Abstract

Background and Aim: Cholera is one of the most important endemic and epidemic diseases in our country. Therefore, keeping track of the changes in antibiotic resistance patterns and continuous genotyping and serotyping of the isolates is essential.
Materials and Methods: One hundred stool samples were collected from the patients with cholera in different cities, including Tehran, Kashan, Kermanshah and Ahwaz. The specimens were subjected to a series of diagnostic tests. Strains were identified by serotyping and their antibiotic resistance profiles were defined through susceptibility tests. We also used a polymerase chain reaction (PCR) assay to identify the hly A gene.
Results:
Group-specific antisera were used for identification of Ogawa, Inaba, Hikojima and NAG (Non-agglutinable ) strains, which constituted 74% , 3% , 0% and 23% of the isolated strains respectively. Antibiotic susceptibility tests showed that all the strains were sensitive to ciprofloxacin, gentamicin and doxycyclin, but there was widespread resistance to Co-trimoxazol (74%), erythromycin (64%) and tetracycline (53%). Vibrio cholerae El tor isolates were 100% positive for hly A gene amplification, but hemolytic activity was seen in 95% of the cases on blood agar.
Conclusion: Compared to previous studies, there appears to be a higher degree of resistance to tetracycline, hence there may be a need to switch to other agents for the treatment of cholera. The predominant serotype and the antibiotic resistance pattern in Kashan were different from those seen in other cities.

Abstract

Background and Aim: Domestic violence against women is a major health problem, as well as a violation of human rights. It adversely affects the physical, mental, and social well being of women, families and communities. This phenomenon exists in all countries around the world. Although its prevalence, strength, types, effects, and risk factors are different, it is not limited to any geographical zone or distinct social class. This article presents the views of Tehrani couples and experts on underlying factors causing domestic violence against women in Iran and action to be taken for prevention.
Materials and Methods: This qualitative study was carried out through eight FGD meetings, in 2 days, with the participation of 50 couples. The couples were divided into 8 groups based on sex, education, and history of violence. Results of the FDGs were categorized and analyzed. Data were discussed in group meetings of 50 experts in different fields, such as health, medicine, law, education, and Islamic studies, from academic, policy making, governmental executive, and women's affairs organizations, divided into three groups of community general education and information dissemination, health system, and judiciary system. Results were discussed and finalized in a general meeting.
Results:
The couples and experts identified underlying factors in 4 categories socio-cultural, economic, legal, and medical. Actions to be taken for prevention and control were believed to be educational, legal, and supporting services.
Conclusion:
On the basis of the findings of this study, it was suggested that strategies/actions/services to solve the problem and reduce harm should include providing appropriate information and education for couples and the community emphasizing the health system responsibility empowering physicians and other health personnel to help women who are victims or susceptible to domestic violence legal transparency and reforms meeting legal needs law reinforcement and implementation providing safe shelters for victims and establishment of a national center for research and planning on violence.

Abstract

Background and Aim: Domestic violence against women is a serious public health problem and women adopt different solutions to fight against it. This study was carried out to determine the prevalence of intimate partner violence and views of women
on adopting ways to fight against it in Arak city, Iran, year 2008.

Methods and Materials: In a cross-sectional study in Arak city in 2008, a total of 1037 women were interviewed. The data collected were analysed statistically.

Results: Findings showed that the common types of abuse were emotional (64.6%), verbal (57.1%), financial (32.3%), and physical (26.8%). The women mentioned that the most efficient ways they chose to counteract the abuses were as follows: resuming talking after the settlement of quarrels (93%), keeping silent on the part of one of the partners (81%), and seeking advice from the older members of the family (81%). The considered going into a temporary sulk (51%) and reciprocation (46%) ineffective methods. Correlations were found between violence on the one hand and education, job, addiction of the spouse, income of the spouse, mental disorder of the partner, and number of children on the other hand (p<0.05).

Conclusion: Results of this study show that a large number of women are subjected to different types of violence. It seems that increasing awareness of married women and girls concerning effective counteractive methods can be an effective step in solving this social problem.

Abstract

Background and Aim: Currently there is no standardized and validated questionnaire for assessing the care of danger signs in children less than 5 years by the mothers based on behavioral patterns. The aim of this study was to develop and evaluate psychometrically an instrument for assessment of the care of danger signs in children less than 5 years old by the mothers based on the Health Belief Model.

Materials and Methods: This cross-sectional study was carried out on 350 mothers in the City of Arak, Iran. After reviewing the related literature, an instrument was designed and its content validity ratio (CVR), content validity index (CVI) and face validity were measured. To measure the reliability of the scales, the internal consistency method was used. Data analysis was done using the SPSS software.

Results: The scores of the CVI and CVR of the 81 items were found to be 0.79 and 0.62, respectively, the items being classified into seven categories. Based on the Cronbach's alpha, the overall reliability was 0.86. The reliability of the questionnaire for checklists of performance was 0.99, followed by those of perceived benefits (0.90), severity (0.83), perceived barriers (0.68), knowledge (0.66), perceived susceptibility (0.65), self-efficacy (0.63), and cues to action (0.56). Conclusion: The output of this psychometric process study is a valid and reliable instrument for measuring symptoms of risk behaviors in the care of vulnerable children based on the health belief model constructs, which can be used in health sciences research aiming at helping to protect the health of children.

Abstract

Background and Aim: Diabetes mellitus is one of the most prevalent and important health problems in the world.Considering the high prevalence of metabolic syndrome in diabetic patients and the potential effects of its components on HbA1C in these patients, the present study was conducted to determine the effect of metabolic syndrome components on HbA1C in type-2 diabetic patients.
Materials and Methods: This retrospective cohort study was conducted on 157 diabetic patients regularly referring to the Diabetes Control Center in Qazvin City during 2014, where complete data about theirmetabolic syndrome components, as well as HbA1C levels were available. The data were analyzed using multiple linear regression (α=0.05 for statistical significance).
Results: Based on the regression model, the effects of age, HDL-C and fasting blood glucose on HbA1C were statistically significant; the effects of other variables were nonsignificant. The R2value for the variables entered for prediction of HbA1C was found to be 0.45 (R2 = 45%).
Conclusion: Considering the significant relationship between some components of the metabolic syndrome such as HDL-C and fasting blood glucose on HbA1C in diabetic patients, it is essential to design and implement programs to reduce the level of HbA1C in these patients.

Abstract

Background and Aim: Accreditation is one of the important measures in the evaluation of health services. Today, the issue of accreditation has been established with the aim of creating and promoting a safety culture and quality of patient care. The purpose of this study was to determine the effect of accreditation on key and performance indicators of public hospitals in Zahedan before and after its implementation.
Materials and Methods: This descriptive-analytic cross-sectional study was conducted in 1396 in governmental hospitals of Zahedan University of Medical Sciences (Khatam-ol-Anbia and Ali ibn Abi Talib, AS). All of the key indicators of hospital performance were evaluated two years before the implementation of the accreditation (years 1391-1392) and two years after the implementation of the accreditation (1394-1395) in these hospitals. Data was collected using a form designed for study purposes. Descriptive statistics, tables and descriptive charts were used to draw information.
 Results: Based on the results of the study, the duration of stay of patients after the implementation of the accreditation scheme in hospitals has improved somewhat and decreased by 0.5 days to 4 days in 1395. The rate of cesarean delivery has risen from 49.2% in 1391 to 56.8 in 1395. The percentage of flat occupancy has risen to over 90 percent in 1395. The bed rotation distance was reduced by 31.1%, and releasing by personal desire in 1395 was 9% lower than in 1394. The personal satisfaction score of the emergency releasing by personal desire also dropped by 7%. It should be noted that there was no change in the net mortality index before and after the validation.
Conclusion: Accreditation has had a favorable impact on hospital performance indicators of course, the implementation of the healthcare reform plan may also have an impact on this. In general, the implementation of accreditation in the hospitals of the country can reduce the problems in this sector and provide more quality services to clients. Also, for better implementation of this program, emphasis on mortality and cesarean section rates can lead to improved hospital performance.

Abstract

Background and Aim: This was a 3-month cross-sectional study conducted from October to December 2021 on 170 people involved in food preparation and distribution in Haraz intercity road restaurants with the aim of measuring their knowledge, attitude and practice regarding food preparation and distribution.
Materials and Methods: The data collection tool was a questionnaire with two sections, that is, a researcher-made demographic questionnaire (age, gender, level of education, type of job in a restaurant, work experience, place of residence and history of attending training courses), and a section with questions about knowledge, attitude, and practice assessment based on the World Health Organization standards.
Results: Analysis of the data showed that 97.1%, 100% and 100% of the people had a good level of knowledge, a positive attitude and good practice, respectively. In addition, there were statistically significant relationships between knowledge and age, educational level and participation in educational courses.
Conclusion: The 25- to 40-year age group, people with university education and those with experience in participating in training courses were found to have the highest level of knowledge. The level of knowledge in key number four (keeping food at a safe temperature) and attitude and practice in key number 3 (full cooking) had the lowest averages. Also, considering that 82.4% of the people had not participated in any training courses, the need to hold appropriate training courses for them by the relevant agencies and motivate them to increase their knowledge and, subsequently, increase and improve their attitudes and practice can improve the situation.
