Abstract

Normal 0 false false false EN-GB X-NONE AR-SA MicrosoftInternetExplorer4 Background: Varicocele is a major cause of male infertility, but its pathophysiology is unclear. Recent studies declare that fertile varicocele people with normal semen analysis are also at risk of loss of infertility. The exact mechanism by which varicocele damages spermatogenesis is still unknown. Some studies have reported increased Reactive Oxygen Species (ROS) is a major factor in semen of men with varicocele. The aim of this study was to investigate whether the source of elevated ROS is intracellular or not. In addition, we studied Mitochondrial Membrane Potential (MMP), viability, antioxidant activity, sperm count and motility in these rats.
Methods: The study group consisted of 28 male rats divided in four groups: control, sham, varicocele 1, varicocele 2, Experimental varicocele was established by partial ligation of the left renal vein in last two groups. Animals were sacrificed two and six months after surgery and dilation of the internal spermatic veins was observed. Then, superoxide anion production and Mitochondrial Membrane Potential were evaluated by Flow cytometry sperm characteristics were evaluated by Flow cytometry. Sperm superoxide anion production was assessed by the dihydroethidium and Mitochondrial Membrane Potential with rhodamin 123.
Results: Our results showed intracellular superoxide anion production significantly increased and Mitochondrial Membrane Potential, viability, sperm count and motility significantly decreased in rats with experimental left varicocele. However, there was no significant difference for seminal plasma antioxidant activity between all groups.
Conclusions: Consequently, our findings suggest that one of the main sources of ROS production is intracellular and we must consider it in treatment.

Abstract

Background: Varicocele is a dilated vein of the pampiniform plexus that cause to det-rimental time-dependent effects so this study describes the effect of varicocele on the level of IL-6 and interferon gamma in serum and testis tissue, number of sertoli and spermatogonia cells, seminiferous tubules diameter and sperm activity in immature rats. Methods: Thirty six immature rats, 5-6 weeks aged were investigated in this study. The sham groups underwent sham operation and varicocele groups underwent partial liga-tion of the renal vein. Serum, testis and sperm samples were collected at 9, 11, and 13 weeks after induction of varicocele or sham operation to evaluate histological parame-ters (seminiferous tubules diameter, number of sertoli and spermatogonia cells), per-centage of sperm motility and viability and levels of cytokines. Testicular morphology was evaluated. Results: Varicocele significantly caused an increase in serum and testis IL-6 and inter-feron gamma, compared to related sham groups and previous varicocele groups (P<0.05). Varicocele significantly caused decreases in sertoli cells and spermatogonia cells number with increasing varicocele time, compared to related sham groups and previous varicocele groups (P<0.05). In the evaluation of seminiferous tubules diameter external, internal and epithelium diameter were decreased compared to sham related groups and previous varicocele groups. In all varicocele groups, all kind of sperm motility and viability decreased compared to the related sham-operated groups (P<0.05). Varicocele had deteriorating effects on testis tissue because our observations in varicocele groups demonstrated that the external, internal and germinal epithelium height was reduced by the time and in the evaluation of testicular cells, sertoli and spermatogonia cells number were decreased by the time compared to sham related groups and previous varicocele groups. Conclusion: This study suggests varicocele had a detrimental time-dependent effect on cytokines levels and decrease in sertoli and spermatogonia cells number, seminiferous tubules diameter and sperm indices.

Abstract

Background: Varicocele is recognized as the most common cause of male factor infertility and is found in 15% of the general population. This prevalence increases to 35% in men presenting with primary infertility and between 70 to 80% in men presenting with secondary infertility. The effect of varicocele on Leydig cell function and testosterone production has been always a question. In this study we examined the effect of varicocelectomy on serum testosterone. Methods: This research protocol was approved by the institutional review board at Royan Institute in infertility department and also this study has been done in Royan Institute (Tehran, Iran) during one year since September 2012 till October 2013. In this cross-sectional study, Serum levels of total testosterone in 79 men with clinical varicocele and in 70 fertile men who served as a control group were compared. Men aged 23–46 years with clinically palpable varicoceles as determined by physical examination were studied. Three to 6 months, testosterone levels were measured again after varicocelectomy, then testosterone levels were compared before and after varicocelectomy. Results: The mean of serum testosterone levels before surgery in infertile men with varicocele and fertile men were 590(230) vs. 583(237) ng/dl respectively. No statistically significant changes were noted in serum testosterone levels for any groups. Three month after varicocelectomy mean serum testosterone levels were significantly increased in infertile men with varicocele compared with preoperative levels from 590 (230) to 663 (242) ng/dl (P=0.009). Also the testis volume of patients were examined, which were divided into two groups included the men with testis volume less than 16 ml (<16) and more than 16 ml (≥16). Conclusion: In infertile men affected with clinical varicocele, varicocelectomy seems to have caused positive impact on the level of serum testosterone increase. It is thought that positive effect is probably caused by improvement of the Leydig cell functions which induce the increase of serum testosterone level.

Abstract

Background: Infertility is defined as failure to achieve pregnancy after one year of unprotected sexual intercourse. Infertility can be related to male or female factors. Varicocele is the most common cause of infertility in men that is correctable with surgery. The purpose of this study was to determine the effects of recombinant follicle-stimulating hormone (rFSH) on semen parameters in infertile men.

Methods: This randomized clinical trial was done on 96 infertile men admitted to the Women's General Hospital Mohebe-Yas from September 2014 to September 2015. Inclusion criteria were to include varicocelectomy for unilateral idiopathic varicoceles and consent to participate in the study. Allergy to the drug combination and patient dissatisfaction were exclusion criteria. Patients participating in the study were divided into two groups randomly, one group received recombinant FSH three times a week and the other group received a placebo (normal saline) in the same way. After three months, the improvement of semen parameters, including motility, morphology and sperm count as well as the complications were determined in both groups. The data were analyzed with statistical software SPSS version 13 (Chicago, IL, USA).

Results: A total of 96 patients were enrolled in two groups of 48 men and women both groups were matched in terms of underlying factors. The rate of improvement in the morphology and motility of sperm in the treated group was significantly more than the placebo group (P= 0.0001) but the changes in sperm count were not significantly different between the groups (P= 0.495).

Conclusion: In summary, based on the results obtained in this study, it can be concluded that recombinant FSH is effective on improving semen parameters in infertile men after varicocelectomy compared with a placebo group and its major impact is on the morphology and motility of sperm.

Abstract

Background: Post-acrosomal sheath WW domain binding protein (PAWP) is one of sperm factors related to oocyte activation and is expressed in elongating spermatid. Previously, the effect of high of testicular temperature in infertile men with varicocele on semen quality, sperm DNA damage, expression of genes and proteins were reported. In this study, expression of PAWP at RNA and protein levels, and also sperm DNA damage were compared between fertile and infertile men with varicocele.
Methods: In this case-control study, semen samples were collected from 35 infertile men with varicocele (grade II & III) and 20 fertile men referring to Isfahan Fertility and Infertility Center from January 2016 to September 2016. Ejaculated semen was obtained by masturbation into a sterile plastic container after 3-5 days of abstinence and was allowed to liquefy at room temperature. Briefly, sperm concentration, motility and morphology were evaluated using a sperm chamber (Sperm meter; Sperm Processor, Aurangabad, India), Computer Assisted Semen Analysis (CASA, Video Test, ltd: version Sperm 2.1, Russia) and Papanicolaou staining, respectively. In addition, DNA fragmentation, expression of PAWP at RNA and protein levels were assessed by real-time PCR, and Western blot technique, respectively. Microsoft Excel 2013 (Microsoft Corp., Redmond, WA, USA) and Package for the Social Studies were used to analyze data. We used independent-samples t-test to compare the mean value between different groups. Differences with values of P<0.05 were considered as statistically significant.
Results: Mean of semen volume, sperm concentration, percentage of sperm motility, and expression of PAWP at both RNA (P=0.0001) and protein (P=0.03) levels were significantly lower in infertile men with varicocele in comparison with fertile men. In addition, mean percentage of sperm abnormal morphology and sperm DNA fragmentation were significantly higher in infertile men with varicocele in comparison with fertile men (P<0.05).
Conclusion: Expression of PAWP as a protein involved in spermatogenesis and fertilization process, has decreased in infertile men with varicocele. In addition, sperm DNA integrity was disrupted in these individuals that can be considered as a main etiology of infertility.
