpretation of microbiological (genotypic) microbiological tests are important too.

Abstract

Background: Ewing's sarcoma/Primitive neuroectodermal tumor (PNET) is a group of tumors with small round cells that originate from nerve stem cells. They are generally more common in children and often occur in the soft or bony tissues of the limbs, trunk, head, and neck. Ewing's sarcoma is a rare disease in the kidney and its tumor thrombosis into Inferior Vena Cava (IVC) is assumed as a very rare condition.
Case Presentation: The patient was a 14-year-old boy who underwent an MRI of the thoracic and lumbar vertebrae due to paresthesia of the lower limbs, which showed the presence of the mass in the vertebrae of T3, T4, T5, and concurrently right kidney. The pathology report confirmed the diagnosis of Ewing’s sarcoma, PNET. The patient underwent T4 laminectomy and bone mass resection and then received 4 courses of chemotherapy with VAC + IE. In the next stage of treatment, the patient had abdominal MRI and MR Venography (MRV) that demonstrated a mass in the middle of the upper right kidney with a 10 cm length tumor thrombosis into IVC with extension to the suprahepatic area and involvement of several aortocaval lymph nodes. In January 2020, the patient was operated in Imam Khomeini Hospital Complex, Tehran by a midline incision, at first, the kidney artery and vein were controlled, and without thrombectomy, the IVC was controlled from above and below the liver, then the tumor thrombosis was removed. The right kidney underwent a radical nephrectomy. Three days later, the patient was discharged in good general condition and referred for chemotherapy.
Conclusion: Because Ewing's sarcoma is a rare condition in the kidney, this case is considered a very rare case due to its tumor thrombosis. This case study showed that despite the advanced stage of the disease, early diagnosis and treatment of patients with Ewing's sarcoma, along with adjuvant treatments can play an important role in the survival of these patients.

Abstract

Background: Degenerative scoliosis most commonly affects the lumbar spine in the elderly, resulting in facet and disc degeneration, leading to increased pain and progressive deformity. Due to the importance of the results of long fusion and the rate of coronary and sagittal correction of imbalance in patients with degenerative scoliosis, the present study was performed to evaluate the results of long fusion surgery in patients with degenerative lumbar scoliosis.
Methods: The present study was performed as a prospective cohort study in patients with degenerative scoliosis who were candidates for surgery at Al-Zahra Hospital in Isfahan. Basic patient information including age, sex, anatomical location of vertebral deviation and slip, degree of severity of deviation based on cobb angle and number of previous surgeries were reviewed and recorded. The surgical characteristics of the patient were extracted from the medical record including the approach used, the anatomical location of the fusion, whether or not to perform decompression, the number of decompression and fusion levels, the amount of bleeding during surgery, and the duration of surgery.
Results: Eleven female patients participated in this study. The mean age of patients was 55.64 years with a standard deviation of 7.67 years. The minimum age was 40 years and the maximum age was 66 years. Patients' symptoms included low back pain-leg pain (3 cases), back pain-lameness (2 cases) and back pain-leg pain-lameness (6 cases). All patients underwent two stages of surgery. Mean amount of bleeding and time of surgery in the second surgery were significantly reduced compared to the first surgery (p <0.05). Complications of surgery included proximal junctional vertebral fracture (PGK) in two patients.
Conclusion: Degenerative scoliosis is a complex clinical condition in which the patient's main problems are pain and disability. Long fusion surgery in patients with degenerative scoliosis significantly reduced the mean deviation, Patient’s pain and severity of disability 6 months after surgery.

Abstract

Background: The goal of the study was to investigate the effect of 5 weeks of high-intensity interval training (HIIT) on the levels of resistin and adiponectin and liver enzymes in Wistar rats with non-alcoholic fatty liver.
Methods: To do this basic research, 24 Wistar male rats were purchased and transferred to Baqiyatallah University then, they were allocated randomly into three groups: Control group (CG, n=8), induced fatty liver diseases group (FG, n=8), and induced fatty liver diseases+exercise group (FEG, n=10). The present study was conducted in July and August of 2019. To induce fatty liver disease, 140 mg per kg of bodyweight oral tetracycline drug was administered to rats by gavage for seven days. Fatty liver (steatosis) was confirmed by measuring liver enzymes. The training group performed HIIT for five weeks and five days per week. First, the maximum aerobic test (MAT) was performed, and based on data of this test; the HIIT protocol was conducted as 5×2 min-intervals with (50-120% Mat) and with 1 min recovery (30-50% MAT). Blood and tissue sampling were taken 48 hours after the last training session. Blood samples were centrifuged at 4000 rpm for 10 minutes, and serum samples were frozen at -20 degrees. One-way ANOVA and Tukey post hoc test were used to analyze data.
Results: Following performing HIIT, serum levels of adiponectin and resistin in the FEG compared to FG increased and decreased significantly, respectively (P=0.001). In addition, the FEG experienced a significant decrease in serum levels of AST and ALT enzymes. The aerobic capacity of the rats in the HIIT group increased significantly compared to the other two groups.
Conclusion: By modulating body fat levels and the secretion of adipokines such as adiponectin and resistin, HIIT was involved in improving the condition of rats with non-alcoholic fatty liver disease.

Abstract

Background: Posturography is a method in which the postural stability of adults is evaluated by measuring the center of pressure sway. This study aims to evaluate the reproducibility of measuring the center of pressure oscillation in standing position with internal perturbation in healthy young women with and without hyperkyphosis.
Methods: Ten women with dorsal hyperkyphosis with the mean age of 23.5±2.65 years and 10 healthy women with the mean age of 21.9±1.3 years (October 2018 to February 2019) were recruited for the study. Subjects were asked to perform rapid bilateral arm elevation while standing on a force plate. Standard deviation of the amplitude and The standard deviation of the velocity of the center of pressure sway in anterior-posterior, Medio-lateral directions and the area of sway were used for statistical analysis. Measurements were carried out by one examiner with a 1-day interval in The Biomechanics laboratory of The School of Rehabilitation Sciences, Shahid Beheshti University of Medical Sciences. The relative reproducibility of the measurements was calculated by Intra-class Correlation Coefficient (ICC), standard error of measurement (SEM) and minimal detectable changes (MDC).
Results: The intra-rater reliability of standard deviation of the center of pressure sway velocity and amplitude of both directions in both groups were more than 0.75. The intra-rater reliability of the area of the center of pressure sway in the healthy and hyperkyphosis group were 0.42 and 0.38 respectively.
Conclusion: Standard deviation of the amplitude and standard deviation of the velocity of the center of pressure sway can be considered as reliable variables for assessing static balance in young women with and without dorsal hyperkyphosis in future studies. However, the inconsistency of sway area especially in women with hyperkyphosis suggests that the use of it for differentiation between subjects and the assessment of the outcome of any interventions on the postural stability should be considered with caution.

Abstract

Background: Low-Molecular-Weight Heparin (LMWH) is recommended as the first-line treatment in patients with active cancer and venous thromboembolism (VTE), but many patients prefer to take oral anticoagulants and non-injectable forms with more reasonable price. Venous thromboembolism is a very common comorbidity in patients with cancer. Therefore, the aim of this study was to evaluate the efficacy and safety of the rivaroxaban compared with enoxaparin in patients with cancer and VTE.
Methods: This randomized clinical trial was conducted on 50 patients with non-hematologic cancer and deep vein thrombosis (DVP) or pulmonary thromboembolism (PTE) enrolled into Imam Khomeini hospital, from November 2019 to March 2020 in Ahvaz. The participants randomly assigned in two treatment groups (25 patients in each group) of rivaroxaban (15 mg every 12 hours for the first three weeks and then orally at 20 mg daily) or enoxaparin (1 mg/kg by subcutaneous injection every 12 hours) and followed for 6 months to evaluate the efficacy, complications and safety (incidence of recurrent VTE, major bleeding and deaths) of these therapies in Ahvaz.
Results: The three most common cancer diagnoses were breast (n=11, 22%), colon (n=10, 20%), and lung (n=7, 14%). Major bleeding at 6 months was only seen in one patient (4%) in the enoxaparin group and did not occur in the rivaroxaban group (P>0.05). Minor bleeding occurred in 1 patient (4%) in the rivaroxaban group and did not occur in the enoxaparin group (P>0.05). One patient in the enoxaparin group died because of fever and neutropenia. The prevalence of DVT and PTE in cancer patients was not significantly different based on patient age (P=0.154), gender (P=0.430), BMI (P=0.490), underlying disease (P=0.294), smoking (P=0.955), type of cancer (P=0.527), and metastatic cancer (P=0.280).
Conclusion: The results of this study suggest that the efficacy of rivaroxaban is not less than that of enoxaparin and therefore can be a potential option for patients with non-hematologic cancer and VTE. However, further randomized, controlled trials are needed to confirm these results.

Abstract

Background: Long-term and intensive physical exercise can change the function of different cells in the immune system in athletes, predisposing them to viral infections such as coronavirus disease (COVID-19). The purpose of this brief report was to provide protocols related to the immune system in athletes to prevent infectious diseases.
Methods: To examine immune system responses to sports activities, articles were collected from all databases: Science Direct, PubMed, Scopus, Web of Science, Springer, Google Scholar, SID, and the most recent articles were selected.
Results: High-intensity and long-term physical exercise can be effective in suppressing immune responses. Therefore, moderate-intensity exercise can be an effective strategy. Maintaining the function of the immune system in athletes was dependent on nutritional strategies, sleep control, stress management, and strict adherence to proper exercise principles and health protocols. Athletes are more prone to viral infections in the early hours after strenuous, prolonged physical exercise; and they should be limited in contact with people who may increase their risk of infectious diseases. We should mention that moderate-intensity physical exercise can improve the function of immunoglobulins, anti-inflammatory cytokines, neutrophils, natural killer cells, cytotoxic T cells, and immature B cells. Maintaining social distance, especially immediately after strenuous exercise, is also recommended for athletes due to the increased risk of infectious diseases. The immune system has been considered an effective part of sports activities in athletes in recent years. The prevalence of viral diseases such as COVID-19 has not been and will not be for the first and last time in life. Therefore, using questionnaires and initial monitoring (adherence to diets, corona testing, adherence to health protocols) can be the first step.
Conclusion: Finally Due to the pandemic of coronary heart disease and its unknowingness, providing some health and nutrition guidelines for starting exercises and sports competitions to prevent the transmission of this disease is on the agenda of this article.

Abstract

Background: Medical malpractice is a serious problem in the health care system. This study aimed to review the medical negligence in Iran.
Methods: Based on the PRISMA checklist, a search for scientific records was done separately by two researchers. All the articles that had selection criteria were evaluated in terms of methodological quality. Medical malpractice was assessed in four main divisions including negligence, Lack of skill, Carelessness and non-compliance with government regulations). The bias test was performed using the Egger’s test. Revman software was used to analyze the data.
Results: In the present study, 25 studies that examined the country's medical malpractice from April 1994 to March 2018 were included in the meta-analysis. Negligence has been implicated in 1,105 cases of the 2,068 claims. Lack of skill in 255 out of 2068 cases, 432 cases of carelessness and 244 cases of non-compliance with government regulations Were recorded the results of the meta-analysis showed that OR negligence was 0.76 (CI 95%: 0.66-0.87), lack of skill was 0.61 (CI 95%: 0.49-0.76), carelessness was 0.62 (CI 95%: 0.50-0.76) and non-compliance with government regulations was 0.66 (95% CI: 0.60-0.73). In the review of the confirmed negligence ratio of the registered complaint files, only 19 studies mentioned this ratio. The results of the meta-analysis of these 19 studies showed that the OR ratio of the confirmed negligence of complaints was 0.6 (95% CI: 0.41-0.86). From all claims, General surgeons had OR of confirmed medical malpractices, equal to 0.47 (CI 95%: 0.37-0.60), gynecologists with OR equal to 0.49 (CI 95%: 0.36-0.66), general practitioners with OR equal to 0.43 (CI 95%: 0.30-0.63) and orthopedic specialists with an OR of 0.44 (CI 95%: 0.32-0.61).
Conclusion: The results of this study help to understand the current position of medical negligence studies in the country to identify the cause of the malpractice and develop new studies for the future.

Abstract

Background: Hospital accreditation is a systematic external evaluation of a hospital's structures, processes, and outcomes by an independent professional accreditation body using published optimum, evidence‐based, and achievable standards. Accreditation is a strategy for ensuring the quality, safety, and productivity of hospital services. Implementing accreditation standards imposes a high cost on hospitals. Therefore, this cost should be offset by increased hospital efficiency. Hence, this study aimed to examine the relationship between the Iranian public hospitals’ efficiency and their accreditation status.
Methods: This descriptive and cross-sectional study was conducted in May 2019. The efficiency of general hospitals in Iran was assessed using the Pabon Lasso chart and three performance indicators of bed occupancy rate, average patient length of stay and hospital bed turnover. Then, the relationship between the efficiency of hospitals and their accreditation grade was examined. Descriptive statistics and ANOVA tests were used for data analysis using SPSS software.
Results: There were 834 general hospitals with 108,275 active beds in Iran in 2018. The Iranian general hospitals had an average bed occupancy of 62.8%, an average patient length of stay of 2.6 days, and an average bed turnover of 93.4 times. Almost 15.3% of the hospitals performed well and were located in area 3 of the Pabon Lasso chart. About 36.1% and 48.6% of the hospitals had moderate and low efficiency, respectively. Approximately, 74% and 21% of hospitals had the accreditation status of 1 and 2, respectively. There was a significant relationship between the degree of accreditation of hospitals with their bed occupancy, bed turnover and bed turnover interval.
Conclusion: The efficiency of public hospitals is low. A hospital accreditation grade is related to some of its efficiency indicators. Low efficiency and waste of resources will reduce the effectiveness of hospital and, consequently, its accreditation grade. Accordingly, the hospital's revenue will decrease and therefore, the hospital's productivity and accreditation grade will decrease and the hospital will be in a vicious cycle. Strengthening hospital accreditation standards and their proper implementation will increase the efficiency of hospitals.

Abstract

Background: At present, in nuclear medicine to calculate the cumulative activity of various organs, different techniques are being used to correct the background radiations in the conjugate view method. Each of these techniques considers the effect and weight of background radiations to a different extent. These background radiations can cause errors in cumulative activity calculations. The purpose of this study was to evaluate and compare different techniques of correcting background radiations to each other in calculating kidney cumulative activity during the nuclear scan of the bladder in children.
Methods: This study was Cross-sectional and performed for patients who were referred to Isfahan Shahid Chamran Hospital between December 2019 and June 2020. In this study using Foley, 1 mCi of TC-DTPA radio-pharmaceutical that was diluted with normal saline was injected into the patient's bladder, then images recorded by gamma cameras were extracted at 2, 5, 10, and 20 minutes, Finally with ROI drawing around kidney with having vesicoureteral reflux, Cumulative renal activity was calculated with four correcting background radiations techniques such as without correction of background radiation, conventional method, Buijs method, and Kojima method Eventually, the data was Eventually, the data was compared to each other to determine how much weight each of these techniques consider for background radiations.
Results: Cumulative kidney activity after calculation with four correcting background radiations techniques such as without correction of background radiation, conventional method, Buijs method, and Kojima method were 14.970±4.776, 13.544±4.357, 13.885±4.451, 13.879±4.449 mCi.min. respectively After comparing the calculated cumulative activity without correcting the background radiations with the cumulative activity calculated by the three techniques of correcting the background radiations such as the conventional method, Buijs method, and Kojima method, the value (P<0.05) was obtained in all cases, which shows that elimination of background radiations is significantly meaningful in the calculations of cumulative activity in the conjugate method for nuclear exams
Conclusion: after comparing correcting background radiations techniques to each other, it was determined that the conventional method technique considers the effect and weight of background radiations more than the two other techniques such as the Buijs method and Kojima method in cumulative activity calculations.

Abstract

Background: Urinary tract infection is one of the most common infectious diseases in children, the most severe form of which is acute pyelonephritis. Vitamin D plays a very important role in regulating the immune system, this study was conducted to compare vitamin D levels in children with urinary tract infections and healthy children in Kerman, Iran in 2020.
Methods: This cross-sectional study was performed on 63 children with pyelonephritis hospitalized in Afzalipour Hospital and 68 healthy children who were referred to Samen Al-Hojaj Clinic in Kerman, Iran from January 2020 to January 2021. Inclusion criteria for patients include the following: a) the presence of clinical signs and symptoms of infection such as fever b) pyuria, neurogenic bladder, urinary stones, chronic renal failure and previous diagnosis of urinary reflux. The case group was evaluated by taking a complete history, completing a questionnaire and clinical examination, and laboratory tests. The two groups were compared in terms of variables of sex, age and vitamin D levels.
Results: The mean age of the case group was 34 months and that of the control group was 38 months. The mean serum levels of vitamin D in the case and control groups were 34.66 and 42.9, respectively). P=0.016( Each group was divided into three groups according to the amount of vitamin D, including groups 1, 2 and 3, respectively, with vitamin D levels below 25 nanomoles per liter of deficiency, 25 to 50 inadequate and above 50 as the appropriate amount of vitamin D was considered. There was no significant difference between girls and boys in terms of vitamin D deficiency compared to the three groups of vitamin D levels.
Conclusion: Inadequate vitamin D levels are associated with the prevalence of urinary tract infections, and vitamin D supplementation can be a low-risk option for preventing urinary tract infections.

Abstract

Background: Surgical margin determination in malignant lesions is essential and has a direct impact on the choice of postoperative treatments and patient follow-up. Therefore, the processes affecting this variable are worth exploring. This study aimed to evaluate the difference in surgical margin values in excisional skin samples during surgery and after formalin fixation.
Methods: A cross-sectional study was performed on forty randomly selected patients with benign and malignant skin lesions who were referred to the plastic surgery ward of Ghaem Hospital in Mashhad from November 2018 to January 2019. Malignant and benign skin lesions were excised with a healthy margin, then the specimens were sent to the pathologist in a formalin-containing container. The amount of tissue shrinkage and the factors affecting them were compared.
Results: Among all 40 participants about 57.5% of the them were male and 42.5% were female. The mean age of the patients was 66.55±14.53 years. Surgical margin was evaluated in three patterns: before surgical incision and after surgical incision (P<0.001), before surgical incision and after formalin fixation (P<0.001), after surgical incision and after formalin fixation (P=0.02). In this study, the relationship between the age and initial length of the skin lesion with sample shrinkage was not statistically significant.
Conclusion: The results of this study showed that tissue shrinkage occurs both after incision and after formalin fixation. The highest rate of shrinkage was seen after surgical excision, which was due to the elasticity of the tissue itself. Increasing age and initial length of skin lesion did not affect this shrinkage. Also tumoral tissue contracted less than healthy tissue due to flexibility of fatty tissue and water and lipid content.
