Abstract

A study was carried out to estimate the prevalence of Coagulase – Positive Staphylococcus aureus in milk collected in the milk collecting center in Garmsar city. Ninety-six raw milk samples (84 composite samples and 12 bulk samples) were collected from Garmsar milk collecting center. Raw milk samples were transferred to laboratory in safe conditions. After serial dilution, the appropriate concentrations (10 -2 to 10 -6) were cultured on Baired- Parker agar medium. The plates were incubated at 37 oC for 2 days (48h). After 48 hours' incubation, the colonies with black centers and light areas were used for staphylococcal assay. The coagulase test was done with rabbit plasma and coagulate- positive colonies were then counted. The results showed that mean counts of coagulate-positive Staphylococcus aureus in composite and bulk milk were 5.4 ×10 5 cfu/ml and 3.2 × 105 cfu/ml, respectively. The counts ranged from 8.6 × 10 4 to 2.1 × 10 6 cfu/ml and 1.4 ×10 5to 5.5 × 10 5 cfu/ ml. The standard deviations were 3.5 × 10 5 cfu/ml and 1.4 × 10 5 cfu/ml respectively. It is clear that the milk collected at this facility was rather heavily contaminated with S. aureus. Therefore, it may constitute a health hazard because of the possible production of enterotoxin in the milk consumed by the people. In order to reduce staphylococcal contamination in milk, it is recommended to treat cases of clinical and subclinical staphylococcal mastitis in livestock. Also, it is necessary to use hygienic procedures during the production process as well as transportation and storage.

Abstract

Background and Aim: Diabetes mellitus is a widespread disease. Diabetes patients should be fully familiar with the different dimensions of this chronic disease and possess the skills required for self-management and self-care. They should realize that each patient is a different case. It is to be noted that promoting self-management behavior is an essential component of case management in type-2 diabetes mellitus. The objective of this study was to assess self-management behavior of type-2 diabetes patients in Isfahan, Iran and determine factors influencing it.

Materials and Methods: This was a 6-month cross-sectional study conducted in 2011. The study population was type-2 diabetes patients consulting an outpatient diabetes center in Isfahan, Iran. The sample included a group of 350 patients selected by the continuous sampling method. The self-management behavior of the patients was assessed using a diabetes self-management instrument (DSMI), containing questions on 5 domains. The responses were rated on a 5-point Likert scale and SPSS software, version 11.5 was used for analyzing the collected data.

Results: The response rate was 88%. The majority of the patients were 50-60 years (55.52 ± 8.42) old and 54.3% of them demonstrated borderline metabolic control according to the World Health Organization criteria. Mean scores of self-integration, self-regulation, interaction with health professionals and other influential persons, self-monitoring blood glucose, and adherence to recommended regime were 33.67±6.36, 29.09±5.9, 27.08±4.81, 11.71±3.91 and 11.46±2.9, respectively. On the whole, the global diabetes self-management behavior seemed to be not quite desirable (the score was at an intermediate level). Further analysis of the data revealed that self-management behaviors (at 3 levels, namely, desirable, intermediate, and undesirable) had significant associations (p<0.001) with demographic (age, gender, marital status, education level) and health-related (duration of diabetes, diabetes-related diseases, diabetes complications, general health, type of treatment, family income, and serum HbA1C level) variables. Self-management behavior had no association with disease history

Conclusion: Empowerment of patients afflicted with diabetes and, more generally, non-communicable diseases, which are quite prevalent and on the increase, in the area of self-management at the individual and community level will be a vital step towards promotion of health of the population.

Abstract

Background an d Aim : Several factors can affect the sexual function of women. The aim of this study was to determine the correlation between exercise and sexual function in post-menopausal women.

Materials and Methods : This was a community-based, descriptive-analytical study including 405 post-menopausal women 40-65 years old selected by multi-stage stratified random sampling . The data were obtained through interviews using the Female Sexual Function Index (FSFI) and a researcher-made questionnaire. The data were analyzed using descriptive and analytical tests such as multiple linear regression and logistic regression models .

Results : On the whole, 61% of the subjects had sexual dysfunction. The most common type of exercise was “walking” (79.8%). For the women who exercised, the mean score in the domain of pain was significantly lower (worse) (p=0.013). The total FSFI, lubrication and pain domains scores were significantly lower in the women who did exercises other than walking as compared to those who did not exercise or practiced only walking. The weekly exercise frequency had positive correlations with lubrication (r=0.18, p=0.014) and orgasm (r=0.146, p=0.045) domain scores. However, multiple regression analysis to determine predictors of total score of FSFI and the domains scores did not give any statistically significant results. Finally, the logistic regression test showed that one additional exercise session per week would result in reduction of chances of sexual dysfunction by 80.2%.

Conclusion : Based on the findings, it may be concluded that exercise can be a w ay to compensate for sexual problems and dissatisfactions caused by reduced sex and sexuality in post-menopausal women. Exercising more frequently during the week could have positive effects on the sexual function of post-menopausal women.

Abstract

Background and Aim: Malfunction in the energy homeostasis central system resulting from melanocortin receptor 4 (MC4R) mutations is considered to be an important factor in development of obesity. Although physical activity can weaken the effect of obesity-related genes, very few investigations have been carried out in relation to the MC4R gene. This study was conducted to investigate the association between physical activity and body mass index (BMI) in different MC4R genotypes (rs17782313).

Materials and Methods: A sample of 400 individuals was randomly selected from among subjects participating in the Iranian Multicenter Osteoporosis Study (IMOS). Their weights and heights were measured and their dietary intakes and physical activity data assessed using validated questionnaires. In addition, the different MC4R genotypes (rs17782313) were determined in the subjects by the PCR-RFLP method. Finally, using the adjusted multiple linear regression model, the probable effect of physical activity on BMI in different MC4R polymorphisms (rs17782313) was determined.

Results: Only in individuals with the CT genotype was a statistically significant association (p<0.05) observed between body mass index and physical activity (hr/d and METs-hr/wk).

Conclusions: Physical activity, particularly in heterozygous MC4R polymorphism individuals (rs17782313), can play an effective role in reducing body mass index.

Abstract

Background and Aim: Prenatal and neonatal care provides an opportunity for counseling, resulting in reduction of pregnancy-related and neonatal complications. It is expected that such care would result in promoting the health status of these vulnerable groups and improving the relevant indicators.

Materials and Methods: This was a cross-sectional survey including 668 mothers and 2-month-old children. Data were collected using questionnaires and structured interviews.

Results: On the whole, 35.9%, 21.0% and 5.1% of the mothers reported, at least, one pregnancy, one complication, and one neonatal complication, respectively. Results of the logistic regression model showed that the odds ratio of reporting at least one of the pregnancy complications increased by more than double in women who had a medical history or a high-risk pregnancy, increase decreased by 63% in those who had continuous care during pregnancy, decreased by 40% in those who had received thorough prenatal care, and increased by 0.05% for every unit increase in age. The odds ratio of reporting at least one postpartum complication increased by 70% per unit of in the density of health workers. Moreover, the odds ratio of reporting at least one neonatal complication decreased by 48% in women who had received full neonatal care and by 50% per every unit of increase in the density of health workers.

Conclusion: The findings of this study show the important role of behvarzes (community health workers) in detecting postpartum and neonatal complications, as well as the importance of antenatal care and quality of antenatal care as regards detecting at-risk women and neonates and prevention of pregnancy and antenatal complications.
