Abstract
Background: Excessive and irrational use of antibiotics in the treatment of acute diarrhea has caused increased resistance to these medications. It is well defined that most cases of diarrhea in children do not require the use of antibiotics. This study was aimed to determine the status of antibiotic administration for treatment of acute diarrhea in children younger than five years. All who admitted at the pediatric ward of Besat Hospital in Sanandaj.
Methods: First, the study was approved by the ethics committee of Kurdistan University of Medical Sciences. The archived files of all children under 5 years with a positive history acute diarrhea who were admitted in Besat Hospital of Sanandaj during the period of 1 years, from May 2016 to May 2016 were reviewed. Demographic information such as age, gender, type of diarrhea, type of nutrition, type of the prescribed antibiotic, results of the stool and blood samples were collected and recorded in the questionnaire.In Stool samples the contained a large number of WBCs and RBCs along with high fever, Shigellosis were considered. The results were analyzed by SPSS software, version 23 (SPSS Inc., Chicago, IL, USA).
Results: A total of 1,029 cases were reviewed. 60% were boys. The highest incidence of diarrhea (75.5%) was observed at 12-24 months and the lowest rate of diarrhea was under 6 months (11.5%). The frequency of diarrhea was lower in winter than in other seasons. Only 31.49% of children received proper and rational treatment with antibiotics. The most commonly used antibiotics in hospitalized patients were ceftriaxone (94.5%) and before hospitalization was cefixime (39.5%). Based on the results of this study, it was found that 66.13% of children younger than 5 years with acute diarrhea had appropriate treatment. 31.94% of patients had inappropriate antibiotic therapy.
Conclusion: In most cases of acute diarrhea in children, no evidence of bacterial or parasitic infection was found. However, a high percentage of patients received antibiotics without laboratory evidence (stool testing).

Abstract
Background: To date, different kinds of treatment methods have been proposed for radiotherapy of cancer patients. Choosing the kind of treatment method affects the quality of the patient's treatment. This study aims to investigate the effect of the number of radiation treatment sessions on the dose received by the patient and the distribution of tumor dose and dose received by organs at risk in breast cancer radiation therapy. These results help us to select the appropriate treatment schedules for the treatment of left breast patients.
Methods: This prospective cross-sectional study was performed on the treatment plans of 35 patients with left breast cancer who referred to Isfahan Milad Hospital between July 2019 and April 2020. They were candidates for left breast radiation therapy. Also, these patients had no history of surgery or chemotherapy, and no supraclavicular or axillary lymph nodes were involved. Patients were treated with a conventional fraction regimen (CF) or hypofractionated (HF) treatment schedule. Different dosimetry parameters for the target and organ at risks such as conformity index, homogeneity index and mean dose were obtained from the dose-volume histogram plot. Finally, the results of both plans were compared with each other.
Results: The data obtained from this study indicate a decrease in the average dose of all organs in the hypo fractionated regimens compared to conventional plans. The differences between two plans were statistically significant for tumor, lung, and skin (P=0.0). Moreover, the maximum dose for the skin was also reduced when hypofractionated regimens were used. However, the values of the homogeneity index and conformity index of tumor in the two methods did not show a significant difference (P were 0.99 and 0.86, respectively).
Conclusion: In general, the results of the current study indicate that the hypofractionated regimen leads to a reduction in dosimetric factors compared to conventional fraction plans. It seems that this method can be used as an alternative treatment plan for breast cancer radiation therapy due to the reduced duration of the treatment period. |

Abstract
Background: Prostate cancer is the most common malignancy in men and the second leading cause of death in all countries of the world. The exact mechanism of prostate cancer is not known. On the other hand, early detection of prostate cancer can lead to a complete cure. Several clinical experiments including Digital Rectum Examination (DRE), biochemistry such as Prostate Specific Antigen (PSA), and pathology such as Trans Rectal Ultra Sonography (TRUS) are used to assess the size and spread of prostate cancer. In this study, the relationship between mean serum PSA and Gleason score as a standard method in patients with prostate cancer was compared using the parameters extracted from DCE MRI.
Methods: This applied-fundamental study was performed on 90 patients with prostate cancer, according to McDonald's criteria who were referred to Shafa Imaging Center in Isfahan, from March 2020 to October 2020. Quantitative analysis is based on modeling the change of concentration of the contrast agent using pharmacokinetic modeling techniques. The pathologist then determined the Gleason score using anatomical landmarks (such as prostate urethra) in the same areas suspected of being cancerous. Existing commercial software captures DCE-MRI data and creates parametric maps such as Ktrans and Kep maps that can be used for diagnostic purposes.
Results: Kep and Ktrans maps showed a significant difference between healthy and cancerous tissue. Kep and Ktrans in prostate cancer were significantly higher than in healthy tissue (P<0.05). Pearson correlation coefficient was used to investigate the relationship between DCE-MRI parameters and histopathological findings. No significant relationship was observed between Gleason score and DCE MRI parameters.
Conclusion: DCE MRI parameters significantly improve the accurate diagnosis of prostate cancer and are useful and effective for diagnosis, management, and evaluation of men with prostate cancer, but should not be considered as a substitute for tissue biopsy.

Abstract
Background: Mesenchymal stem cells are non-hematopoietic stromal cells that are used in the treatment of many chronic and autoimmune diseases by modulating the immune system. Due to the limitations of using autologous mesenchymal stem cells, the use of allogeneic stem cells is a promising therapeutic approach in the treatment of immunological disorders. This study aimed to investigate the ability of allogeneic mesenchymal stem cells to induce Programmed death-ligand 1(PD-L1) expression on the surface of splenic lymphocytes and the role of this molecule in the mesenchymal stem cell-treated cells tolerogenicity.
Methods: This study was conducted from February 2019 to December 2020 in the department of Immunology of Mazandaran University of medical sciences. Mesenchymal stromal cells were isolated from the femur and tibia of C57 mice. C57 bone marrow-derived mesenchymal stem cells were co-cultured with allogeneic BALB/c splenic cells. After 72 hours, the expression of PD-L1 on the surface of splenic lymphocytes was evaluated by flow cytometry. Interferon-gamma (IFN-γ) and Interleukin-10 (IL-10) cytokine assay were done in the cell culture supernatant. Mesenchymal stem cell-treated BALB/c lymphocytes were then exposed to allogeneic C57 splenocyte as stimuli in the mixed lymphocyte reaction (MLR) and the rate of proliferation was assessed by CFSE.
Results: The amount of PD-L1 positive BALB/c splenic lymphocytes were significantly increased after allogeneic C57 mesenchymal stem cells exposure (P=0.001). The levels of IFN-γ and IL-10 cytokines in the supernatant of cell culture also increased significantly (respectively, P=0.0009, P=0.01). C57 splenocytes proliferation notably decreased after mesenchymal stem cell-treated BALB/c lymphocytes exposure compared to the group were cultured with naïve BALB/c lymphocytes (P=0.002).
Conclusion: Allogeneic mesenchymal stem cells are capable to induce of PD-L1 on the surface of lymphocytes. PD-L1 expression on mesenchymal stem cell-treated cells makes them less immunogenic than naïve cells. These tolerogenic cells can reduce allogeneic responses. It seems that PD-L1 plays an important role in mesenchymal stem cell immunomodulation

Abstract
Background: Early prediction of the outcome situation of COVID-19 patients can decrease mortality risk by assuring efficient resource allocation and treatment planning. This study introduces a very accurate and fast system for the prediction of COVID-19 outcomes using demographic, vital signs, and laboratory blood test data.
Methods: In this analytic study, which is done from May 2020 to June 2021 in Tehran, 41 features of 244 COVID-19 patients were recorded on the first day of admission to the Masih Daneshvari Hospital. These features were categorized into eight different groups, demographic and patient history features, vital signs, and six different groups of laboratory blood tests including complete blood count (CBC), coagulation, kidney, liver, blood gas, and general. In this study, first, the significance of each of the extracted features and then the eight groups of features for prediction of mortality outcomes were considered, separately. Finally, the best combination of different groups of features was assessed. The statistical methods including the area under the receiver operating characteristic curve (AUC-ROC) based on binary Logistic Regression classification algorithm were used for evaluation.
Results: The results revealed that red cell distribution width (RDW), mean corpuscular hemoglobin (MCH), mean corpuscular hemoglobin concentration (MCHC), and mean corpuscular volume (MCV) in CBC features have the highest AUC with values of 85.29, 80.96, 79.94 and 79.70, respectively. Then, blood oxygen saturation level (SPO2) in vital features has a higher AUC with a value of 79.28. Moreover, combinations of features in the CBC group have the highest AUC with a value of 95.57. Then, coagulation and vital signs groups have the highest AUC with values of 85.20 and 83.84, respectively. Finally, triple combinations of features in CBC, vital signs, and coagulation groups have the highest AUC with the value of 96.54.
Conclusion: Our proposed system can be used as an assistant acceptable tool for triage of COVID-19 patients to determine which patient will have a higher risk for hospitalization and intensive care in medical environments.

Abstract
Background: Magnetic resonance imaging (MRI) is a non-invasive imaging technology that shows detailed anatomical and pathological images. It is often used for disease detection, diagnosis, and treatment monitoring, in particular with neurodegenerative diseases, such as Multiple sclerosis (MS), Alzheimer's and amyotrophic lateral sclerosis. However, conventional MRI provides only qualitative information and cannot distinguish between myelin and axon destruction. One of the new methods in early detection for axonal injury is the DTI sequence, which can be used to observe and quantify the various dimensions of these plaques, including the direction of diffusion and average diffusion. In other words, in DTI, quantitative data from the image helps to estimate the physiological and pathophysiological information of plaques in the brain. This study aimed to investigate the relationship between DTI parameters including FA, MD, RD, AD, axon injury and severity of clinical symptoms in patients with MS.
Methods: This is a cross-sectional control case study that was performed in Isfahan Milad Hospital from December 2020 to August 2021. DTI imaging was performed on 41 patients with MS and 41 normal individuals, and DTI indices including fractional anisotropy (FA), mean diffusivity (MD), axial diffusivity (AD), and radial diffusivity (RD) in different areas of the brain were evaluated. A visual analog scale (VAS) was used to assess the severity of clinical symptoms in patients. The relationship between DTI indices and the severity of clinical symptoms and axon injury was analyzed by the Kolmogorov-Smirnov test.
Results: Correlation analysis showed that there is a significant correlation between DTI sequence parameters and the VAS visual analog scale (P˂0.05) and according to the positive values of the correlation coefficient, there was a positive and significant relationship between VAS and the mentioned parameters and with increasing parameters. The DTI sequence of the Visual Analog Scale (VAS) was significantly increased.

Conclusion: Indicators obtained from the DTI sequence can be used in the prognosis of the disease and the estimation of the severity of clinical symptoms during the patients' involvement.

Abstract
Background: Glioma is the most common primary brain tumor, and early detection of tumors is important in the treatment planning for the patient. The precise segmentation of the tumor and intratumoral areas on the MRI by a radiologist is the first step in the diagnosis, which, in addition to the consuming time, can also receive different diagnoses from different physicians. The aim of this study was to provide an automated method for segmenting the tumor and intratumoral areas.
Methods: This is a fundamental-applied study that was conducted from May 2020 to September 2021 using multimodal MRI images of 285 patients with glioma tumors from the BraTS 2018 Database. This database was collected from 19 different MRI imaging centers, including multimodal MRI images of 210 HGG patients, and 75 LGG patients. In this study, a 2D U-Net architecture was designed with a patch-based method for training, which comprises an encoding path for feature extraction and a symmetrical decoding path. The training of this network was performed in three separate stages, using data from high-grade gliomas (HGG), and low-grade gliomas (LGG), and combining two groups of 210, 75, and 220 patients, respectively.
Results: The proposed model estimated the Dice Similarity Coefficient (DSC) results in HGG datasets 0.85, 0.85, 0.77, LGG datasets 0.80, 0.66, 0.51, and the combination of the two groups 0.88, 0.79, 0.77 for regions the whole tumor, tumor core, and enhancing region in the training dataset, respectively. The results related to Hussdorf Distance (HD) for HGG datasets were 8.24, 9.92, 4.43, LGG datasets 11.5, 11.31, 2.23, and the combination of the two groups 7.20, 8.82, 4.43 for regions the whole tumor, tumor core, and enhancing region in the training dataset, respectively.
Conclusion: Using the U-Net network can help physicians in the accurate segmentation of the tumor and its various areas, as well as increase the survival rate of these patients and improve their quality of life through accurate diagnosis and early treatment.

Abstract
Background: Sever Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) in some patients leads to cytokine storm and causes hemophagocytic lymphohistiocytosis (HLH) secondary to this viral agent. HLH is a rare and aggressive disease that increases in children, and is caused by changes in the immune system that associated with high mortality. Consideing the current pandemic, early diagnosis and treatment is important. Herein, we report a case of a 14-month-old infant presented with COVID-19 and prolonged fever who was eventually treated with the diagnosis of HLH. The study has been done in Sanandaj in January 2020.
Case Presentation: A 14-month-old female infant with Covid-19, in November 2019 with prolonged fever lasting for 12 days, weakness and lethargy was admitted to Sanandaj Social Security Hospital, in November 2019. The infant did not have any history of illness, drug or allergy and had received vaccination according to the country's protocol. Later she was transferred to Besat Hospital in Sanandaj due to persistent fever and elevated liver enzymes. The patient had severe leukocytosis and hepatosplenomegaly in the second center. During the diagnostic measures for the patient and the rule out of other possible causes through imaging and laboratory investigations and also the evaluation of relevant criteria, HLH was suggested and treated with antibiotics (ceftriaxone, meropenem), IVIg, dexamethasone and pantoprazole. After that the fever decreased, then oral prednisolone was prescribed and a follow-up was recommended. She was discharged in good general condition.

Conclusion: The current Covid-19 pandemic causes infection of children and may lead to occurrence of secondary HLH disease. therefore, assessment of children with liver manifestations and resistant fever should be considered by performing a PCR test to avoid complications and consequences as much as possible by timely treatment. more studies should be done in the field of its various dimensions.
