Abstract
Cystic Fibrosis (CF) is an inherited disease that affects multiple organ systems. It is the most common cause of severe progressive lung disease and exocrine panceratic insufficiency. In our investigation 67 patients had CF. Of these, 79% had panceratic insufficiency and 92% had lung disease under 2 years age. Also 67% of patients were < 2 percentile and 28% between 3-10 percentile weight for age. Unfortunately 27% of them died because of respiratory problems

Abstract
The purpose of this study was to assess the prevalence of malnutrition and related factors that may affect the nutritional status of 0 to 59 month old children in the rural areas covered by the Birjand district health network. A total of 626 children were selected by random sampling. Data were collected using a questionnaires and face-to-face interviews with the mothers and by measuring weight and height of te children. The nutritional status of the children determination was based on the z-score (cut-off poing under-2Sd from median reference population). Based on the z-score and under 2-standard deviation from median of NCHS in rural areas of Birjand, 18.5%, 40.1% and 3% of children were suffering from past-and-present, and present, malnutrition, respectively. Therefore the highest prevalence of malnutrition was related to the past-malnutrition and the stunting was the main problem. The chi2 test showed a statistically significant relationship between the children's past malnutrition and fathers litarcy (P<0.001), also between the children's past and present malnutrition and mother's literacy (P<0.021) and past malnutrition (P<0.001). There was also significant relationship between, father's main occupation, per-capita income (P<0.0001, P<0.025 respectively) and children's past malnutrition in the cases under study. In this research, relationship between sex and prevalence of malnutrition in both peresent-past and past malnutrition was meaningful, (P<0.024, P<0.017 respectively). This study did not show a significant relationship between nutritional independent variables and nutritional status of the children.

Abstract
Background, Materials and Methods: In order to evaluation the newborn’s nutrintional status in Bojnoard, 566 healty, Single full term newborn were examined. In this examination, the indicators of weight for age, height for age, weight for height and head circumference for age in the beginning of birth were calculated and compared to National Center of Health Statistics Standards (NCHS), with making use of fifth percentile of reference society for malnutrition edge, was specified that, nutritional status in above newborn’s is better than reference society’s newborns. Results&Conclusion: The data analysis of this research showed that between mother’s near delivery weight (P = 0.037) and mother’s nutritional attitude (P= 0.004) and numbers of family( P = 0.006 ) with height for age, between newborn’s sex (P= 0.004), mother’s age(P= 0.038), mother’s near delivery weight(P = 0.042) and mother’s nutritional practice during pregnancy (P= 0.041) with weight for height and between mother’s age (P= 0.043), mother’s near delivery weight (P= 0.048), mother’s height (P= 0.025), mother’s nutritional knowledge (P = 0.046) with head circumference for age, there is significant statistic connection. Also there was a weak statistic connection between newborn’s nutritional status and grade of birth (P= 0.09), but there is no significant meaningful connection between wanted or unwanted newborn and mother’s nutritional knowledge.

Abstract
Background: Adolescence (10-19 years) is one of the most challenging periods in human development. A second period of rapid growth occurs during the teen years. Not much information is available on the effect of the socio-economic status on the outcome of nutrition education in teenage girls. Objective : The purpose of this study was to determine the effect of socio- economic status on the efficacy of nutrition education in promoting the nutritional knowledge, attitude and practice in first-grade guidance school girls in Tehran.
Material & Methods: A total of 300 students were selected from 11 guidance schools by simple sampling and divided into three groups: 1- guide-book, 2- group discussion, 3- control. The knowledge and attitude data were collected using pre-test and post-test questionnaires and the personal-socioeconomic data were collected using general questionnaires. The students in group 1 were given the guide-book and required to study it at home, while group 2 students were told to discuss among themselves the contents of the guide-book. The control group was given nothing to study or to do.
Results: Nutrition education did not have a significant effect on the increase in the attitude score in the students whose grade-point average and their mother’s education level were high. (p= 0.13, p=0.29). Only mother’s education level independently from the type of education was associated with the difference of knowledge score means (p =0.07).There was interaction between nutritional education and monthly pocket- money with regard to the increase in the knowledge score (p =0.03).
Conclusion: Overall, exception of monthly pocket money, the effect of education on the nutritional knowledge and attitude was independent from other variables. Only mother’s education associate with the difference of knowledge score means.

Abstract
Background: The aim of this study was to determine the effect of two different regimes, containing different carbohydrate to protein ratios on performance, serum glucose and insulin after exercise in soccer players in Tehran Iran.
Methods: Nineteen male soccer players under training [age = 17/5 +/- 1.5 (SE) yr] were selected and completed two sequential trials separated by 1 week, in a paired cross-over study design. In each trial, subjects after running to fatigue received one of three regimes, using a random- order design as follows: HPRO ( CHO 56%, PRO 19% & fat 25% of total energy) HCHO ( CHO 64%, PRO 11% & fat 25% of total energy) or control ( CHO 60%, PRO 15% & fat 25% of total energy). The calorie of 3 regimes were equal. After consumption of meal up to120 min, blood was obtained before and at intervals. After 3 hours athlete performance was measured.
Results: The study indicates no significant difference in the serum insulin and glucose response among three regimes (P > 0.05). There was no difference in performance between three regimes after 3 h (p > 0.05).
Conclusion: The results suggest that post exercise regimes have no influence on performance, serum glucose and serum insulin. Thus total energy content and carbohydrate content may be important in recovery after exercise.

Abstract
Background: The effects of population aging are becoming apparent throughout the world. Diseases, such as cardio-vascular disease (CVD) and diabetes, are among the most important factors affecting morbidity and mortality, especially in the elderly. These diseases not only result in huge economic costs for treatment and care, but also results in hardship and time lost for relatives of the afflicted individuals. The association between nutritional status and disease is well known. In the present study, the effects of both under-nutrition and over-nutrition on the prevalence of disease are monitored in an urban Iranian elderly population. Thus far, no similar study has been performed in the Iran.
Methods: This cross-sectional study was conducted in Isfahan, Iran, during late 2003 on 1694 elderly people (731 males, 963 females), aged 60 years and older. Subjects were randomly chosen from all urban elderly people during a door-to-door and weight and height survey. They were selected using a cluster sampling method, each containing 30 clusters. From each cluster, 58 elderly were selected at random. Using each subjects body mass index (BMI), the nutritional status was categorized as overweight (BMI greater than 25), underweight (BMI less than 19) and normal (having a BMI equal to or more than 19 and equal to or less than 25). Any illnesses known to each subject were also recorded.
Results: Results showed that 4.7% of the subjects were underweight and 61.2% overweight. Women were more likely to be overweight and long periods of watching television increased the risk of overweight in all subjects. Being overweight was associated with diabetes and coronary vascular diseases, and lean people were less likely to suffer from such diseases.
Conclusion: This study indicates a high prevalence of overweight among the Iranian elderly population, indicating the need for improvement in nutritional status in order to reduce the prevalence of diabetes and CVD.

Abstract
Background: Proper nutrition has an important role in the physical and psychological development of children. The aim of this survey is to compare the nutrition of a community to the recommended daily amount (RDA) to determine deficiencies and present recommendations for improvement.
Methods: In this cross-sectional study conducted in Tehran, Iran, 788 children, all seven years of age, were selected via cluster sampling and evaluated using standard questionnaires with a 24-hr recall regarding the frequency and type of food intake. Then by measurement of weight and height, their anthropometric and DMFT (decayed/missing/filled teeth) indices were determined using the Kruskal-Wallis one-way analysis of variance, and Mann-Whitney U and chi-square tests.
Results: Relative to the RDA, 23.6% of children had low intake of Ca, and 3.2% and 5.2% were deficient for Fe and Zn, respectively. Anthropometric measurements showed that 15.7% were malnourished based on weight for age, 10.5% based on height for age and 16.8% based on weight for height indices. The DMFT indices for children with dietary Ca levels under 75% of the RDA were significantly higher than those with sufficient Ca intake (P<0.001). The mean DMFT indices for dental development were 0.22 for permanent teeth (four molars SD=0.64) and 4.64 for primary teeth (SD=3.24). We found a significant correlation between DMFT indices and low Fe intake: the DMFT indices of children with iron deficiency were much higher than those with sufficient iron intake.
Conclusions: Children in Tehran do not receive enough Ca Fe and Zn from their diets. This problem causes mild malnutrition and increases the risk of dental caries.

Abstract
Background: Nutrition as an important factor in health can be effective in educational
success of students. Malnourished students have less concentration and attention. Lack of
breakfast consumption for a long period has negative effect in behavior and health. The
purpose of this study is to determine the effect of a nutritional teaching booklet about
breakfast on the knowledge of fourth grade girls at Tehran’s 6th educational and teaching
district.
Methods: This interventional study performed on 100 fourth grade girls from 10 schools in
Tehran city, randomly divided into two groups case who received booklet and control group.
Subject: At first a pretest of knowledge was done for all the students (case and control). The
case group was given the booklet to read by themselves. No teaching provided for the control
group. A knowledge posttest was performed two weeks later. Two types of questionnaires
(general and knowledge) were used to collect data. The following methods were used: chisquare
test for comparing case and control groups from the aspect of independent variables,
pair t- test for comparing the knowledge scores between case and control groups and one-way
ANOVA for assessment of independent variables effect on the mean difference of knowledge
in samples.
Results: The mean score of knowledge in the case group against the control increased 2.22
(p<0.001). The one-way ANOVA method showed that none of the independent variables
(family size, parents’ career and educational level, dinner breakfast sleeping and waking up
time, the person who prepares breakfast at home, breakfast consumption status in other
members of family) had significant effect on the mean of the knowledge scores’ difference.
Conclusions: The results of this study showed the nutritional teaching booklet about
breakfast caused an increase in the knowledge of this age group of girl students.

Background: Calcium metabolism disorders can be acute or chronic and chronic disorders can
cause different disease states such as dental problems.
Methods: In
this descriptive cross-sectional study done in Children's Medical Center affiliated
to Tehran University of Medical Sciences during 2005-2009,
all (93)
patients with hypoparathyroidism, nutritional rickets, hypophosphatemic rickets
and renal osteodysthrophy from the endocrinology and nephrology departments of
the Center were referred to a dentist there for orodental examination.
Subsequently, the frequency of dental problems including taurodontism, enamel
hypoplasia, dental abscess, dental caries and gingivitis were recorded and
analyzed.
Results: Nutritional rickets was the most common disorder in this study and delay in
dentition was the most frequent dental problem in the patients (61.9%).
Most cases of taurdontism and enamel hypoplasia were seen in patients with
hypoparathyroidism (33% and 50%,
respectively). Dental abscess, dental caries and gingivitis were more common in
patients with renal osteodysthrophia (50%, 90% and
20%, respectively). In addition, dental caries and
delay in dentition were the most prevalent disorders in this study (69.8%
and 49.5%, respectively).
Conclusion: According
to the above findings, it seems that effective screening, regular periodic
examinations, proper diagnosis and timely treatment of dental diseases are the main
principles of prevention of orodental problems. Moreover, dentists as well as pediatricians
should be aware of the features of the aforesaid disorders which lead to dental
problems so that early intervention could prevent subsequent serious and more
invasive dental problems.

Abstract
Background: Percutaneous endoscopic gastrostomy (PEG) is one of the ways to non-oral feeding in patients with dysphagia caused by conditions such as stroke, oropharyngeal malignancy and motor neuron disease. The aim of study was assessment of early outcome of PEG in hospitalized patients according to clinical situation and underlying disease.
Methods: This study was cross- sectional and prospective. Sixty five patients were included with PEG from April 2011 to July 2012, at Sina University Hospital, Tehran, Iran. Tube function, feeding, and patients' symptoms were assessed and patients were followed six month at least to one year after procedure .The follow up of patients were stopped if the patient died or removal of tube.
Results: Sixty five patients (33 men, 32 women) enrolled in this study. The mean age of patients was 57.45±16.83 years (19-94 years). Head trauma and postoperative thoracic surgery were in 15 (23.1%) and 4 (6.2%) cases. 36 of patients (55.39%) had no symptoms. The complications include lickage (11 cases), stenosis (9 cases), cellulitis (7 cases), and PEG separation (2 cases). A total of 29 patients were complicated and 21 patients (72%) of them complicated 2 -5 months after PEG. No statistical significant differences between two sexes in PEG complication. 27 patients (41%) died that have no relation to complications of PEG.
Conclusion: Finally, the study showed the PEG is a safe method to enteral nutrition. The advantages of this method include its simplicity, no need to general anesthesia and low rate of complications especially in inpatient under the supervision of medical staff. Therefore it can be used as a favorable method.

Abstract
Background: The effects of consumption of nutrients on coronary artery disease (CAD) are widely observed. The aim of this study is to assess the exclusive Iranian dietary pattern on coronary artery diseases (CAD) and its risk factors. Methods: In this description-cross sectional study, 801 inhabitants of Borujerd City were included by random stratification method and a questionnaire regarding micro- and macronutrients for subjects over 35 years old from March 2011 to February 2012 was filed out. Of all the subjects consent to participate were prepared by volunteers. The complete description and a standard questionnaire to record demographic and socioeconomic status of people were taken. The prevalence of coronary artery disease and its risk factors determined Based on rose questionnaire and Monica questionnaire respectively. Results: In this study mean age of the subjects were 54.8±1.2 years, 388 male and 413 female. There was no meaningful relation between red meat consumption and coronary artery disease (P= 0.117). Nut consumption was higher in patients with CAD (P= 0.028). Use of fresh and cooked vegetables, dried fruit, animal oils cakes, french fries and some dairies are correlated with CAD (P< 0.05). However, artificially flavored soda in contrast to ordinary soda was not associated with coronary artery disease. Conclusion: Although there are some differences between dietary pattern in patient with coronary artery disease, nutritional attitudes are not significantly different in normal and CAD patients. However, according to the importance of diet in CAD, balancing these attitudes should be on the educational system high priority.

Abstract
Background: Children's health, preserving and promoting it is a health priority of any society. Malnutrition affects all age groups, but malnutrition in children is one of the most common nutritional problems that can delay body growth, shortened height, frequent infections, mental retardation, the prevalence of mental disorders, lack of academic achievement and reduced efficacy. Because of the importance and impact of this issue in children, the purpose of this study was a systematic review and meta-analysis of the prevalence of underweight in Iranian children, in order to highlight the importance of this issue to health policy-makers.
Methods: A meta-analysis was performed for relevant articles in scientific databases including Scientific Information Database (SID), ScienceDirect, PubMed and Google Scholar. Entrance criteria included cross-sectional studies (March 1996 to February 2017) that had a low incidence of underweight in Iranian children in different parts of Iran. Non-relevant articles included review articles, interventions, cohorts and case studies, and studies that looked at obesity, overweight and the relationship between these factors and other diseases, excluded from the study list. Reference lists of identified articles were reviewed for additional articles. Heterogeneity of study was checked using I2 index and the possibility of publication bias by funnel plot and Begg and Mazumdar's rank correlation test and a significance level of 0.1. Data were analyzed using the comprehensive meta-analysis software, version 3 (Biostat, Englewood, NJ, USA).
Results: In 26 articles reviewed in the study, total number of participants was 142938 persons at the age of 1 month to 12 years. The overall prevalence of underweight in Iranian children was 15.5% (Cl 95%: 12%-19.7%) during 1996 to 2017. The highest prevalence of underweight in was recorded in Birjand city in 2006 and Zahedan city in 2009, 68.6% (Cl 95%: 63.3%-72.9%) and the lowest prevalence of underweight in was found in city of Jahrom in 2013, 1.8% (Cl 95%: 1.2%-2.9%).
Conclusion: According to the results of this study, the prevalence of low-birth-weight in the country is high and requires interventional measures.

Abstract
Background: Growth disturbance is a common phenomenon in children with congenital heart diseases (CHD). Malnutrition and nutritional disturbances have a higher prevalence among children with down syndrome, especially children with Down syndrome; on the other hand, the prevalence of CHD is higher among syndromic children, which needs surgical repair as the definitive treatment. The nutritional status plays an important role in determining the postoperative complications and recovery. The purpose of this study was to investigate the growth status of children with Down syndrome and congenital heart disease before cardiac surgery.
Methods: This study was conducted as a retrospective study by evaluating the records of all syndromic patients undergoing cardiac surgery at Children’s Medical Center of Excellence, Tehran University of Medical Sciences, Tehran, Iran, from March 2011 to March 2017. Age, weight, height, weight-for-age z-score (WAZ), weight-for-height z-score (WHZ), height-for-age z-score (HAZ), mortality and hospitalization rate in an intensive care unit (ICU) were recorded in these patients. The z-scores more than -1 were considered as normal, between -1 and -2 as mild malnutrition, between -2 and -3 as moderate malnutrition and below -3 as severe malnutrition.
Results: 35 (51.5%) patients were female and 33 (48.5%) were male. The mean age, weight, and height of these children were 26.9±24.9 months, 9.1±4.95 kg, and 79.55±17.95 cm, respectively. The mean of WHZ, WAZ, and HAZ in these children was -2.18+1.65, -1.95+2.25 and -1.22+3.11, respectively. Based on the values of WAZ, WHZ, and HAZ, 85.3%, 77.9% and 75% of patients have malnutrition (mild to severe forms, z-score less than -1). The most common cardiac defect was ventricular septal defect (VSD) accompanied by pulmonary arterial hypertension.
Conclusion: Considering the high prevalence of impaired nutritional status in these children and considering the effect of preoperative malnutrition on surgical outcomes, including mortality, assessing the nutritional status is much important. The adequate nutritional support in these patients leads to a reduction of the mortality, postoperative complications and morbidities.
