Abstract
Background: A variable hematological value without clinical sign of sepsis or hematological disorder is often observed in newborns. The purpose of this study was to investigate hematological measurements in umbilical cord vein of newborns delivered spontaneously or by cesarean section.
Methods: We studied 150 pregnant women who delivered term normal infants. The patients were divided into two groups according to the route of delivery: vaginal (n=75) and cesarean section (n=75). Immediately after delivery, umbilical cord blood samples were collected. Any possible relationship between mode of delivery, gestational age, gravidity, parity, sex, weight of the new born infant, and the duration of different phase of labor with the hematological parameters was investigated.
Results: We observed a significant increase in all hematologic parameters including hemoglobin and hematocrit levels and WBC, platelet and RBC counts in newborns delivered spontaneously (p<0.001). The lymphocyte counts were similar in two groups. Male infants and newborns with higher gestational age showed a higher hemoglobin and hematocrit levels. Increased parity and gravidity correlated with a decrease in all hematological values of umbilical vein blood except of lymphocyte count (p<0.001). Prolonged duration of first and second stage was associated with higher mean leukocyte, neutrophil, platelet, hemoglobin and hematocrit values.
Conclusion: The mode of delivery, gestational age, parity, gravidity, sex and weight of newborn, and the duration of first and second stage of labor can influence on hematological parameters of umbilical cord blood.

Abstract
Background: Accurate computation of the radiation dose to the breast is essential to mammography. Various the thicknesses of breast, the composition of the breast tissue and other variables affect the optimal breast dose. Furthermore, the glandular fraction, which refers to the composition of the breasts, as partitioned between radiation-sensitive glandular tissue and the adipose tissue, also has an effect on this calculation. Fatty or fibrous breasts would have a lower value for the glandular fraction than dense breasts. Breast tissue composed of half glandular and half adipose tissue would have a glandular fraction in between that of fatty and dense breasts. Therefore, the use of a computational code for average glandular dose calculation in mammography is a more effective means of estimating the dose of radiation, and is accurate and fast.
Methods: In the present work, the Sobol-Wu beam quality parameters are used to write a FORTRAN code for glandular dose calculation in molybdenum anode-molybdenum filter (Mo-Mo), molybdenum anode-rhodium filter (Mo-Rh) and rhodium anode-rhodium filter (Rh-Rh) target-filter combinations in mammograms. The input parameters of code are: tube voltage in kV, half-value layer (HVL) of the incident x-ray spectrum in mm, breast thickness in cm (d), and glandular tissue fraction (g).
Results: The average glandular dose (AGD) variation against the voltage of the mammogram X-ray tube for d = 4 cm, HVL = 0.34 mm Al and g=0.5 for the three filter-target combinations, as well as its variation against the glandular fraction of breast tissue for kV=25, HVL=0.34, and d=4 cm has been calculated. The results related to the average glandular absorbed dose variation against HVL for kV = 28, d=4 cm and g= 0.6 are also presented. The results of this code are in good agreement with those previously reported in the literature.
Conclusion: The code developed in this study calculates the glandular dose quickly, and it is complete and accurate. Furthermore, it is user friendly and useful for dose optimizing in mammography imaging.

Abstract
Background: Infertility is one of the most serious social problems. Illicit drug use can be an important cause of male factor infertility. Kerack which its use is rising up in Iran refers to a high purity street-level heroin (heroin Kerack). Heroin Kerack used in Iran is an opioid and has harmful effects on body organs. The aim of this study is to investigate the effects of Kerack used in Iran on fertility adult mice.
Methods: In this study, 25 male mice were divided into five groups (control, sham and three experimental). Experimental groups of Kerack-dependent mice (received ascend-ing dose of Kerack for seven days) were divided into three categories, experimental I, II and III. Experimental I was given Kerack at a dose of 5 mg/kg, experimental II 35 mg/kg and experimental III 70 mg/kg, intraperitoneally twice a day for a period of 35 days. The sham group received normal saline and lemon juice (2.6 µl/ml) whilst the control group just received water and food. Mice were then scarified and sperm removed from cauda epididymis were analyzed for sperm count, motility, morphology (normal/abnormal) and viability. Testes were also removed, weighed and processed for light microscopic studies.
Results: The results showed that fertility were significantly decreased in addicted mice compared with control groups (P≤0.05). Epididymal sperm parameters and thickness of seminiferous epithelium were significantly decreased in experimental groups (dose-dependent) compared with sham and control groups (P≤0.05). Gonadosomatic index was significantly reduced with high dose Kerack injected (70 mg/kg) in comparison with control testes (P≤0.05).
Conclusion: This study has shown the deleterious effects of Kerack used in addicted Iranian people on fertility for the first time. This effect is especially on epididymal sperm parameters in adult mice.

Abstract
Background: Shortness of breath is a common disorder and means difficulty in breathing and shortness and shallow breathing that the patient feels unpleasant, difficulty and shallow breathing. The main purpose of this study was to analyze patients undergoing pulmonary angiography with a prognosis of pulmonary embolism in the emergency department.
Methods: This descriptive-analytical study was performed in the adult emergency department of Shahid Mostafa Hospital in Ilam in March2018-March2020 after receiving permission from the ethics committee. This study evaluated patients over 18 years of age who underwent CTPA with a pre-diagnosis of PE.
Results: The highest frequency was reported in CT findings of 31.9% normal, 27.6% PE. In our study based on CT results, 30.2% of patients suffered from PE. In the present study, 65.5% of patients presented with shortness of breath, of which CT results showed 28.9% (PE), 38.2% (Non PE) and 98% (Normal). Other complaints of patients with shortness of breath were reported with CP (7.8%), lower edema (6%), respiratory distress (8.6%), CP and fever (5.2%) and hemotypy (1.7%).
Conclusion: Based on the above study, we conclude that the criterion for selecting patients for CT angiography to rule out PTE is the most important criterion for clinical examination and the Welsh Criteria score. As we can see, the results showed that patients with a Wales score above 8-12 had 80% PTE and patients with a Wales score of 3-6 had 23.5% PTE and the rest had a negative answer, so the Wells criteria were good criteria which unfortunately Due to some conditions, the diagnosis of a general practitioner or carelessness in examinations are not taken into account and causes a large percentage of patients with complaints of shortness of breath to undergo CT angiography while not having the necessary criteria based on Wells scores and clinical examination. CT angiography is either normal or another condition that does not require CT angiography and radiation.
