Abstract
Background: One of the most important criteria of the development of countries at the national and international levels is the survey of scientific productions indexed in authentic databases. This study aimed to analyze the scientific productions by Iranian researchers on gastroenterology and hepatology in the Web of Science (WOS) database.
Methods: This applied study used a scientometric approach to review the scientific productions by Iranian researchers on gastroenterology and hepatology in the WOS database. Therefore, the total number of scientific documents related to the category of gastroenterology and hepatology, Iran, and other countries were recovered. The time frame was until the end of 2017. Data were collected in April 2018. This research was conducted at the Clinical Research Unit in Afzalipour Medical Center in Kerman, Iran.
Results: The results showed that during 1983 to 2017, the total number of 2180 scientific productions (0.31%) on gastroenterology and hepatology were published in Iran, ranking the country 34th in the world. Compound annual growth rate (CAGR) in Iranian scientific publications was 9.5%. The «Tehran University of Medical Sciences» was identified as the most productive center in gastroenterology and hepatology field with a contribution rate of 35.69%. The results also showed that the most international collaboration of Iranian researchers on gastroenterology and hepatology field was with the United States of America (USA) scientific centers (5.82%). Most articles in the field of gastroenterology and hepatology (26.69%) were published in the «Hepatitis Monthly» journal. Most scientific productions in this field were related to the microbiology. The majority of scientific productions on gastroenterology and hepatology field were published in the form of article. Most scientific productions by Iranian researchers on gastroenterology and hepatology in the Web of Science (WOS) database were in English language.
Conclusion: Results showed that although compound annual growth rate (CAGR) in Iranian gastroenterology and hepatology publications was high rather than gastroenterology and hepatology scientific productions CAGR in the world, it’s not satisfying. Most articles have published in journals with low impact factor. Therefore, we should resolve the problems and increase the scientific level of researches.

Abstract
Background: Emergence from general anesthesia and removing of tracheal tube can be associated with coughing, agitation, and hemodynamic disturbances. Dexmedetomidine is an alpha two adrenoceptor agonist that has effective sedation with less cardiovascular unstability and respiratory depression and may be useful for extubation and prevention of hemodynamic response during tracheal tube removing. The aim of this study was to evaluate the effect of dexmedetomidine on hemodynamic responses during endotracheal extubation and sedation level in recovery room.
Methods: In an analytical study, fifty women aged 20-50 years old candidate to cholecystectomy under general anesthesia and tracheal intubation were entered randomly to this study in two groups (no. 25) at Imam Khomeini Hospital, Urmia, Iran, and under support of Urmia University of Medical Sciences Urmia, Iran, from May 2017 to May 2018. Ten minutes before end of surgery, 0.8 µg/kg dexmedetomidine in the study group and for the other patients in control group normal saline as placebo were infused over ten minutes. During the emergence phase, blood pressure, heart rate and oxygen saturation were recorded at 0,1,2,3 and 5 minutes after extubation. Also, sedation index was evaluated via the Ramsay sedation score and recorded at recovery room.
Results: Heart rate, systolic blood pressure and diastolic blood pressure in patient with infusion of dexmedetomidine were lower significantly at 1,2,3 and 5 minutes after extubation than control group. Data for heart rate, systolic and diastolic pressure, at min 1 after extubation were 81±6 vs. 88±9, 120.64±13.21 vs. 137.52±11.06, 72.84±8.32 vs. 81.36±9.26 in dexmedetomidine and control groups respectively. Data for heart rate, systolic and diastolic pressure, at min 5 after extubation were 73±6 vs. 80±8, 110.64±10.68 vs. 119.88±10.01, 69.84±8.32 vs. 73.48±5.13 in study and control groups, respectively. As well as 80% of the patients in dexmedetomidine group had satisfactory sedation and cooperation in compare to the 28% in control group (P=0.001).
Conclusion: Intravenous infusion of 0.8 µg/kg dexmedetomidine 10 minutes before extubation of endotracheal tube and during emergence, facilitate extubation and lead to hemodynamic stability and satisfactory sedation.

Abstract
Background: Arterial pressure is one of the most important physiological variables and often needs to be monitored repeatedly or continuously in perioperative period. Arterial pressure monitoring is one of the standard monitoring in operating room. During general anesthesia, blood pressure can be measured by using a noninvasive arterial pressure method or continuous invasive arterial pressure by an invasive arterial line. Comparison of invasive measurements in the patient’s candidate to esophagectomy has not been assessed. The aim of this study was to compare invasive and noninvasive blood pressure in these patients.
Methods: In a prospective, cross-sectional, observational study, 42 hemodynamically stable patients candidate for esophagectomy under general anesthesia in supine position were evaluated at Urmia Imam Khomeini Hospital operating room from June 2017 to April 2018. The patients had American Society of Anesthesiologists (ASA) physical status II or III and the patients who had complete heart block and marked arterial blood pressure differences greater than 10 mmHg in the two arms were excluded. After induction of anesthesia and patients monitoring, the radial artery was cannulated for invasive blood pressure monitoring and noninvasive blood pressure was measured via the arm cuff on the other hand at the four-time intervals: after radial artery cannulation (T1), during release of esophagus (T2), during anastomosis (T3) and at the end of operation (T4).
Results: The mean difference between indirect and direct systolic blood pressure was 0.85±2.93, -8.42±2.9, 6.50±3.60 and 2.67±2.6 mmHg and for diastolic blood pressure was 3.53±2.67, 4.57±2.22, 2.10±2.58 and 1.03±1.53 mmHg respectively, at the T1 to T4. At the all-time intervals, there were no statistically differences between systolic and diastolic blood pressure measurement regarding invasive and noninvasive blood pressure (P=0.77)
Conclusion: Noninvasive arterial blood pressure showed acceptable agreement with invasive measurements for systolic, diastolic and mean pressure. According to fhe finding of this study, there were no statistical differences between systolic and diastolic blood pressure measurement regarding invasive and noninvasive blood pressure and these two methods can be used in selected patients.

Abstract
Background: Coronary artery ectasia (CAE) is dilatation of an arterial segment to a diameter at least 1.5 times that of the adjacent normal coronary artery. The incidence of coronary artery ectasia is distinct in different countries that can be found in 1.2% to 5% of angiographic examinations.
Methods: This is a retrospective study that was conducted from September 2019 to February 2020 in Kermanshah University of Medical Sciences and the results were reported briefly. To obtain the desired articles, electronic searches were conducted in databases including the Scopus, PubMed, and Science Direct databases without time limited until October 2019. The keywords used were Coronary Artery Ectasia AND (Diabetes OR "Diabetes Mellitus"). This was done by two individuals separately and the final results were confirmed by a third person. Mixed method appraisal tool (MMAT) was used to evaluate the quality of studies. The structure of writing and the process of performing and reporting the study are based on the PRISMA checklist.
Results: Based on the search strategy carried out at PubMed, Scopus and Science Direct databases, 106 studies were found, which resulted in 24 articles being analyzed based on inclusion and exclusion criteria of which three were conducted in China, 18 in Turkey and one in Sweden, Egypt, and France. Finally, 24 articles were analyzed and the results showed a direct and effective relationship between diabetes mellitus and CAE (OR=1.19, CI: 0.94, 1.51).
Conclusion: Based on these results, the risk of CAE in subjects with diabetes mellitus was 19% higher than in subjects without diabetes mellitus.

Abstract
Background: Mycoplasma pneumoniae is one of the causes of upper and lower respiratory tract infections especially in children, and antibiotics affecting the cell wall do not affect this type of infection. This study aimed to evaluate the prevalence of Mycoplasma pneumoniae and macrolide resistance in children with community-acquired pneumonia in Kerman city.
Methods: This cross-sectional study was conducted in a selected teaching hospital in Kerman city from July 2018 to July 2019. Fifty-one children hospitalized in the pediatric ward were diagnosed with bacterial pneumonia acquired from the community. These children were included in the study by census method. The data collection tool was a standard checklist. The checklist was completed by parents. Pharynx and nasal swab samples were taken from all patients. PCR was used to identify mycoplasma.
Results: The highest number of patients with bacterial pneumonia acquired from the community were male (52.9%). The average age of these cases was 5.52±3.93 years and the average weight of these patients was 17.23±10.55 kg. Lung involvement was one-sided in more than half of the patients. The most of lung involvements were lobar (31.4%). More than half of the patients didn’t have any associated involvement (52.9%). The most common type of associated involvement was peribronchial thickening (29.4%). The most commonly used antibiotic was Ceftriaxone (76.5%). The prevalence of Mycoplasma pneumoniae infection in children with pneumonia was 2%. These children had an rRNA gene at position 2063 that had high levels of macrolide resistance.
Conclusion: The results of this study showed very low rates of Mycoplasma pneumoniae infection in children with bacterial pneumonia acquired from the community. Therefore, it can be concluded that in addition to Mycoplasma pneumoniae, other viral and bacterial agents are also involved in respiratory infections that need to be identified and addressed.

Abstract
Background: Vesicoureteral reflux (urinary reflux) is one of the most common congenital anomalies in children. This study aimed to compare the ureteral opening distance from the midline in children with vesicoureteral reflux (VUR) and healthy children
Methods: This cross-sectional study was performed on the children with pyelonephritis who were referred to Afzalipour Hospital in Kerman, Iran from August 2019 to August 2020. Twenty children with vesicoureteral reflux (urinary reflux) and twenty healthy children were randomly selected. Then children with vesicoureteral reflux (urinary reflux) and healthy children were divided into two groups. The distance from the ureter to the midline in these children was then measured by a radiologist with a Philips affinity 70 ultrasound machine.
Results: In both groups (healthy children and ones with vesicoureteral reflux), ninety-five percent of the children were girls. The mean age of the children in the second group (the children with urinary reflux) was 5.8±2.58 years and the mean age of the children in the first group (the healthy children) was 4.75±1.54 years. The mean age difference between the children with urinary reflux and healthy children was not statistically significant (P=0.127). The mean distance of the ureter from the midline in children with vesicoureteral reflux was 11.44±2.60 mm and the mean distance of the ureter from the midline in healthy children was 9.32±2.74 mm. The mean difference distance of the ureter from the midline in children with vesicoureteral reflux (VUR) and healthy children was statistically significant (P=0.002).
Conclusion: Considering that the distance between the ureter and the midline in patients with urinary reflux is significantly longer than non-reflux ureters, it can be used as a non-invasive method for the diagnosis of vesicoureteral reflux (urinary reflux) in children.

Abstract
Background: Tic disturbances are a group of developmental neurological disorders that often occur in childhood because of abrupt and automatic constrictions of muscles. This study aimed to evaluate the serum levels of 25-hydroxyvitamin D in children with a tic disorders.
Methods: This present case-control research was performed on 63 children with tic disorders and 63 children who were healthy and were referred to Besat clinics in Kerman, Iran from October 2019 to October 2020. The clinical information of patients with tic disorders was collected using a questionnaire. Serum vitamin 25(OH) D3 levels were assessed in patients with tic disturbances after identifying tic disorder.
Results: In the group of children with tic disorders, the highest number of patients were girls and in the group of children who were healthy, the highest number of patients were girls. The average age of patients in the case group was 10.37±0.31 years old and the average age of patients in the control group was 10.06±0.41 years old. The average age difference between the children with tic disorders and healthy children was not statistically significant (P=0.971). The average body mass index (BMI) in the case group was 16.98±0.35 and the average BMI in the control group was 16.0±84.56. The average body mass index difference between the children with tic and healthy children was not statistically significant (P=0.838). The mean serum Vit 25(OH) D3 levels in the control group were higher than the average serum level of vitamin D in the case group. The mean serum vitamin D level difference between the children with tic disorders and healthy children was statistically considerable (P=0.036).
Conclusion: The results showed that the average serum level of vitamin D in children with tic disorders was significantly lower than in children who were healthy. To investigate and confirm this relationship, more long-term studies with a larger number of patients are needed.

Abstract
Background: Juvenile idiopathic arthritis is the most common rheumatic disease in children, which includes a group of disorders that all have a common clinical manifestation of arthritis. The aim of this study was evaluation of the clinical and laboratory symptoms of children with idiopathic arthritis who treated with intravenous methylprednisolone pulse.
Methods: This study is a descriptive cross-sectional study of 20 years in Imam Khomeini Hospital from 1993 to 2013. Information of 202 hospitalized patients diagnosed with JIA who had files, containing demographic, clinical and laboratory data were recorded. The software 17spss was used for data statistically analyzed.
Results: The mean age of patients was 6.98±3.65 years. After treatment in all age and sex groups, the mean of Hb and HCT increased significantly and WBC and PLT decreased significantly. Mean CRP and ESR decreased significantly. Mean alkaline phosphatase was significantly reduced in boys. There were no significant changes in AST and ALT. At Ca and K levels, the decrease was significant. There was significant increase in Na. Ca level in boys and K level in girls decreased and Na level in boys increased significantly. Serum sodium also increased significantly in the group over ten years, potassium in the group of five to ten years and calcium in the group up to 5 years and over ten years decreased significantly. FBS did not change significantly. Blood pressure increased significantly. In this study, the highest joint involvement related to knee joint that was 89.7% and the lowest joint involvement related to neck joint that was 7.4%, which ultimately improved significantly By 76.7% in response to treatment.
Conclusion: Due to its availability, improving patient function and reducing clinical symptoms, this treatment is recommended. However, the need for further studies and consultation of other specialists, including cardiovascular counseling, seems necessary.

Abstract
Background: Preterm premature rupture of membranes (PPROM) is one of the factors that can increase maternal and neonatal mortality, which is affected by several factors. This study aimed to investigate the factors affecting the frequency of preterm premature rupture of membranes in pregnant women.
Methods: This descriptive-analytical study was conducted in Afzalipour Hospital in Kerman from January 2018 to January 2019. All pregnant women with PPROM and normal pregnant women referred to this Hospital were included in the study through convenient sampling. The data collection tool was a checklist containing patients' demographic information (age, education, occupation, gestational age, number of pregnancies, urinary tract infection (confirmed by the attending physician), history of premature rupture of the water sac, vaginal bleeding (bleeding in any period of pregnancy as the person had visited the doctor), history of premature birth, pregnancy care and trauma (any trauma)). To analyze the data, descriptive statistics (frequency, percentage, mean, and standard deviation), analytical (Chi-square test) and SPSS software version 22 were used.
Results: In this descriptive-analytical study, 400 pregnant women were studied. Two hundred of pregnant women had preterm premature rupture of membranes and 200 of pregnant women did not have such condistion and were normal. The mean age of pregnant women was 27.4±5.4 years old and the mean gestational age of pregnant women was 34.1±2.1 weeks. The mean parity of pregnant women was 2.3±1.4. Risk factors such as maternal age (P=0.011), number of pregnancies (P=0.035), maternal education (P=0.018), history of preterm premature rupture of membranes (P=0.046), history of preterm delivery (P=0.019), trauma (P=0.037) and pregnancy care (P=0.037) affected preterm premature rupture of membranes.
Conclusion: The results of this study showed that maternal age, number of pregnancies, maternal education, and history of preterm premature rupture of membranes, history of preterm delivery, trauma, and prenatal care are risk factors for PROM. Therefore, by educating pregnant mothers about these risk factors, the incidence and complications of preterm premature rupture of membranes can be reduced.

Abstract
Background: Psychosocial improvements of successful strabismus surgery have been reported in previous studies. But the effect of strabismus surgery on the emotional intelligence of patients is unclear. The aim of this study was to evaluate the effect of strabismus surgery on emotional quotient in patients referring to Imam Khomeini Hospital, Ahvaz.
Methods: This prospective interventional study conducted on patients underwent strabismus surgery in Imam Khomeini Hospital, Ahvaz during May 2021 to March 2022. A total of 38 patients between 8-37 years were included. Patients with previous surgery were excluded. All subjects were evaluated before and three months after successful strabismus surgery. Three months post-surgery the improvement of emotional quotient, self-esteem, body image, social anxiety and social avoidance were evaluated and compared with previous surgery. All data were analyzed by SPSS (V25). P-Value less than 0.05 was considered as significant level.
Results: 38 subjects including 18 males (47.4%) and 20 females (52.6%) were included. The mean age of patients was 18.11±9.33 years old. Our results showed that compared with before surgery, significant improvements were noted after surgery, within the emotional quotient subscales including self-awareness (P=0.016), self-regulation (P<0.0001), self-motivation (P<0.0001), empathy (P<0.0001), social skills (P<0.0001) and emotional quotient total score (P<0.0001). Further analysis revealed statistically significant improvements in body image (P<0.0001), social anxiety (P=0.004) and social avoidance (P=0.002). The result showed that the improvement of emotional quotient after strabismus surgery was significantly higher in the age group of less than 12 years (P<0.0001). No significant difference was fund between two genders for emotional quotient subscales (P>0.05).
Conclusion: The results of this study showed a significant improvement in the psychosocial factors including emotional quotient, body image, social anxiety and social avoidance after strabismus surgery. This result suggests that beyond functional and cosmetic improvements, successful strabismus surgery can result in improved emotional quotient and social anxiety, with the greatest effect noted in younger patients.

Abstract
Background: One of the factors affecting the outcome of pregnancy is the primary body mass index (BMI) and the amount of weight gain during pregnancy. This study aimed to check the relationship between mother's initial body mass index and weight gain in pregnancy with pregnancy outcomes.
Methods: This cross-sectional study was conducted on 455 pregnant women referred to Afzalipour Hospital in Kerman, Iran, from August 2021 to August 2022. These pregnant women were included in the study through easy and accessible sampling. The mother's initial weight was recorded through the mother's health card. The height of the mother was measured using a standard meter and the final weight of the mother before delivery was measured using a standard scale available in the department. Other information of the mothers was extracted from their records. Weight gain during pregnancy and initial body mass index were calculated and divided into four categories, less than normal, normal, overweight and obese. Maternal-fetal complications were included in the information registration form. Descriptive and analytical statistical methods and SPSS version 24 software were used to analyze the data.
Results: The average age of women was 27.56±6.82 years. Body mass index of more than 40% of them (44.4%) was normal (19.8-26) and more than 45% of them were overweight 11.5-16 (45.5%). There was a significant relationship between initial body mass index and weight gain during pregnancy (P=0.001), gestational diabetes (P=0.001) and newborn weight (P=0.019). There was also a significant relationship between weight gain during pregnancy with premature birth (P=0.001), vaginal delivery (P=0.001), gestational diabetes (P=0.001) and newborn Apgar (P=0.001).
Conclusion: High body mass index of the mother and weight gain during pregnancy can cause complications in the mother and the fetus. Therefore, prenatal care should be done more carefully and health care providers should place mothers who have abnormal body mass index and inappropriate weight gain in pregnancy in the high-risk group and under special care to minimize maternal and fetal complications.
