Abstract

Background and Aim: Pressure ulcer is a common problem and a large drain on hospital resources, especially in wards such as ICU where patients stay for a long time. The aim of this study was to explore the likely factors contributing to pressure ulcers in the ICU units of Tehran University of Medical Sciences teaching hospitals, Tehran, Iran.

Materials and Methods: This project was conducted in all the seven ICUs of four teaching hospitals affiliated to Tehran University of Medical Sciences. In the first phase the researcher performed a direct observation of all the 90 patients who were admitted to the ICU units to check the presence of any pressure ulcer. In the second phase, 310 patients discharged from the same ICU units between March 2007 and February 2008 was randomly selected and their medical records reviewed. A structured questionnaire was used to collect the data.

Results: Pressure ulcer occurred more frequently in the elderly women patients who were less active and had less mobility, hospitalized for a long time, or those with fever, diabetes, high blood pressure, paralysis, or respiratory diseases. In addition, pressure ulcer was seen more frequently in the patients hospitalized in the general ICUs, as well as in those with infections and different types of tumors.

Conclusion: Using a standard risk assessment tool and paying attention to the main risk factors of pressure ulcer can be a useful method for identifying the high-risk patients before admission in order to prevent them from developing such ulcers.

Abstract

Background and Aim: Presently not sufficient attention is paid to adolescent health promotion in many countries, because of the widespread belief that adolescents are generally healthy. The purpose of this study was to evaluate predictors of adolescent girls' quality of life using path analysis based on Pender's health promotion model.

Materials and Methods: The Pender's health promotion model served as the framework of this cross-sectional study, which included a total of 500 adolescent girls (grades 9-11) selected by stratified random sampling from 20 high schools for girls in 4 districts of Kashan province, Iran. Inclusion criteria were being in grades 9-11 and a written informed consent (from teachers, parents and the girls themselves). Data were collected through interviewing the girls, the interviewers being trained persons, using several standard questionnaires (for perceived self-efficacy, perceived affect, perceived barriers, perceived social support health promoting lifestyle, and health-related quality of life). Data analysis was done using SPSS18 and LISREL 8.8, the statistical test being path analysis.

Results: All constructs of the health promotion model and health promoting lifestyle were significantly related to adolescent girls' quality of life. Perceived self-efficacy (β=0.70, p<0/01) and perceived affect (β=0.21, p<0/01) had the highest correlation with the quality of life. The model and the health-promoting lifestyle as a mediator accounted for 73% and 39% of variance in adolescent girls' quality of life, respectively.

Conclusion: The findings of this study indicate that the health promotion model can be used to explain and predict the quality of life of adolescent girls. Thus, the Pender's health promotion model is recommended to health planners for promoting the quality of life in this population.

Abstract

Background and Aim: Considering the social and cultural characteristics of Iranian adolescents, none of the quantitative instruments designed so far to assess their health needs is quite appropriate. The purpose of this study was to design a valid and reliable questionnaire to assess the health needs of Iranian adolescent females.

Materials and Methods: Both qualitative and quantitative approaches to instrument development were adopted in this study conducted in the District of Sari in the north of Iran. Sampling was objective-based, with an attempt to have female adolescent sample as varied as possible. In the qualitative phase, a content analysis approach was used to explore the concept of health needs as seen by female adolescents. Data were collected using 8 focus group discussions (FGDs) with the presence 6-10 adolescents 12-18 years old in each FGD, as well as 11 semi-structured interviews, each one involving 11 well-informed individuals. In the quantitative phase, the psychometric properties, including content validity, face validity, construct validity, concurrent validity, internal consistency, and test-retest reliability, were determined.

Results: Five themes were identified in the qualitative phase of study, including psycho-emotional health, social health, physical health, and educational needs, as well as spiritual beliefs. The mean scale-level content validity index (S-CVI) was found to be 0.92. The factor structure of the instrument was identified by doing a Principal Component Analysis. Five factors were extracted with a total variance account of 45.37%. The reliability and consistency of the instrument were established with the Cronbach's alpha coefficient (0.90) for the entire scale and test-retest reliability with a 2 week-interval Intraclass Correlation Coefficient (ICC=0.984, p<0.001).There was a concurrent correlation between the instrument and Pediatric Quality of Life Inventory (PedsQL TM 4.0) (r=0.66, p<0.001).

Conclusion: The instrument developed in this study is a valid, reliable and culturally appropriate instrument for assessment of health needs of Iranian adolescent females.

Abstract

Background and Aim: There is evidence that physical activity declines during adolescence in Western countries. However, this pattern has not yet been shown in Iranian youth. The purpose of this study was to detect changes in physical activity behaviors, psychological factors (including perceived benefits, barriers, and self-efficacy), and interpersonal influences associated with exercise behavior during the transition from secondary school to high school in Sanandaj, Iran.
Materials and Methods: The data on a group of high school students in Sanadaj were collected in a cross-sectional study in 2006, with a follow-up in 2009. The study population in 2009 included 844 students (51% males and 49% females) with a mean age of 16.42 ± 1.73 years. Data on physical activity-related perceived benefits, barriers, self-efficacy, and interpersonal influences, as well as physical activity behavior, was collected, using self-reporting questionnaires. The SPSS-16 software was used for data analysis.
Results: The test of time effects indicated significant differences between the 2 sexes as regards physical activity and most of the psychological variables and interpersonal influences associated with exercise activities. The girls had less physical activity than boys at both time points. The duration of moderate physical activity at the first time point was 31.82 and 53.75 minutes for girls and boys, respectively the corresponding values at the follow-up were 23.7 and 44.7 minutes. Girls had lower self-efficacy and fewer perceived benefits and more perceived barriers for physical activity over time. The correlation test indicated more stability of family interpersonal influences for girls than for boys.
Conclusion: The results of this study provide evidence for decreased physical activity in boys and girls and sex differences in cognitive variables and interpersonal influences.

Abstract

Background and Aim: The objective of this study was to determine the relationship between educational practice of volunteer health workers (VHWs) and preventive behavior of households (HH) under their coverage regarding cutaneous leishmaniasis (CL) based on the BASNEF model in Yazd City, Iran

Materials and Methods: In this descriptive cross-sectional study, all the 60 VHWs in the CL-endemic regions in Yazd City were interviewed and completed a valid and reliable questionnaire designed by the investigator. In addition, from among the households under coverage of each one of the VHWs, 2 HHs were selected randomly and their heads were interviewed and completed the relevant questionnaire (total n=120). Data were analyzed using descriptive and inferential statistics.

Results: There was a positive correlation between the knowledge and attitude of VHWs and knowledge of the households, as well as between the VHWs' behavioral intention and households' behavior. Further analysis of the data also revealed a positive correlation between the enabling factors of VHWs and the knowledge, behavioral intention, enabling factors and behaviors of families, as well as between educational behavior of VHWs and knowledge, attitude and behavior of the families.

Conclusion: Volunteer health workers can potentially play an important role in case-finding, prevention and timely treatment of cutaneous leishmaniasis. However, merely promoting their awareness of the disease will not lead to improvements in their educational behavior in the community. More effective education aiming at promoting their awareness and enabling factors, as well as developing better attitude in them, will certainly result in better behavior of households and control of the disease

Abstract

Background and Aim: Visceral leishmaniasis is a systemic parasitic disease with a high fatality rate in under-5-year-old children. The disease is endemic in some parts of Iran, particularly in the north-west region. In 2001 a visceral leishmaniasis (VL) surveillance system was established for children aged ≤ 12 years in the primary health system in Meshkin-Shahr District, Ardebil Province, situated in the north-west of Islamic Republic of Iran.

Materials and Methods: All cases with clinical signs and symptoms of VL and confirmed positive by the direct agglutination test (DAT) were referred for physical examination and treatment.

Results: The mean annual incidence of VL decreased significantly from 1.88 per 1000 children before (1985-2000), to 0.77 per 1000 child population after (2001-07), the intervention. In the control area with no surveillance, it increased from 0.11 to 0.23 per 1000.

Conclusion: Early detection of VL using serological tests and timely treatment of cases can decrease the mortality and morbidity rates of VL in endemic areas.

Abstract

Background and Aim: Priority setting is one of the most important issues in health research systems. No national health system can afford to finance all research projects proposed by researchers, especially in developing countries. Therefore, we decided to set the main national health research priorities in Iran using the Essential National Health Research (ENHR) method.

Materials and Methods: All of the Iranian universities of medical sciences and other stakeholders collaborated in this study. The methodology for research priority setting was based on needs assessment and ENHR. 

Results: The total number of research priorities gathered from universities of medical sciences was 6723. The proportions of topics related to basic science, applied and development subjects were 17%, 78% and 5%, respectively. Further analysis of the data showed that almost half (46.3%) of the research priorities were descriptive, 36.0% analytical, and 17.6% interventional. The research priorities were divided into 9 main areas, namely, communicable diseases, noncommunicable diseases, health system research, pharmaceutical sciences and industrial pharmacy, basic science, traditional and herbal medicine, nutrition, environmental health, and dentistry.

Conclusion: Up to now the common procedure for research priority setting has been, with a top-to-bottom approach, managed by a limited number of researchers and experts, while in the method presented in this paper a bottom-to-top approach is used, which is more effective.

Abstract

Background and Aim: This paper reports on the predictors of high school students' oral health behaviors (OHBs) based on the Pender's Health Promotion Model (HPM) in an attempt to identify effective factors that may be addressed through intervention efforts.

Materials and Methods: A multistage cluster sample of 403 high school students were recruited from 8 high schools in Marivan County, Iran. The study was conducted between April and July, 2010. Appropriate instruments were used to measure the relevant variables. The statistical analysis of the data included bivariate correlations, t-test, one-way ANOVA and linear regression.

Results: All HPM variables (perceived self-efficacy, perceived benefits and barriers, activity-related affects, interpersonal and situational influences and commitment to a plan of OHBs) were significantly related to OHBs among the respondents (p < 0.01), and a negative association was found between OHBs and perceived barriers (p < 0.01). All HPM variables were statistically significant predictors of OHBs and accounted for 42.2% of the variance.

Conclusion: Promotion of interpersonal influences and the students' perceived self-efficacy and also commitment to a plan of action should be priorities of any program aimed at promoting OHBs among students. School health care providers should suggest strategies to overcome the barriers of performing OHBs, rather than noting the benefits of these behaviors.

Abstract

Background and Aim: Child growth disorders are a result of nutritional ignorance and lack of appropriate care and growth monitoring. This study was conducted to assess the effects of mothers' educational program based on the precede model on the prevention of growth retardation in 6 to12-month old children in the health centers of Shiraz city, Fars Province, Iran.

Materials and Methods: This quasi-experimental study included 120 mothers (60 cases and 60 controls) with a single exclusively-breastfed child under the coverage of health centers in Shiraz city, Fars province, Iran. A questionnaire was used to collect data on demographic characteristics and the precede model components (knowledge, attitude, enabling and reinforcing factors and maternal behavior). In addition, the children were weighed and their weights recorded. The mothers' educational intervention consisted of six 55 to 60-minute sessions. The questionnaire was completed by the case mothers before and 4 months after the intervention.

Results: The interventional educational program brought about statistically significant increases in the mean scores of knowledge (p<0.001) and attitude (p<0.001). Further analysis of the data also showed that the enabling (training sessions and using references) and reinforcing factors, as well as practice score of the mothers and the mean weight of the children in the case group improved significantly as compared to the control group (p<0.001).

Conclusion: The results of this study indicate that an interventional educational program for mothers based on the precede pattern had a positive effect in the prevention of growth retardation in the study population.

Abstract

Background and Aim: Diabetes mellitus is a widespread disease. Diabetes patients should be fully familiar with the different dimensions of this chronic disease and possess the skills required for self-management and self-care. They should realize that each patient is a different case. It is to be noted that promoting self-management behavior is an essential component of case management in type-2 diabetes mellitus. The objective of this study was to assess self-management behavior of type-2 diabetes patients in Isfahan, Iran and determine factors influencing it.  

Materials and Methods: This was a 6-month cross-sectional study conducted in 2011. The study population was type-2 diabetes patients consulting an outpatient diabetes center in Isfahan, Iran. The sample included a group of 350 patients selected by the continuous sampling method. The self-management behavior of the patients was assessed using a diabetes self-management instrument (DSMI), containing questions on 5 domains. The responses were rated on a 5-point Likert scale and SPSS software, version 11.5 was used for analyzing the collected data.

Results: The response rate was 88%. The majority of the patients were 50-60 years (55.52 ± 8.42) old and 54.3% of them demonstrated borderline metabolic control according to the World Health Organization criteria. Mean scores of self-integration, self-regulation, interaction with health professionals and other influential persons, self-monitoring blood glucose, and adherence to recommended regime were 33.67±6.36, 29.09±5.9, 27.08±4.81, 11.71±3.91 and 11.46±2.9, respectively. On the whole, the global diabetes self-management behavior seemed to be not quite desirable (the score was at an intermediate level). Further analysis of the data revealed that self-management behaviors (at 3 levels, namely, desirable, intermediate, and undesirable) had significant associations (p<0.001) with demographic (age, gender, marital status, education level) and health-related (duration of diabetes, diabetes-related diseases, diabetes complications, general health, type of treatment, family income, and serum HbA1C level) variables. Self-management behavior had no association with disease history 

Conclusion: Empowerment of patients afflicted with diabetes and, more generally, non-communicable diseases, which are quite prevalent and on the increase, in the area of self-management at the individual and community level will be a vital step towards promotion of health of the population.

Abstract

Background and Aim: Demographic, physical, social and psychological factors make breastfeeding and its maintenance complex. Despite extensive breastfeeding promotion programs, no progress has been achieved. As breastfeeding is a phenomenon embedded in culture and folk perceptions, identification of its key determinants is essential when designing policies and implementing programs.

Materials and Methods: In this qualitative study, data were collected from 35 health professionals through a 4-day group interview using a structured questionnaire (open-end questions). The participants were purposefully recruited from among the health professionals attending a 4-day breastfeeding promotion workshop.

Results: The determinants of breastfeeding were categorized into 2 main domains, namely, people's perception of breastfeeding phenomenon and approaches employed in its promotion. The former domain was described by beliefs, demographic characteristics, social variables, and indigenous theories, and the later by practice in the cultural and social context, policies, interactions and practical skills.

Conclusion: From the participants' perspective, misperceptions of people can determine the degree of success or failure of a breastfeeding promotion program. Our findings confirm that people's perceptions are deeply influenced by their demographic characteristics, folk theories and social variables. As regards the breastfeeding promotion domain, the barriers are social-cultural factors rooted in the local beliefs and folk theories. The participants listed the breastfeeding promotion determinants as lactation policies and health providers' performance related to their skill in effective communication with mothers. Our findings also indicate that wrong beliefs, maternal and child diseases, caesarian section, and mothers' employment were believed to be deterrents in promoting breastfeeding in the community and that pediatricians, obstetricians and other health professionals can play key roles in the field. Based on our findings, we can further conclude that the phenomenon of breastfeeding and its maintenance are deep-rooted in social structure and cultural diversity. When designing and implementing breastfeeding promotion programs, local variables and determinants should be taken into consideration.

Abstract

Background and Aim:  Shift work may induce adverse effects such as health problems on human life. The purpose of this study was to investigate shift work-related health problems and their associated risk factors among workers of petrochemical industries.

Materials and Methods: This cross-sectional study was carried out in 21 petrochemical companies affiliated to the National Petrochemical Industries Company, Iran. The study population consisted of 3580 workers (44.2% shift- and 55.8% day-workers). Data were collected on demographic characteristics, shift schedules, and health problems using anonymous questionnaires. Statistical analyses and modeling were performed using SPSS version 11.5.

Results: The mean age of the day-workers with a job tenure of 8.87±7.22 years and of the shift-workers with a job tenure of 8.22±7.02 years was 34.56±8.19 and 33.15±7.73 years, respectively. Analysis of the data showed that prevalence of health problems among shift-workers was higher than that of day-workers. Furthermore, based on multiple linear regression analysis, it was revealed that, in addition to shift-working itself, other variables such as long working-hours, type of employment, age, job tenure, education, and job title also had a role in creating problems.             

Conclusion: Although in the shift-working system the main risk factor associated with health problems among the workers is the work itself, there are also other risk factors, such as demographic and occupational variables.

Abstract

Background and Aim: Access to reproductive health information and services is a basic right for the youth. Lack of suitable educational/counselling materials about reproductive health can decrease effectiveness of educational programs. This study aimed to determine the premarrital reproductive health educational needs of the youth.

Materials and Methods: In a descriptive-analytical study a total of 450 young males and females were selected by stratified random sampling to participate in pre-marital counseling in Tehran. Data about pre-marrital reproductive health educational needs were collected using a questionnaire developed by the investigator, with a 5 degree-Likert scale. SPSS-15 was used for data analysis, the statistical tests being t-test, one-tailed ANOVA and chi-square.  

Results: Based on the Likert scale (1-5), felt need for reproductive health education was 3, it being higher among females (p<0.05). Both the males and females thought that what they needed most was education/counseling on healthy sexual relationship. The educational needs did not seem to be influenced statistically significantly by socio-demographic variables in either gender.

Conclusion: Based on the results of this study, it seems that the current educational materials are not sufficient for pre-marrital counseling. Comprehensive information on reproductive health, with particular emphasis on sexual relationship, should be included in the existing programs, so that the youth can begin a better, healthier married life.

Abstract

Background and Aim: Internet has become an effective medium to transfer new scientific and research findings, knowledge and information in educational processes across the world. It also facilitates essential communication among millions of people globally and offers users plenty of entertainment for their leisure time. However, excessive computer use interferes with daily life and carries a risk of addiction. Therefore, the concept of internet addiction is now considered a social problem. This study was conducted to determine the association between internet addiction and general health among dormitory students of Tehran University of Medical Sciences in 2010.

Materials and Methods: This cross-sectional study, conducted in 2010, included 125 female- and 110 male-students from dormitories of Tehran University of Medical Sciences, selected by stratified random sampling. The instruments for data collection were two standard questionnaires - YANG Internet Addiction and Goldberg's General Health (GHQ). Data were analyzed by SPSS-19, the statistical tests being Kolmogorov-Smirnov, Mann-Whitney U, Chi-square, correlation coefficient, and multivariate regression.

Results: The results showed that, based on the YANG classification, 34% of the students were addicted to the internet. In comparison with ordinary users, the general health of the internet-addicted users was at a higher risk (p<0.001). The two groups were also different with regard to anxiety and depression (p<0.001), but not with regard to physical function or social function.                     

Conclusion: Internet addiction among dormitory students is a cause for concern. Well-designed programs focusing on education, counseling and behavioral intervention are needed to reduce internet addiction amongst vulnerable students.

Abstract

Background and Aim: Improvement of national and provincial health indices requires intersectoral collaboration and community participation So, National High Health and Food Security Council and Provincial Health and Food Security Councils (PHFSC) have been implemented since 2006. The main responsibilities of PHFSCs are, assessment of province health situation and effects of executive organizations on it's improvement, to define and approve the province health strategic plan, developing of AIDS and HIV prevention and control plan and resolving intersectoral issues of health. The goal of this study is first 3 years performance assessment of the PHFSCs.
Materials and Methods:
This is a descriptive evaluation study which included all the PHFSCs. The outcome assessment indicators were determined by content analysis of "the approved tasks" and also "the essential aspects for promoting of PHFSCs" by focus group discussions. Situational analysis was provided by two researcher made questionnaires one for co Assessment of the contents of content analysis of PHFSCs all meetings minutes (from establishment year to the end of 2008) and another for gathering viewpoints of Secretaries of PHFSCs.
Results:
Data analysis shows that the meeting's agendas of the PHFSCs have less followed the prioritized major health issues of provinces and national burden of diseases most issues were in the domain of ​​communicable disease control and non-communicable diseases risk factors control included scarcely. Only in two Universities of Medical Sciences (UMSc) province comprehensive health plan were proposed. None of the UMSc approved a secretariat for PHFSC in its organizational top chart. PHFSCs secretaries believed that, the main obstacles of the PHFSCs' successes, are weak financial resources, and lack of decisions executive enforcements.
Conclusion:
PHFSCs performances are moderate and moderate to low due to lack of comprehensive strategic plan and no implementation model for intersectoral decisions, other organizations believe on the effectiveness of the PHFSCs less and the main burden of disease of the provinces not mostly included in the agenda of PHFSCs. Following proposals are highly recommended for PHFSCs promotion: training and empowerment of UMSc and organizations for developing comprehensive health plan in the province and defining the responsibilities of each organizations for, determining policy priorities of common health issues of the provinces by High Health and Food Security Council and publicizing them by the Ministry of Health and the other relevant ministries and approving the structure and working system of the PHFSC secretariat in UMS by the Board of Trustees of UMS.

Abstract

Background and Aim: Cervical cancer is the second most common cancer in women with incidence of more than 500000 cases per year. It is preventable because of its long pre-cancerous period, suitable screening programs and effective treatments. Since the social and cultural barriers alongside the lack of knowledge are deterrents to participation in screening programs, the current study was performed to assess the effect of educational intervention through the Health Belief Model on knowledge and attitude of teachers about pap smear, considering their role in making people aware. Materials and Methods: This was a controlled before-after interventional study conducted on 72 individuals selected through simple sampling, assigned to two groups. Data collection tool was a questionnaire used and compared before training and one month after that. Training methods (intervention) included lectures with questions and answers. Data were analyzed with SPSS, version 11.5. Results: 47.2% of intervention and 45.8% of control group were 40 to 50 years old and majority of them (47.2% and 50% respectively) had undergraduate educational level and 86.1% were married. Respectively, 67.2% and 74.2% have experience of being examined via Pap smear, while the knowledge of women in both groups was moderate about the disease. Study results revealed significant differences in knowledge, attitude and Health Belief Model constructs within intervention group (before and after training) as well as between groups (p<0.05). Conclusion: Using Pap smear test among the participants was lower compared with healthy people aims targeted by 2010. Therefore, necessity of education and knowledge promotions are obvious.

Abstract

Background and Aim: Puberty as a stage of life is an important developmental process. Teen girls have numerous health care needs and their specific issues should be considered. This study mainly aimed to determine the impact of education based on health belief model on promotion of physical puberty health among female teenagers who were residents in welfare boarding centers in Tehran city.
Materials and Methods: It was a quasi-experimental intervention study, conducted in welfare boarding centers of Tehran city among inhabitant girls with 12 to 19 years old. All eligible persons were selected (n=61). Data collection tool was a questionnaire including demographic information, health belief components, physical function during menstruation, completed before and one month after training by interview. Educational intervention was implemented in 3 months. Data was analyzed by Wilcoxon statistical test using SPSS16 software.
Results:
After educational intervention, perceived severity (p<0.001), perceived susceptibility (p<0/001), perceived barriers (p<0.001), perceived benefits (p<0.001), and physical function during menstruation were significantly different.
Conclusion:
Educational intervention improved girls' perceptions and performance. Thus, health belief model should be used for puberty health education planning in female adolescents.

Abstract

Background and Aim: Educational programs for couples' mental health promotion in marital life are a necessity.  The first step in planning an educational program understands the needs of the target group. This paper reports the mental health educational needs in youth planning to marry.

Materials and Methods: In a descriptive-analytical cross sectional study, 450 individuals taking part in pre-marriage education courses in Tehran were selected by randomized stratified sampling. The data was collected using a questionnaire that measured the educational needs in the area of mental health by the 5-point Likert-type scale. Data analysis was done using the SPSS-15 software, the statistical tests being the independent T-test, one-way analysis of variance (ANOVA), and the chi-square test.

Results: On the whole, the subjects' felt need for mental health education was more than moderate (3 on the Likert scale) women expressed a higher need than men (p=0.001). The main concern of the participants - both men and women - was How to keep romance alive in married life. The extent of need expressed for mental education was not statistically different in different socio-demographic groups of women, while worker men expressed a higher need than self-employed men (P=0.018).                                                                        

Conclusion: Based on the findings of this study, young people planning to marry have a felt need for education in the area of mental health. It is recommended to design and implement educational programs to promote their knowledge in this area with due consideration of priorities stated by them.

Abstract

Background and aim: Delivery or parturition is the peak of a woman’s pregnancy period, accompanied with birth of one or more babies. Unfortunately, Cesarean section, despite its probable undesirable health and economic consequences, has become more and more common. Considering that the goal of health education is behavior change in people, the objective of this study was to investigate the role of health volunteers in educating mothers consulting the health care centers in Ardabil in the area of choice of delivery method based on the Behavioral Intention Model (BIM).
Materials and Methods: In this experimental study, 140 primaparous mothers in Ardabil city in the seventh month of pregnancy were selected by systematic random sampling and divided into a case and a control group, and a written pretest was given to both groups. Education based on the BIM was imparted to the case group. One month after delivery, both groups were post-tested. The data on awareness, attitude, results evaluation and practice were analyzed using the Wilcoxon, Mann Whitney-U and Chi-square tests.
Results: The findings revealed statistical differences between the case and control groups after intervention with regard to awareness, evaluation of results and attitude (p<0.001). However, behavioral intention did not change (p=0.106).
Conclusion: Although health volunteers' participation results in an increase in knowledge and promotes attitude and evaluation of behavioral results in pregnant women, it does not change their behavioral intention. It is suggested to conduct similar studies for comparative purposes involving other trainers, such as physicians and nurses in other cities, using other educational models and methods.

Abstract

Background and Aim: The most important single determinant of the quality of life of the elderly is health. Studies have shown that health declines with age. Because of high vulnerablity of the elderly due to many problems, they need particular attention and self-care programs for their health promotion. The objective of this study was to assess the quality of life of elderly clients under coverage of the Southern Tehran Health Center and determine the effects of a self-care program on it.

Materials and Methods: This was a controlled interventional study conducted in 2010-2011. A total of 132 elderly clients (52 in the intervention, and 80 in the control, group) were selected by randomised cluster sampling from 5 Health Care Centers in Southern Tehran. A 3-month educational program was prepared with contents based on books published by the Ministry of Health and Medical Education and implemented. The quality of life was assessed in both groups at the beginning and 2 months after the intervention using a short questionnaire (sf36). Data were collected through a structured interview (demographic characterstics) and analyzed using descriptive and analytical statistical tests.

Results: The mean quality-of-life score of the experimental group increased after education the mean score of physical domain of the quality of life increased from 52.75 in 66.11 and that of the emotional domain from 53.61 to 70.85. The scores of different domains of quality of life of the control subjects did not change significantly.

Conclusion: The findings show that a self-care educational program can have a positive affect on quality of life of the elderly.
