Abstract

Background and Aim: Self-efficacy and decisional balance in the Trans-theoretical Model play an effective role in oral self-care behavior. The objective of this study was to determine the impact of education on inter-dental cleaning behavior based on the Trans-theoretical Model in pre-universstudents in the city of Yazd, Iran in 2009.

Materials and Methods: This was an intervention study. Based on statistical calculations, 361 pre-university students (mean age 17.53±0.55 years) were randomly allocated into groups of intervention (n=185, 51.2%) and control (176, 48.8%). Data on demographic characteristics, self-efficacy, and decisional balancing (perceived benefits and perceived barriers) were collected using a questionnaire with confirmed validity and reliability. Based on the trans-theoretical model, a 3-month educational program was prepared with sections on self-efficacy and decisional balance and conducted. At the termination of the program variables of the model were evaluated and the gingival index was determined by a dentist. The data were analyzed using the SPSS software, the statistical tests being ANOVA, paired t-test, t-test, Fisher, and correlation coefficient.

Results: Implementation of the educational program resulted in a significant decrease in perceived barriers (p<0.001) and significant increases in perceived benefits (p<0.001) and self-efficacy (p<0.001). In addition, the gingival index improved in the intervention group (p=0.01). The final mean self-efficacy, perceived barriers, and gingival index were not significantly different between the case and control groups, but a significantly positive effect was noticed in perceived benefits (p=0. 01).

Conclusion: The findings indicate that the educational intervention influenced desirably the gingival index, self-efficacy and perceived benefits in the students. Since the self-efficacy factor is an important predictor in oral self-care behavior, it is suggested to use the trans-theoretical model in educational interventions aiming at improving self-efficacy, increasing perceived advantages, and reducing perceived obstacles.

Abstract

Background and Aim:  AIDS is a lethal disease, for which there is no treatment or any vaccine to prevent it. The injection-drug use and unprotected sexual behavior are two factors important  in HIV transmission. This study was conducted to determine changes in high-risk sexual and injection-drug use behaviors associated with HIV among drug users referring to Methadone Maintenance Treatment centers in Shiraz, Iran.

Materials and Methods: This before-after interventional study with no control group included drug-users (n=694) referring to all the 7 MMT centers in Shiraz, Iran in 2009. The data were collected by interviewing patients and using a questionnaire. The software used for data analysis was SPSS 15, the statistical test being the Wilcoxon test.

Results: The average age of the clients was 36.5 ± 9.5 years. Most of them were males (% 92.1) and single (% 43.1). 37.3 % of them being injection-drug users. The proportion of the injection-drug users having had at least one shared injection one week before referring to an MMT center was 23.3%, which decreased to 9.2% after one week. Analysis of the data also showed that 25.2% of the addicts had had at least one unprotected sexual contact during the previous month the proportion decreased to 21.0% one month after referring to an MMT center. The reductions in the mean numbers of both shared injections and unprotected sexual contacts were statistically significant (p <0.001).

Conclusion: Considering the effectiveness of the MMT centers in reducing risky behaviors of injection-drug users, it is essential to expand these centers at both the national and regional levels quantitatively and qualitatively and encourage drug-users to refer to them and seek help. This will help greatly in the prevention of HIV.

Abstract

  Background and Aim: The community assessment is the basis of strategic policies for health professionals and managers. The aim of community assessment is collection of the data that relating to public health problems, data analysis and concludes about necessary capacities for providing the necessary resources to resolve problems.

  Materials and Methods: In this study the databases of Scientific Information Database, MagIran, MEDLINE, Google Scholar, and IranMedex were searched up to April 2014 (farvardin1393). All papers of community assessment in the country according to our standard community assessment model were included in this study.

  Results: Overall 11 studies were evaluated and the common feature of all research is to identify problems of society and local problems. The drug addiction was classified in 3 of the studies as a main problem and in 7 of studies was classified in the ten major problems. Poverty and unemployment are allocated to the next priority, the second priority in 2 studies, the third priority in 3 studies and the fourth priority in 3 studies. Environmental health problems and socio-cultural problems were the next priorities.

  Conclusion: The proposed model can be used to suggest new health services with due consideration of community problems, as well as develop suitable, effective intervention strategies. This type of community study should not be conducted only once. Rather, it should be repeated periodically in any community, results compared and used for policy-making.

Abstract

Background and Aim: Nowadays, prioritizing problems and distribution of resources is determined according to the basic needs of the individuals in the population. Based on the study done by researchers in Tehran University of Medical Sciences School of Public Health in Golhesar Village, Tehran, taking action about "drug abuse" was ranked by the community as the first priority. Therefore, the present study aimed to estimate the size of the population of drug abusers using the network expansion method.

Materials and Methods: This cross-sectional study was conducted on a purposive sample of 320 persons 15-65 years old. The study included two parts: 1. estimation of the social network based on 32 population subgroups using the frequency of network expansion approach; 2. estimation of the size of the hidden population. The relevant correction coefficients were used for adjusting errors usually observed in this method.                                                                                                                                                    Results: The mean of the social network was 160.79±16.68) 95% CIs: 128.78-194.30.(The frequencies of cigarette smoking, hookah smoking, alcohol consumption, opium use, meth use, crack use, heroin use, injecting drugs and ecstasy were 103.43, 88.25, 96.9, 76.46, 69.37, 30.03, 20.48, 7.36, and 4.23 per 1000; respectively. The percentage of drug abusers was higher among men.

Conclusion: As compared to results of previous reports, the size of drug abuser population in Golhesar Village is currently higher. In order to solve this problem an appropriate action plan should be implemented by the planners and policy-makers in partnership with community members.

Abstract

Background and Aim: The purpose of phenylketonuria (PKU) screening is to assess the risk of, and prevent, the disease in the newborns likely inflicted with it. The aim of this study was to investigate why and how PKU screening was decided to be included in the national health agenda, initiated and implemented in Iran.
Materials and Methods: This qualitative study was conducted based on the Kingdon’s framework model in 2015. The participants were thirty-eight policy-makers, managers and researchers selected by purposeful sampling.Data were collected using semi-structured interviews and analyzed using framework analysis.
Results: The physical and mental disabilities, medical and maintenance costs and the need for rehabilitation services were examples of the problem stream. In addition, sampling based on the National Congenital Hypothyroidism Program and implementation feasibility at the national level were examples of the policy stream. Finally, pressure by the patients' families, the Scientific Children Association and executive managers were examples of the political will stream. When the three streams occurred simultaneously, health policy-makers agreed to implement the program and then was opened the window of opportunity.                                                                                                          
Conclusion: Successful implementation of the National Phenylketonuria Screening Program in Iran has been the result of interactions among three streams, namely, nature of the problem, political will and optimal use of policy-makers of the window of opportunity opened.

Abstract

Background and Aim: The World Health Organization considers the implementation of the referral system to be a key step in improving the quality of services in health care systems. The purpose of this study was to explain the executive challenges of the electronic referral system in Golestan Province, Iran.
Materials and Methods: A qualitative study was conducted in Golestan Province, Iran in 2019 with a content analysis approach. The participants were family physicians, specialists, university staff managers, responsible health officers in the Offices of Deputy Ministers of Health, the General Directorate of Health Insurance, Department of Information Technology, Rural Health Network managers, health network managers and patients selected purposefully. Data were collected through semi-structured interviews and analyzed using the content analysis method.
Results: The main challenges of giving appointments to patients included the lack of time of family physicians, the irregular presence of specialists, difficulty in getting an appointment, and reverse referrals. As regards referrals and waiting times, the main challenges were found to be the lack of sufficient admissions following cancellation of the health insurance contract with the private sector, the delay of physicians in attending the health centers, long waiting times to be visited, and writing the feedback by the physicians. In addition, the main challenges of the visits and diagnosis included the short visiting times, the higher quality of the specialists' visits in the private sector, and the lack of clear guidelines. Finally, the main challenges as regards giving feedback were the reluctance and noncommitment of some specialists to provide feedback, as well as the low quality and lack of usability of the feedbacks given.
Conclusion: Based on the findings of this study, the following actions and measures may help improve the current situation as regards the current referral systems: allocating sustainable resources; allocating financial credits; designing and implementing a reward and punishment system; reforming the queuing system; empowering medical students; and equipping specialists with electronic pens for giving written feedback.

Abstract

Background and Aim: Among the valuable developments in the primary health care system has been the special attention paid to the issue of mental health and the addition of a self-care program for community mental health education. However, no integrated measures have been taken in relation to the needs of service recipients, educational priorities and the required facilities. This study was conducted to explain the priorities of this program in primary health care centers affiliated to Iran University of Medical Sciences, Tehran, Iran.
Materials and Methods: This was a qualitative study with a content analysis approach. The sample consisted of 12 stakeholders selected by purposive sampling with maximum diversity. Data were collected through in-depth and semi-structured interviews and a modified checklist of the Community Health Needs Assessment Program and analyzed using the Granheim and Landman approach.
Results: Based on data analysis, the needs were grouped into two main categories, namely, the needs of service providers and the needs of service recipients. The service providers’ needs included continuous coordination with non-health sectors, a virtual education and information system and continuous training and educational supervision, as well as appropriate space, facilities and equipment, while the of needs of service recipients were classified based on services needed for age groups, namely, services for children, adolescents and young people, adult and middle-aged, elderly and others.
Conclusion: Identifying needs can lead to the optimal use of limited resources aiming at imparting applied education and training. The results of this research can be an effective step towards expansion based on the needs for mental health education and a great help in achieving the goals of this program in primary health care centers. Based on the results it is recommended to plan personnel training and revise the services based on the identified needs in order to increase the effectiveness of this program.
