Abstract
Background: Anti-dsDNA antibodies frequently found in the sera Systemic Lupus Erythematosus patients, particularly in active disease stage. Nowadays exploit different eukaryotic and prokaryotic dsDNA as antigen source and different reagents as binder. The aim of this study to compared two dsDNA different sources and tow different kinds of reagents for binder in ELISA test.
Methods: In this study bacterial genomic DNA from E.coli (ATCC 25922) and genomic DNA from calf thymus extracted with high purity and were used as antigens for IgG anti-dsDNA detection by ELISA. To coat dsDNA in microtiter wells, tow different kinds of reagents including methylated -BSA and poly-l-lysine (for pre-coating) are used. Sera from systemic lupus erythematosus patients and from normal blood donors are used to assess sensitivity and specificity of our ELISA test in compared with IF test and commercial kits.
Results: Our results displayed pre-coating of microtiter plates with methylated -BSA reduce nonspecific binding reaction and the relative sensitivity and specificity of ELISA increased when calf thymus DNA is employed as antigenic source in compared with IF test and commercial kits 80%, 88% and 100%, 98% respectively, but when E.coli DNA is used 73%, 69% and 85%, 79%, respectively.
Conclusion: The genomic DNA from calf thymus is a potentially useful source of antigen for detection of anti-dsDNA by ELISA. Also the use of methylatted- BSA could have an effective role in reducing of nonspecific binding reactions.

Methods: A case-
control study was performed on 370 pregnancies inflicted
by GDM in Hazrat
Zaynab
Hospital,
Shahed
University.
The maternal and perinatal outcomes and prevalence of risk factors based on recommendation
of the fourth workshop- conference on GDM
in these women with GDM were compared with the
same data and risk factors of randomly selected 600
pregnant women at the same time and in the same hospital, they all underwent
universal testing for GDM, and their OGTT
were normal.
Results: The prevalence of all risk factors was significantly higher in the group with GDM,
but 45 of these women (12%)
had no risk factors. 107 women (29%)
with GDM were at low risk and would remain undiagnosed
if selective screening method was used. The
main neonatal complications in the low- risk group did not differ from the
complications in other women with GDM.
Conclusions: The
universal screening of all pregnant women seems to justified whereas the
recommendations for not screening low- risk group are doubtful and require further
examination.

Background: Nerve
entrapment neuropathies are relatively frequent in the upper limb. Significant
costs related to resultant disability and treatment, and also simultaneous
occurrence of some of these syndromes can result in alteration in the treatment
approach. The aim of this study was to evaluate the frequency of these
syndromes, the rate of concurrence of carpal tunnel syndrome as the most
prevalent syndrome with others, related risk factors, and accordance of
clinical and electrophysiological findings.
Methods: In a retrospective study, data of
the 170
affected patients to these syndromes operated during a period of 10 years in a
referral center were searched using recorded documents. Information about patient's gender, age, occupation and
associated co-morbidities, clinical and surgical findings and their accordance
to the EMG-NCV findings, and also concurrence of these syndromes
were assessed.
Results: Patients' range of age was 10-91 year (mean:
48.09
year) and 74.5% of them were female. In this study carpal tunnel and
cubital tunnel syndrome were the most frequent (81.7% and 15.8%
respectively). In 23.5% of patients with carpal tunnel syndrome,
electrophysiological findings were negative but there was no false positive
result. Concurrent carpal tunnel syndrome with other syndromes were found in three
cases of cubital tunnel syndrome, two cases of thoracic outlet syndrome and one
case of Guyon's canal syndrome.
Conclusion: Constellation of symptoms, physical examination and electrophysiologic-al
findings altogether should be considered for correct diagnosis of nerve
entrapment syndromes in the upper extremity. Simultaneous entrapment in the
other regions of the same nerve or other nerves in the same extremity is a
probable condition

Background: It has been
suggested that Q/R
192 polymorphism of
paraoxonase1 (PON1) and 5A/6A
polymorphism of matrix metalloproteinase-3 (MMP-3) might be associated with the
predisposition to coronary artery disease (CAD). Therefore to investigate the significance of these polymorphisms in the
pathogenesis of CAD we performed an association
study of the polymorphisms with CAD and the
number of diseased vessels in patients with CAD.
Methods: We studied
the human PON1 and MMP3 gene polymorphisms in patients with CAD by polymerase chain reaction/ restriction
fragment length polymorphism (PCR/RFLP). These
polymorphisms were determined in 129 CAD patients and 115 control subjects who underwent
coronary angiography. CAD was defined
as the presence of one or more stenoses>50% in at least one major coronary artery and subjects with <10% stenosis served as controls.
Results: The
frequencies of the PON1
192 RR genotype and MMP-3 6A6A genotype were increased among CAD patients compared with controls (p<0.05 and p<0.001 respectively). The combined
genotypes of RR/6A6A had significantly higher
frequency in the CAD patients compared with subjects
who possess neither the MMP-3 6A6A nor the
PON1 RR genotype (p<0.001) and finally in the study of
relationship between genotypes and severity of disease, distribution of PON1 192 and MMP-3 5A6A genotypes were not associated
with the number of diseased vessels (p>0.05).
Conclusion: The combined PON1
192 and MMP-3
5A6A polymorphisms are associated with CAD but don't have any effect on the number of
diseased vessels.

Abstract
Background: Coronary Artery Disease (CAD) is one of the leading causes of mortality
and morbidity all over the world. One of the most important predictors of outcome of patients with coronary aterey disease is the site of stenosis i.e. Proximal versus nonproximal stenosis. This study designed to evaluate the relationship between CAD risk factors and site of stenosis.
Methods: In this case- control study in the patients undergone coronary angiography (CAG) in Imam Khomeini Hospital, Tehran, Iran a total of 125 CAD patients with proximal lesion in CAG enrolled the study as case group and equal sex and age matched number of patients with non proximal lesion selected as control group. Two groups were compared based on presence or absence of DM, hypertension, hyper cholestrolemia, hypertriglyceridemia and cigarette smoking.
Results: Relative frequency of DM was 33.6% and 10.4% in case and control group respectively (p< 0.0001). Relative frequency in two groups were 33.6 vs 28.8% For HTN (p= 0.41), 30.4% vs 29.6% for hyper cholestrolemia (p= 0.89), 19.2% vs 16.8% for hypertriglyceridemia (p= 0.062) and 28.8 vs 39.2 for C/S (p= 0.08). Multivessle disease was significantly more prevalent in diabetics compared with non diabetic patients
89.1% vs 61% (p< 0.0001), no relationship was seen with HTN (p= 0.41), Hyper cholest- rolemia (p= 0.052) hypertriglyceridemia (p= 0.38) and cigarette smoking (p= 0.375). Conclusion: Proximal involvement of coronary arteries and multivessle disease in CAD patients is related to the history of DM but not to the history of hypertension, hypercholestrolemia, hypertriglyceridemia and cigarette smoking.

Abstract
Background: Beta- lactamase enzymes are the most important resistant factors to beta lactam antibiotics among gram negative bacteria. Nowadays, the prevalence of beta- lactamase infection is increasing worldwide and drawn the scientists attention as an important subject. Due to high prevalence of bacteria contained TEM beta lactamase and AmpC enzymes, using molecular methods especially designing universal primers could be valuable to detect all of them. The aim of this study was to determine the prevalence of TEM and AmpC (Dha and MOX) beta- lactamase genes using universal primers.
Methods: A total of 500 clinical specimens from various Hospitals in Tehran, Iran were collected and analyzed for E. coli based on biochemical tests. These clinical specimens were also screened by Disk diffusion agar, combined disk method and PCR to detect the samples producing extended- spectrum beta- lactamase.
Results: Overall 200 isolates of Escherichia coli were collected from the 500 clinical specimens out of which 128(64%) isolates were positive by PCR assay and showed bla- TEM, bla- AmpC (Dha, MOX) genes, 74(57.8%) and 5(3.9%) to have bla- TEM and bla Dha, respectively. Mox gene was not detected in any of the specimens.
Conclusions: Our results revealed that using the molecular methods with phenotype methods is very essential for complete detection of Beta- lactamases. There is the need for updating the treatment protocol because the prevalence of this resistance is increasing.

Abstract
Background: Resistance to beta-lactam antibiotics in clinical isolates frequently results
from the production of β-lactamase enzymes. In recent years, the production of extended spectrum β-lactamases (ESBLs) and AmpC β-lactamase have greatly increased, especially in clinically isolated Escherichia coli. On the other hand, beta lactamase genes have several subfamilies and designing universal primers could be valuable in their detection. The beta-lactamase-producing E. coli which is resistant to beta-lactam antibiotics may pose a great risk to the patients. The CTX-M-1 gene is responsible for beta lactamase resistance. The purpose of this study was to find the percentage of CTX-M-1 carrying E. coli strains.
Methods: A total of 500 urine samples were collected from different hospitals in Tehran, Iran during September to February 2009. The samples were cultured on EMB agar and incubated at 37 C for 24 hours. Some biochemical tests were carried out on
the isolated samples. The presence of CTX-M-1 gene was determined by PCR on the
isolates already identified phenotypically by disk diffusion agar and combined disks. Results: In general, 200 out of the initial 500 samples were identified as E. coli, among which 128 (79.5%) were ESBLs producing strains. PCR used for the detection of CTX-M-
1 gene, showed that 99 (77.34%) out of 128 isolates contained such gene.
Conclusion: The results of this study showed a high percentage of β-lactamase resistant E. coli strains. This is a serious matter and would pose a public hazard and every step should be taken to avoid such hazard.

Abstract
Background: Trauma to the hand is very common and consequently, metacarpal fractures are not rare entities. Some of these fractures need surgery. Considering the diversity of surgical methods available for these kinds of fractures and also the importance of achieving full function and speedy return to work for patients that are mainly young workers or athletes, this study was undertaken to investigate the outcome of treating these fractures by mini-plates.
Methods: Eighteen patients with open or comminuted fractures of metacarpal bones who were admitted to the emergency department of Sina Hospital between the years 2007 and 2010 underwent fixation surgery using mini-plates. Fourteen patients with 17 metacarpal fractures completed the study.
Results: Thirteen out of 14 patients had complete fracture ::::union::::. The patient with non- ::::union:::: underwent revision surgery and bone graft. Four individuals developed an extensor lag of 15 degrees without functional impairment. Two patients had joint stiffness that was relieved after a period of physiotherapy and one developed wound dehiscence and discharge that improved with debridement and use of antibiotics without plate removal. Six patients had complaints regarding their hardware prominence and were satisfied after its removal. Overall, there was a 79% satisfaction rate upon the treatment.
Conclusion: Despite the afore-mentioned complications, use of mini-plates seems to be a suitable treatment option for patients with comminuted metacarpal fractures. However, more studies with larger sample sizes are suggested to reach a firm conclusion.

Abstract
Background: Numerous use of Beta Lactame in treatment of bacterial infections resulted in increments of drug resistance of such bacteria. One of difficulties in treatment of hospital infections is Extended Spectrum Beta Lactamase (ESBL) among isolated clinical strains of E.coli. Since some of ESBL strains shows double reaction in drug sensitivity test at in vitro and in vivo condition, therefore it makes difficulties in selection of right treatment. In the last years, CTX-M enzymes have become the most prevalent ESBLs in worldwide. The prevalence of ESBL types largely remains unknown in many parts of the Iran. This study was made to find the prevalence of ESBL-producing E.coli and molecular detection of CTX-M-1 in Tabriz.
Methods: In the present study, 400 urine samples collected between November 2009 and April 2010, from Tabriz Hospitals were studied. Out of the 400 samples, 188 E.coli isolates were detected by standard biochemical tests. Susceptibility to antimicrobial agents was tested to 10 antibiotics by the disk agar diffusion (DAD) method. ESBL production was screened by phenotypic test that included both separate and combined disk agar diffusion techniques. The screened isolates were investigated by PCR assay to detect CTX-M-1 gene.
Results: From the total 188 E.coli isolates, 82 (43.6%) were shown to produce ESBLs by phenotypic test. During the PCR method on the 82 isolates, 69 (84.1%) were confirmed as CTX-M-1 producing group.
Conclusion: The present study showed that CTX-M-producing isolates were increasing among E.coli strains and indicated the need for adequate susceptibility tests in laboratories for choosing the appropriate antibiotics for treatment.

Background: Breast
cancer is the most common form of hereditary cancer worldwide and is an
important cause of morbidity and mortality. Approximately 5-10% of breast
and ovarian cancers are
due to the highly penetrating germline mutations in cancer predisposing genes. Two genes, BRCA1 and BRCA2, account for
at least half of these cases. The demand for BRCA1 and BRCA2 mutation screening is rapidly
increasing as their identification will affect the medical management of people
at increased risk for the disease. Therefore, the aim of this study was to
investigate BRCA1/2 mutations in 100 high risk Iranian families.
Methods: One hundred families who met the
minimal risk factors for breast/ovarian cancer were screened among
the families referred to Kawsar Human Genetics Research
Center for the diseases in 2009-2011. The entire coding sequences and each
intron/exon boundaries of BRCA1/2 genes were screened for by direct
sequencing and MLPA in both patients and the controls.

Conclusion: Ten mutations seemed to be pathogenic and the disease-causing
mutations were seen in 16% of the families. In addition, from the

Abstract
Background: Improvement of growth, nutrition and calories intake in neonates is derived by massage.
Methods: This study is a randomized clinical trials settled in Vali-e-Asr Hospital Neonatal Intensive Care Unit (NICU) ward in 2012. The control group consisted of 19 infants who were not massaged on them. 15 infants in the intervention group received massage therapy for 10 days, three times a day by trained massage theurapist. Massage last 15 minutes and was done one hour after feeding. Massages were done in 2-7 days neonates. Weight gain, intake calories and oral feeding were compared between studied groups. Data was registered in SPSS v.18 and was analyzed via compatible statistics tests.
Results: There were no significant different anthropometric measures at birth (weight-head circumferences and height) and gestational ages of delivery between two groups. Massages had no side effects on cases. Caloric intake at the end of 10 days (end of intervention) showed significant differences between the two groups (P=0.04). But no differences was shown for weight gain. Cases who received massage reached sooner to oral feeding but this difference was significant at 90% significance level (P=0.08).
Conclusion: After 10 days, massage therapy increases oral nutritional intake but to find more accurate details requires further studies to be planned.

Abstract
Background: Although sexual dysfunction is a common problem in patients with obsessive-compulsive disorder (OCD), there are sparse clinical research on the study of the correlation between OCD subtypes and different phases of sexual response cycle. This study was undertaken to assess sexual function and its different phases in a group of Iranian patients with OCD. Methods: This was a descriptive cross-sectional study. The subjects consisted of 56 married OCD patients (36 female, 20 male) who suffered from OCD according to a psychiatric interview and DSM-IV questionnaire based on structured clinical interview for DSM (SCID). Patients were between 18 to 50 year age that had been referred to the outpatient clinic of Roozbeh Psychiatric Hospital and three private psychiatric clinics in Tehran (from September 2011 to February 2013). Five Questionnaires were used in this study: Iranian validated form of Female Sexual Function Index (FSFI), International Index of Erectile Function (IIEF), Maudsley Obsessional Compulsive Inventory (MOCI), Obsessive Compulsive Inventory-Revised (OCI-R) and a questionnaire which has provided demographic data and other relevant information regarding sexual function and OCD. Results: The prevalence of sexual dysfunction among female was 80.6% and the frequency of disorders in different subscales of FSFI including sexual desire, sexual arousal, lubrication, orgasm, satisfaction and sexual pain were 50%, 58.3%, 36.1%, 44.4%, 41.7% and 52.8% respectively. Sexual disorder is reported in 25% of male OCD patients which subscales' evaluation of IIEF shows low sexual desire in 10%, erectile disorder in 20%, orgasmic disorder in 25%, sexual dissatisfaction in 40% and 50% decreased in the total sore of IIEF. Moreover, the correlation coefficients between the total score of OCI-R with erectile and satisfaction subscales of IIEF were statistically significant. The score of washing subscale in OCI-R and sexual satisfaction was significantly correlated. Conclusion: High prevalence of sexual dysfunction in OCD women and significant correlation between male sexual dysfunction and OCD (r= -481.0 between total score of OCI-R with erectile dysfunction and r= -458.0 between total score of OCI-R and sexual satisfaction) could confirm a relation between OCD and sexual disorders. So, evaluation of sexual function in all patients with OCD is recommended.

Abstract
Background: Blood pressure decreases during sleep and is markedly increased in the morning in healthy individuals. Lack of nocturnal blood pressure fall (non-dipping) has been associated with cardiovascular morbidity, mortality and other organ damage. However, their importance in chronic renal failure is unclear. This study aimed to investigate relationship between circadian rhythm of blood pressure and renal failure severity in patients with chronic kidney disease.
Methods: This cross-section study was done in April 2016. The study population was 95 patients, more than 30 year old with hypertension and chronic renal failure. Patients were selected from clinics of two private and university hospitals affiliated to Qom University of Medical Sciences Shahid Beheshti Hospital and Vali-e-Asr Hospital, Iran. Checklist containing data such as age, sex, duration of renal failure and cause of renal failure were filled. Serum creatinine and serum urea levels were measured and entered in the checklist. The circadian rhythm of blood pressure in all patients was assessed by Holter monitoring. patients who had less than 10% decrease in blood pressure overnight were considered non-dipper and those who had 10% or more decrease in blood pressure overnight were considered dipper.
Results: Average (SD) 24-hour ambulatory systolic and diastolic of blood pressure was 136.56 (16.66) and 84.84 (10.86) mmHg, respectively. 70 patients (73.7%) had non-dipper blood pressure pattern and 25 patients (26.3%) had dipper blood pressure pattern. There was no significant difference between two groups (dipper and non-dipper) based on distribution of gender (P=0.744), age (P=0.407), serum creatinine (P=0.569), serum urea (P=0.689) and renal failure duration (P=0.812). Mean of glomerular filtration rate in dipper group was 68.64±4.13 and in non-dipper group was 65.09±16.27 (P=0.337).
Conclusion: The results of this study did not show a significant relationship between circadian rhythm of blood pressure and renal failure severity. In addition, patients with chronic renal failure showed higher rates of non-dipping pattern of blood pressure.

Abstract
Background: 2-phenylethanol is a colorless and aromatic compound with antimicrobial effects which is used extensively in perfumes and cosmetics, as well as in the food industry. Chronic vulvovaginal candidiasis is a vulvovaginal inflammation which is caused by Candida spp. Resistance to clotrimazole which is one of the most common drugs in the treatment of this disease was reported in many patients. In order to improve the treatment, the effect of 2-phenyl ethanol was investigated in combination with clotrimazole on Candida species isolated from chronic vulvovaginal candidiasis.
Methods: This interventional study was performed in Iran University of Medical Sciences from February, 2016 until December, 2016 on Candida species isolated from women with chronic candidial vulvovaginitis who had been referred to Lolagar Hospital of Tehran. All specimens were examined by direct microscopy, culturing on Candida CHROMagar medium (to primary identification), sabouraud dextrose agar medium) to preservation the isolates) and determining the internal transcribed spacer (ITS) sequence (in order to final determination of Candida species). Then clotrimazole and 2-phenyl ethanol alone and in combination, was examined on isolated species, according to Clinical and Laboratory Standards Institute (CLSI) M27-A3 protocol (micro-broth dilution method). Finally, findings were analyzed.
Results: From 40 detected strains of Candida species in this study, 95% were Candida albicans and 5% were Candida africana. The mean minimum inhibitory concentration (MIC) and minimum fungicidal concentration (MFC) of clotrimazole were 24.73±28.87 µg/ml and 30.18±33.004 µg/ml, respectively and the mean MIC and MFC of 2-phenylethanol were 2580±932.38 µg/ml and 3200±1403.29 µg/ml, respectively. The MIC50 and MIC90 of clotrimazole were 16 and 64 µg/ml, respectively. The MIC50 and MIC90 of 2-phenylethanol were both 3200 µg/ml. Most of the isolates were resistant to clotrimazole (82.5%). In combination test, the mean MIC of 2-phenylethanol and clotrimazole alone were 3200±0 µg/ml and 56±40.16 µg/ml, respectively. The fractional inhibitory concentration index (FICI) range was 0.14-0.37. Also, there was a significant difference between clotrimazole MIC values alone and in combination (P= 0.021).
Conclusion: The synergistic effect was observed in combination of clotrimazole and 2-phenylethanol.

Abstract
Background: Association of Addison's disease with connective tissue diseases such as scleroderma and Sjogren have been rarely reported. Anti-centromere antibody (ACA) has been associated with exocrine gland dysfunction in anti-Ro, anti-La negative Sjogren’s syndrome and may be one of the causes of xerostomia in community. The purpose of this article was to introduce a rare case of scleroderma-Sjogren intermediate phenotype with positive anti-centromere antibody in a known case of Addison’s disease admitted for dental caries and xerostomia.
Case Presentation: A 29-year-old woman with Addison’s disease referred to a dental clinic due to recurrent dental caries. Addison’s disease was confirmed by low basal serum cortisol level and unresponsive serum cortisol level to adrenocorticotropin hormone (rapid ACTH stimulation test). Signs of xerostomia, xerophthalmia, Raynaud’s phenomenon, gastro-esophageal reflux, masked face, osteoporosis, positive anti-centromere antibody, negative anti-Ro and anti-La antibodies and failure to match the pathology of the minor salivary gland of lip with Sjogren's disease were found. The diagnosis of scleroderma-Sjogren intermediate phenotype was raised with considering some of the symptoms of scleroderma and Sjogren and not the exact classification criteria for each of these two diseases. Hydroxychloroquine, fluoride and Biotene® mouthwash (Laclede, Inc., CA, USA) (oral moisturizing saliva), chewing gum containing xylitol plus artificial tear droplet was prescribed. Drinking plenty of fluids was recommended. Due to gastroesophageal reflux and osteoporosis, Pantoprazole and CinnoPar® (Cinnagen, Iran) (parathyroid hormone analogue) plus calcium and vitamin D supplements was administered. Regarding adrenal insufficiency, Prednisolone and Fludrocortisone were continued.
Conclusion: For evaluation of recurrent dental caries, especially in patients with autoimmune disease, anti-centromere antibody may be useful to identify the cause of dry mouth, as well as early detection of limited scleroderma or scleroderma-Sjogren intermediate phenotype.

Abstract
Background: Contact lenses are transparent, thin plastic disks that cover the surface of the cornea. Appropriate lens prescription should be performed properly by an expert to provide better visual acuity and reduce side effects. The lens administration is a multi-stage, complex and time-consuming process involving many considerations. The purpose of this study was to develop a decision support system in the field of contact lens prescription.
Methods: In this fundamental study, data were collected from 127 keratoconus patients referred to the contact lens clinic at Farabi Eye Hospital, Tehran, Iran during the period of March 2013 to July 2014. Five parameters in the contact lens prescribing process were investigated. Parameters were collected as follows. “Lens vertical position”, “vertical movement of the lens during blinking” and “width of the rim” in the fluorescein pattern were obtained by recording videos of the patients while wearing the lens. “Fluorescein dye concentration” under the lens was evaluated by the physician and “patient comfort” was obtained by asking the patient to fill a simple scoring system. Approved and disapproved lenses were judged and recorded based on the decision of an expert contact lens practitioner. The decision support system was designed using artificial neural networks with the mentioned variables as inputs. Approved and disapproved lenses are considered as system outputs. Artificial neural network was developed using MATLAB® software, version 8.3 (Mathworks Inc., Natick, MA, USA). Eighty percent of the data was used to train the support vector machine and the rest of the data (20%) to test the system's performance.
Results: Accuracy, sensitivity and specificity, calculated using the confusion matrix, were 91.3%, 89.8% and 92.6% respectively. The results indicate that the designed decision support system could assist contact lens prescription with high precision.
Conclusion: According to the results, we conclude that hard contact lens fitness could be evaluated properly using an artificial neural network as a decision support system. The proposed system detected approved and disapproved contact lenses with high accuracy.

Abstract
Background: Degenerative scoliosis most commonly affects the lumbar spine in the elderly, resulting in facet and disc degeneration, leading to increased pain and progressive deformity. Due to the importance of the results of long fusion and the rate of coronary and sagittal correction of imbalance in patients with degenerative scoliosis, the present study was performed to evaluate the results of long fusion surgery in patients with degenerative lumbar scoliosis.
Methods: The present study was performed as a prospective cohort study in patients with degenerative scoliosis who were candidates for surgery at Al-Zahra Hospital in Isfahan. Basic patient information including age, sex, anatomical location of vertebral deviation and slip, degree of severity of deviation based on cobb angle and number of previous surgeries were reviewed and recorded. The surgical characteristics of the patient were extracted from the medical record including the approach used, the anatomical location of the fusion, whether or not to perform decompression, the number of decompression and fusion levels, the amount of bleeding during surgery, and the duration of surgery.
Results: Eleven female patients participated in this study. The mean age of patients was 55.64 years with a standard deviation of 7.67 years. The minimum age was 40 years and the maximum age was 66 years. Patients' symptoms included low back pain-leg pain (3 cases), back pain-lameness (2 cases) and back pain-leg pain-lameness (6 cases). All patients underwent two stages of surgery. Mean amount of bleeding and time of surgery in the second surgery were significantly reduced compared to the first surgery (p <0.05). Complications of surgery included proximal junctional vertebral fracture (PGK) in two patients

| Conclusion: Degenerative scoliosis is a complex clinical condition in which the patient's main problems are pain and disability. Long fusion surgery in patients with degenerative scoliosis significantly reduced the mean deviation, Patient’s pain and severity of disability 6 months after surgery. |

Abstract
Background: Urinary tract infection is one of the most common infectious diseases in children, the most severe form of which is acute pyelonephritis. Vitamin D plays a very important role in regulating the immune system, this study was conducted to compare vitamin D levels in children with urinary tract infections and healthy children in Kerman, Iran in 2020.
Methods: This cross-sectional study was performed on 63 children with pyelonephritis hospitalized in Afzalipour Hospital and 68 healthy children who were referred to Samen Al-Hojaj Clinic in Kerman, Iran from January 2020 to January 2021. Inclusion criteria for patients include the following: a) the presence of clinical signs and symptoms of infection such as fever b) pyuria, neurogenic bladder, urinary stones, chronic renal failure and previous diagnosis of urinary reflux. The case group was evaluated by taking a complete history, completing a questionnaire and clinical examination, and laboratory tests. The two groups were compared in terms of variables of sex, age and vitamin D levels.

Results: The mean age of the case group was 34 months and that of the control group was 38 months. The mean serum levels of vitamin D in the case and control groups were 34.66 and 42.9, respectively). P=0.016( Each group was divided into three groups according to the amount of vitamin D, including groups 1, 2 and 3, respectively, with vitamin D levels below 25 nanomoles per liter of deficiency, 25 to 50 inadequate and above 50 as the appropriate amount of vitamin D was considered. There was no significant difference between girls and boys in terms of vitamin D deficiency compared to the three groups of vitamin D levels.

Conclusion: Inadequate vitamin D levels are associated with the prevalence of urinary tract infections, and vitamin D supplementation can be a low-risk option for preventing urinary tract infections.

Abstract
Background: The prevalence of emerging and re-emerging diseases has made the need for basic preparations for all health care organizations more crucial. Strengthening preparedness and formulating crisis strategies will have a great impact on reducing casualties. Given the importance of preparing hospitals to deal with such an outbreak and reduce the resulting mortality, the present study was conducted to assess their readiness against Covid-19.
Methods: The present study is a quantitative and descriptive cross-sectional research conducted from October to March 2019. Data collection used the standard checklists prepared by the European Center for the Prevention and Control of Coronavirus and the Centers for Disease Control and Prevention, consisting of eight domains and 21 components. The minimum score that each hospital could get in this checklist was 143 and the maximum was 429. The sampling method in the present study was a census, and nine reference hospitals for Corona were included in the study. All hospitals’ directors, managers, quality officers and crisis secretaries and others related to hospital readiness during Covid-19 were recruited by the census.

Results: On average, the hospitals scored 391 out of 429, indicating a fairly "high readiness" in dealing with Covid-19. The highest score obtained by the hospitals was 425 and the lowest score was 349. In terms of preparation areas, the hospitals’ readiness was higher than 80% in all areas. The highest readiness of hospitals was in the fifth domain, i.e. Hand hygiene, personal protective equipment and hospital waste management. The 7th domain namely, patient placement and relocation, and patient visitor access was of the lowest preparation.
Conclusion: The hospitals were of fairly appropriate readiness to deal with Covid-19. This level of preparedness, despite being desirable, might not reflect the real capacity of hospitals to deal with this disease. Regular evaluation of the Covid referral hospitals could help make these hospitals more prepared. Also, the experiences of hospitals that were more prepared should be used to improve the condition of other hospitals.

Abstract
Background: Mesenchymal stem cell (MSC) transplantation is a promising therapy for kidney repair. This study compared the regenerative effects of feline MSCs (fMSCs) and telmisartan, a renin-angiotensin blocker (RAB), in a feline model of chronic kidney disease (CKD).
Methods: The fMSCs were obtained from 35 Persian cats with CKD and characterized by CD44, CD90, and CD105 markers by using real-time RT-qPCR. The cats were randomly allocated to four groups, fMSCs injection (first group), telmisartan administration (second group), no treatment (third group), and healthy controls (fourth group). The study was conducted in Kerman province from December 2018 to December 2019. The factors that may affect the risk of CKD, such as age, weight, and history of kidney diseases, were considered as independent variables. The presence or absence of CKD was the dependent variable. The cats were followed up for 120 days and evaluated by physical examination, glomerular filtration rate (GFR), blood urea nitrogen (BUN), serum creatinine (SCr), serum urea, alanine transaminase (ALT), urine specific gravity (SG), and kidney histopathology. Statistical analysis was performed using SPSS software (version 20) with two-way ANOVA and Tukey test. P<0.05 was considered statistically significant.

Results: The fMSCs group showed significant improvement in GFR, BUN, SCr, serum urea, SG, and kidney histology compared to the other groups. The fMSCs group also showed increased expression of CD44, CD90, and CD105 genes in the kidney tissue, indicating enhanced stem cell activity. The telmisartan group showed modest improvement in blood pressure and proteinuria, but no significant effect on other parameters. fMSCs transplantation can restore the kidney function and structure in cats with CKD by modulating the apoptosis and proliferation of renal cells. The telmisartan patients benefited from the anti-hypertensive and anti-proteinuric effects of the drug, but not from its anti-fibrotic or anti-inflammatory effects.
Conclusion: fMSCs transplantation was more effective than telmisartan in improving kidney function and reducing kidney damage in cats with CKD. fMSCs may be a potential therapeutic option for CKD patients.
