Abstract

Owing to importance of employing native and traditional medicinal plants with good efficacy against malaria parasites, an ethanolic extract of Peganum harmala L. was tested on Plasmodium berghei in sourian mice and a comparison was made with the effect of chloroquine on the same parasite. In this study 56 sourian mice were divided into 8 groups, each consisting of seven animals. The first 7 groups were infected with P. berghei and the last group was used as control. The first seven groups were given chloroquine, Peganum harmala at four different concentrations (20, 100, 300, 600 mg/kg) and placebo, respectively and the seventh group did not receive any treatment. The evaluation was done by Rane test. In each group the level of parasitaemia was determined on days 4 & 7, and compared with values from Day 0 (just before treatment) in order to record the decline in parasitaemia in treated groups. Results were analysed using SPSS soft ware and one-way analysis of variance (ANOVA). The results indicated that, although all the four concentrations of the Peganum harmala extract significantly reduced parasitaemia in the infected subjects, the 100 mg/kg solution showed optimal effectiveness on the parasites in comparison with other concentrations and the no-treatment option. We conclude that although the ethanolic extract of peganum harmala is not as effective as chloroquine in reducing parasitaemia, it can nonetheless cause a significant decrease when compared to control and placebo groups.

Abstract

Human Enteroviruses replicate in gastrointestinal tract and are excreted to the sewage system through feces, so isolation of Enteroviruses from sewage can be considered as a sensitive indicator for virus cirulation in society. They are originally given the name of Enteroviruses, but the inadequacy of this term became apparent when some Coxackie and Echoviruses were also found in acute respiratory infections. Therefore, these viruses can produce acute or paraclinical infecions, the shedding of virus is more than 1010 virus per each gram of feaces. In this study, 63 sewage samples were obtained from the 6 main sewage disposal systems in Tehran by grab sampling: Direct, Pellet, Two–phase methods in 2 sensitive cell lines (Hep2 & RD) and neutralization test were used to determine Enterovirus circulation in one year. None-Typable Enteroviruses, E11 and E25 were isolated more frequently than other Entroviruses. Out of 63 sewege specimens, we isolated 13 (20.63%), 25 (39.68%) & 27 (42.83%) Enteroviruses by Direct, Pellet and Two-phase methods respectively.

Abstract

Background and Aim: Iran National Polio Laboratory (NPL) is a member of the World Health Organization (WHO) Polio Laboratories Network. NPL receives stool specimens from acute flaccid paralysis (AFP) cases from all the provinces throughout Iran for poliovirus detection and identification. Furthermore, the NPL also detects non-polio enteroviruses (NPEVs) in these specimens.  Recently, NPEVs have come to be believed to be one of the most important causes of AFP following wild poliovirus.  This paper reports the prevalence of different types of NPEVs isolated from the specimens of AFP cases between 1995 and 2000.

Materials and Methods: Stool collection, virus detection and serotype identification were performed according to the WHO standard procedures.

Results: A total of 2180 stool specimens from AFP cases were received at the National Polio Laboratory. Coxsackie B viruse and echoviruses 6, 11, 7 and 13 had the highest frequency, identified in 23.7%, 14.4%, 12.7%, 11% and 10.2% of the NPEVs isolated from AFP cases, respectively. Four cases of echovirus 20 were identified, in 2 cases the patiets having died and in one the patient having been afflicted with residual paralysis. There have been no reports of death or residual paralysis (paralysis continuing after 60 days) due to echoviruse 20.

Conclusion:  Considering the upward trend of AFP cases in Iran, even after wild poliovirus eradication, studies are needed to determine the frequency and type identification of NPEVs and the relationship between NPEVs and residual paralysis in the post-eradication era (2000 onwards).

Abstract

Background and Aim: Malaria remains an important vector-borne disease globally and is a threat for human life. Forty percent of the world’s populations who are living in low-income countries are at risk of malaria. The disease exists in Iran and caused economic and social damages. As result of malaria control program that has been done during the past years, the disease is eliminated from the most parts of the country, so that it is only reporting from a small part in these years.

Materials and Methods : During this study, all available papers, books and thesises were reviewed and articles from Iranmedex, DIS and PubMed databanks were also used. Furthermore the related reports from different sources were noted.

The extensive studies have important information about malaria vectors. In this study the data about malaria vectors and related training courses are listed. During this study the related papers, Books and thesises which have been reviewed.

Although efforts, surveillance system, diagnostic and treatment facilities, as well as knowledge and attitude of peoples regarding to health behavior are improved nowadays, there are significant improvements about decreasing the malaria cases. Risk of the disease exists because of population exchange and asymptomatic cases. The malaria can be studied with both public health and economical aspects. This paper represents entomological studies of malaria during 1935 by the end of 2008.

Conclusion: our study revealed that, based on recent malaria national program, the authorities should make an emphasis on vector control monitoring, resistance management, malaria evaluation and because of weak supervision on all malaria operation at stage of elimination of malaria, accurate and careful suppersional require to reach the objective and goal of elimination.

Abstract

Background and Aim: Expanded program on immunization is one of the strategic universally accepted methods for control of childhood diseases including poliomyelitis. Oral Polio Vaccine (OPV) which consists of live attenuated poliovirus is considered as appropriate and most convenient tool for nation-wide vaccination. Polio virus is sensitive to heat, so OPV should be kept frozen and transferred to vaccination centers under cold chain conditions. Thermo-stability of vaccine during transportation is very important. Potency test is used to evaluate the quality and stability of vaccine. This is the first documentated study on evaluation of OPV potency kept out of cold chain conditions in Iran. Materials and Methods: To study the effects of time and temperature on potency of polio vaccine, vaccine vials were exposed to 24°C (room temperature) and 37°C (average temperature in tropical regions) for one to seven days. Vaccine potency evaluation was performed according to World Health Organization protocol. Results: It can be inferred from comparison of the results of this study with the international standards that OPV is stable at 37◦C for only one day, but if it is exposed to room temperature for 4-5 days, serotypes 1 and 2 remain unaltered but serotype 3 will lose its potency to a great extent. Conclusions: Although Polio viruses are relatively resistant to environmental conditions, their sensitivity to heat is the reason to transport the vaccine, which contains live attenuated virus, under cold chain conditions. This will prevent the titer of the vaccine virus to decrease. Vaccine stored at temperature below 4°C is preferred, otherwise the vaccine kept at room temperature (24°C) is useable for 4-5 days, and at 37◦C the vaccine is potent only for one day.

Abstract

Backgrond and Aim: Demodicosis is a common skin disease. Great number of admissions to the clinics is occurring in the country. Regarding the high prevalence of this parasitic arthropod, investigating on pathogenesis, route of transmission and the complications they may produce, is considered of great importance. Generally, demodex parasites often exist in the skin tissue and tend to live in the face specifically in cheeks, forehead and nose, where sebum excretion is active and may provide a favorable habitat for living and breading the parasite. Diagnosis is carried out during an exploratory skin test for a cne, pityriasis rosacea and ophthalmic infectious diseases such as blepharitis. Determination of their biological role in chronicity of general skin disorders is an interesting topic in research.

Materials and Methods: Patients were admitted to the Razi hospital, Research Center for skin diseases and leprosy, and three other clinical laboratories considered for assays. The questionnaire was filled for each patient and the sample was taken from the affected area. Ecto parasites were transferred to the potassium chloride solution 10% and studied under a light microscope.

Results: A total of 100 patients were studied, from which 78 patients were diagnosed as infected with the Demodex folliculorum. Of these, (%92) were female and (%7.6) male (p <0.05). The highest frequency was observed in the age group 41 to 50 years (P <0.05).

Conclusion: In this study, the relative frequency of demodicosis was more common in women and in the middle-aged group. Generally this high level of frequency will intensify the necessity of the identification of parasites and its pathological effects in chronic infectious diseases.

Abstract

Background and Aim : Asymptomatic malaria is a great challenge in the control, elimination and eradication programs of the disease in the endemic areas. The infected individuals with asymptomatic malaria are not cured and are, consequently, a potential source for contamination of the mosquito vectors and spread of the disease in the area. Therefore, detection of asymptomatic infected people is very important as regards combating the disease. This study was conducted to determine the presence and prevalence of asymptomatic malaria in Jask district, Hormozgan Province, Iran during 2012-13, in the hope that the results will help in designing strategies to eliminate the disease in the area.

Materials and Methods: A total of 200 persons under coverage of health centers in Jask district were selected randomly and enrolled in the study. From each subject a 5-ml blood sample was taken in 3 occasions (total number of samples = 600), slides p repared and examined using microscopic and molecular (PCR) methods, as well as rapid diagnostic (RDT) tests.

Results: None of the 600 slides prepared microscopically showed any positive malaria case. Neither did any of those prepared by RDTs or Nested-PCR.

Conclusion : The findings of this study indicate that implementation of the malaria control program has been successful in the area therefore the malaria elimination program should continue.

Abstract

Background and Aim: Considering the ongoing national malaria elimination program in Iran, establishing a bank of human Plasmodium genes and proteins can be very useful for research purposes. This study was conducted to collect some of the native isolates of human Plasmodia from endemic areas in the country.

Materials and Methods: A 2ml vein-punctured blood sample was prepared from each confirmed malaria case. The samples were dispensed in EDTA pre-dosed tubes and cryopreserved for further tests. Moreover, relevant Geimsa-stained thick and thin blood smears were kept in a safe place. Tests for genetic indicators of MSP-1 was performed for each of the P. vivax samples with the RFLP-PCR techniques. In addition, an in vivo drug sensitivity test was performed for each P. falciparum case. Collecting and cryopreserving samples will continue.

Results: A total of 131 samples, including 109, 19 and 3 P. vivax, P. falciparum and mixed samples, respectively, were preserved with relevant data such as species, parasitaemia and nationality of the donor. MSP-1 gene classification resulted in three different haplotypes including Hap.1, Hap.2 and Hap.3 with frequencies of 20.6%, 41.2% and 38.2%, respectively. The In vivo drug sensitivity tests on P. facilparum isolates showed that all of the isolates were sensitive to the current drug of choice, namely, a combination of artesunate and fansidar.

Conclusion: This study resulted in the preservation of considerable amounts of P. vivax and P. falciparum samples for further relevant studies and research purposes.

Abstract

Background and Aim: Malaria is still one of the major health problems in comparison with any other parasitic disease in Iran with considerable economic and mortality consequences. Sistan-and-Baluchestan, Hormozgan and Kerman are the most affected provinces in the country approximately 96% of the cases are reported from these three provinces. The aim of this study was to determine the frequency, distribution and rate of parasitaemia of human Plasmodium (P.) species in patients infected with malaria parasites in Kerman province.

Materials and Methods: A total of 92,798 peripheral blood smears were collected from suspected malaria patients during the period 2009-10. Thin and thick blood smears were prepared according to the World Health Organization (WHO) standard procedure. Percentage of parasitaemia was determined based on the number of parasites in the positive slides. The Chi-square test was used for data analysis.

Results: A total of 571 samples were found to contain human Plasmodium species, including 523, 44, and 4 cases of P. vivax, P. falciparum and mixed infection, respectively. The results also showed that, as compared with the previous year, the total number of P. vivax cases

decreased in 2010 by 33.96%. The highest level of parasitaemia was observed in one of the patients infected with P. falciparum, with 77240 parasites/µl of blood, and the lowest in a patient infected with P. vivax, with 48 parasites/µl of blood. There were no differences between the positive and negative cases as regards parameters such as nationality, habitat or gender (Chi-square, p<0.05).  Furthermore, based on the Mann-Whitney test, there was no significant difference between the mean counts of P. falciparum and P. vivax (p-value = 0.464).

Conclusion: Considering that Iran is in the elimination stage of malaria, patient finding and rapid, timely diagnosis of the disease are very important, particularly cases coming from Pakistan and Afghanistan, helping sustainability of the elimination program.

Abstract

Background and Aim: Medicinal plants provide an excellent source of drugs and new drug combinations, including the basis for the development of drug resistance to common anti-malarial treatments. In this study we investigated and compared the effect of an alcoholic extract of saffron and its constituents with that of chloroquine on malaria in mice.

Materials and Methods: A total of 65 male mice similar as regards gender, weight and age were randomly divided into 13 groups of 5 each. Eleven of the groups were infected with Plasmodium berghei and treated with saffron extract and its constituents and chloroquine based on the proposed method of Ryley and Petrs. Upon detecting the parasite in peripheral blood of the infected mice, they were treated with an aqueous, alcoholic or ethyl acetate solution at a dose of 350, 700 or 1050 mg/kg body weight (BW), chloroquine at a dose of 20mg/kg BW, and an iron chelator (50mg/kg BW). The solutions with the most effective concentrations were determined. Oral treatment at every stage lasted for up to 4 days. Parasite reduction in the groups treated with the extract was determined on days 4 and 7 and compared with that in the control, placebo and chloroquine groups. The Group 13 mice had no parasite injection and received no drug and were kept in the animal house only to control accidental death.

Results: Data analysis (t-test; SPSS) showed the most effective concentrations to be 700mg/kg ethyl acetate solution and 350 and 700mg/kg saffron extract, causing a significant reduction of parasitemia in the infected mice on day 7 (p<0.05).

Conclusion: The findings of this study indicate the potential effect of the alcoholic extract of saffron on Plasmodium berghei and, therefore, should receive proper attention.
