Abstract

Leishmaniasis affects 1.5-2 million people annually and has been recognized as a major public health problem by the World Organization.
In Iran the incidence rate of the disease among school children in Ardestan province was 3.2 per thousand. As no effective vaccine has been found yet, control of the disease depends on effective drug therapy. The pentavalent anitmonials (pentostam and glucantime) are the current drugs of choice, but their use has been associated with side effects and a certain degree of toxicity. Furthermore, resistance towards these medications is not uncommon. For these reasons we studied the effectiveness of Vinca major alkaloids in treating Leishmania major infections both in vitro and invivo. Vinca has been used for centuries as a traditional remedy in the treatment of various diseases. The effectiveness of different concentrations of Vinca on Leishmania major promastigotes was evaluated and the results showed a significant decrease in the number of promastigotes with higher concentrations and increasing exposure times (PO.005). Results of in vivo trials showed that intralesional injections of purified extracts in chlorofromic phase induced a significant decrease in the mean lesion diameter in Balb/c mice compared to the untreated group.

Abstract

The Intrauterine device (IUD) is very effective in preventing pregnancy, it is the cheapestform of birth control in the long run, allows almost immediate return to normal life andwork, and it can be used as an alternative to hormonal methods of emergencycontraception.This study was designed to assess factors causing IUD iscontinuation. It is a cross-sectional study among 343 women in Urmia in 2001 who had had lUD'splanted back in 1998.Within 3 years, more than 50 percent had theirlUD's removed. The 1-year IUD survivalrate was 62 percent and the 3-year rate about 42 percent.54.5 percent of users had at least one complication (P=0.000). The main complications were: Bleeding 28.7% , Pain 27.3% and Infection 18.5%.In cases where the device was planted by a midwife, the overall removal rate was 62.5%(p= 0.009). Pregnancy rate was 0.49% per 100 woman - year and the IUD expulsion ratewas 6.5%o.Bleeding is clearly the most common complication. If cases are chosen more carefully, the effective life of device will probably increase.

Abstract

Measuring of service quality is the basic prerequisite for improving quality. Patients&apos expectations and perceptions of service quality play an important role in the choice of hospital, loyalty to organization and behavioral intention. A first step towards improvement would be to determine areas of quality that are most defective. Studies show that consumers are in close contact with service providers, and they are involved in the service process. Therefore, this group can measure services quality better than any other group. This study was conducted to measure service quality based on patients&apos expectations and perceptions in Zanjan hospitals.
The cross-sectional design was used in this research. For data collection, SERVQUAL questionnaire was employed. To ensure the reliability of the questionnaire, the internal consistency reliability test (Cronbach alpha) was performed.
Findings: Cronbach alpha for service quality was .94. Results show that the assurance (knowledge, experience and ability to maintain patients&apos confidence and trust) was rated as the most important dimension. (SQ mean = -1.3). There were differences between patients&apos expectations and perceptions in all the dimensions.
the assurance dimension constitutes the most serious problem facing hospitals. It is recommended that physicians thoroughly explain the disease condition to patients, and that patients should be treated with dignity and respect.

Abstract

Evaluation assures the success of each project, program or activity. It is one of the most important components of any management process. Because of its high importance, loss of an effective evaluation mechanism in a hospital not only increases costs, but also endangers community health. The current study tries to assess the perceptions and expectations of hospital managers and decision-making boards in Hamedan regarding the evaluation system and to determine gap between them. This is a cross-sectional study. Its target group in each hospital includes: head, manager, matron, emergency administrator, managers of medical records, laboratory, radiology, pharmacy sections plus the afternoon and night shift supervisors. The study took the form of a census, involving a total of about 400 subjects.The data were collected through a questionnaire and analyzed using the SPSS program . Findings: 1. The highest levels of target group expectation regarding evaluation indices were related to the “patient safisfaction” index. 2. The highest levels of target group perception of the evaluation indices were related to the “ values and norms” index. 3. All the gaps detected between target group’s perception and expectation from evaluation indices were significant. 4.The highest percentage of difference between target group’s perception and expectation was related to the “staff satisfaction” index and the lowest one was related to the “values and norms” Index.

Abstract

Background and Aim: Problems related to conventional Fenton oxidation, including neccesity of having a low pH and production of considerable amounts of sludge, have prompted researchers to consider chelating agents to improve the pH operating range and iron nano-oxide particles to reduce excess sludge. The main objective of this study was to remove pyrene from contaminated soils by a modified Fenton oxidation method  at neutral pH.

Materials and Methods: Experiments were conducted using various concentrations of H2O2 (0-500 mM), iron nano-oxides (0-60 mM), reaction times (0.5-24 hours) and several chelating agents, namely, sodium pyrophosphate, ethylene diamine tetra-acetic acid, sodium citrate, fulvic and humic acids, to eliminate pyrene from soil (concentrations of 100-500 mg/kg).

Results: The efficiency of removal of pyrene at an initial concentration of 100 mg/kg was 99 % at the following reaction conditions: H2O2 and iron nano-oxide concentrations of 300 mM and 30 mM, respectively pH=3 and a reaction time of 6 hours. The initial pyrene concentration of 100 mg/kg decreased to 7 mg/kg at optimum conditions using sodium pyrophosphate as the chelating agent at pH 7.    

Conclusion: The modified Fenton oxidation method, using iron nano-oxide at optimum conditions as defined in this research, is an efficient alternative for chemical remediation or pre-treatment of soils contaminated wih pyrene at neutral pH.

Abstract

Background and Aim: Considering the lack of attention to the health promotion of adolescents due to their perception of being healthy in most countries of the world, we aimed to evaluate the predictors of adolescent health promoting lifestyle through path analysis based on IMB model.

Materials and Methods: This is a descriptive-analytic study on 400 high school students in Andimeshk city in two urban and rural areas. The sample size was obtained using the Cochran formula.They were selected by classified random sampling. In this study, demographic data, information about IMB model structures and student adherence to healthy lifestyle were collected in four areas nutrition, physical activity, stress control and responsibility. Correlation between total structures was analyzed using SPSS-23 software. Lisrel 8.5 software. Meanwhile, the relationships between variables were analyzed using Path analysis to fit and approve model fit model using LISREL8.8 software.

Results: All structures of IMB model had a Significant relationship with adolescent healthy lifestyle. Self-efficacy structure had the most predictive power. In general, self-efficacy variables, 75%, motivation 66%, and 50% knowledge had a positive and significant effect on behavior. This model was able to explain 71% of the variance in adolescent lifestyle behavior. Also, in this model, the level of self-efficacy variance (behavioral skills), 52%,, And motivation variance was explained 57%. Motivation and knowledge indirectly influenced the lifestyle behavior (behavior) by 55% and 50%, respectively.

Conclusion: The findings of this study showed that the IMB model has a good application for explaining and predicting the healthy lifestyle of adolescents. Accordingly, it is recommended that health planners apply the IMB model in order to promote adolescent lifestyle in this population.

Abstract

Background and Aim: In recent decades, the use of the Internet as a suitable and affordable means of communication has become increasingly popular among people, especially young people. However, the excessive use of this technology has led to negative physical, behavioral and psychological consequences, causing concern in many families and even in authorities involved in the educational system. Therefore, the purpose of this study was to investigate the association of family emotional atmosphere, self-control and sleep quality with Internet addiction in teenagers.
Materials and Methods: This research was a cross-sectional and descriptive correlational study. The statistical population was all the 300 male students in the 4th to 6th grades of two non-governmental high schools in Rasht city, Iran in the academic year of 1401-1401. A sample of 169 was selected based on the Morgan's Krejci table and using the available sampling method. The data collection tools were the Young's internet addiction, Pittsburgh sleep quality, Tanji self-control and Nodargah family emotional atmosphere questionnaires.
Results: The analysis of the data showed that there is a significant positive correlation between Internet addiction and family emotional atmosphere (r=0.597) and sleep quality (r=0.331), while a negative and significant correlation exists between self-control and Internet addiction (r=0.668). Furthermore, the regression test results showed that all the three variables, namely family emotional atmosphere, self-control and sleep quality, can predict the changes in the internet addiction variable, such that per one standard deviation change in the familial emotional atmosphere, self-control and sleep quality a change of 0.163,  ̶ 0.382 and 0.375 deviation occurs in Internet addiction. Finally, simultaneous regression analysis showed that 51% of the students' internet addiction level can be explained by family emotional atmosphere, self-control and the students' sleep quality.
Conclusion: Based on the findings it is concluded that the variables family emotional atmosphere and sleep quality have a direct positive, while self-control has an inverse negative, relationship with Internet addiction. In addition, the family emotional atmosphere, high self-control and good sleep quality are effective in managing the use of the Internet by teenagers, which would mean that the proper awareness of parents and school counselors of the subject can help prevent internet addiction in teenagers.
