Abstract

Background and Aim: Considering the fact that injecting drug is the most common way of HIV transmission and having the point in mind that in spite of syringes availability, many of the IDUs still continue sharing needles, determining behaviors which lead them not to use HR packages seems invaluable. Determining the influential factors in sharing needles among IDUs who are Kermanshah's DIC clients and are provided with training program and HR Packages (syringes, needles, bleaching and condoms).

Materials and Methods: These subjects were studied in 2 steps. The first FGD (focus group discussions) and the second a cross sectional study. To gather data that we use a questionnaire based an FGG which includes: general characteristics, characteristics related to drug abuse and information about HIV.

Results: out 437 of cases that took part in the interview, 367 cases were IDUS. Among whom 28% individuals shared needles. The medium age was 33/4 and there was no significant difference between the two age groups. Only 27% were married. 61% of cases were unemployed. The unemployment rate among IDUs who share needles and syringes was a lat more significant (p=000) than those who don't share needles (P=000). The percentage of IDUs who share needles and don't living houses was higher than of those who live in houses (P=000). Most of cases started taking drugs before they were 20 years old.

Conclusion: Low Education degree unemployed, no married. Live in street, start IDUs in low age the period of referring to DIC were the most important factors for share needles injection

Abstract

Background and Aim: Environmental health impact assessment of industrial estates will help greatly in better planning for sustainable development and reducing disease risk in a community. The objective of this study was to assess the positive and negative effects of establishing an industrial estate in Jovein, Sabzevar, Iran in 2006-7 and suggest managerial strategies to reduce undesirable environmental health impacts.

Methods and Materials: A modified Leopold Matrix and Scaling checklist method was used. The existing environmental situation was investigated and then environmental impact assessment alternatives were determined, bearing in mind the amounts and kinds of predicted pollutants in the construction and operational phases . The assessment was made considering the immediate, direct, and indirect impacts in the short and long terms. It was attempted to use a wide range of values of different factors (-5, +5) in order to estimate the impact of all activities on the environmental status.

Results: The results of the study are presented with two alternatives, i.e., "No" (conducting the project with no concern for environmental issues), and "Yes" (performance of the project with application of methods to reduce the environmental harmful impacts), when establishing the industrial estate, in the construction and operation phases. The impact assessment with the "No" alternative scored -791, meaning that execution of the project had to be rejected. However, after reducing the harmful impacts the score rose to +252, indicating that the project would be accepted.

Conclusion: The method of reducing harmful environmental impacts along with environmental management programs introduced are accepted in this study. It is recommended that the Ministry of Health and Medical Education and The Iranian Environmental Protection Agency collaborate closely in the area of environmental health impact assessment of industries and industrial estates.

Abstract

Background and Aim: High concentrations of organic compounds and ammonia and entry of toxic pollutants lead to a low efficiency of direct biological treatment of landfill leachate. The aim of this study was to determine the efficiency of a repeated fed-batch biological reactor with powdered active carbon (PAC) for landfill leachate treatment.

Methods and Materials: Raw leach ate was pretreated by coagulation and flocculation processes and, then, the ammonia was removed by air stripping at a high pH, before biological treatment. The pretreated leach ate was treated bio-treated in an aeration tank by the repeated fed-batch method. Three cycles of 1×40, 5×8, and 4×10 hours were attempted in the absence and presence of 2 g PAC /L.

Results: Results indicate that the presence of PAC increased the COD removal efficiency significantly. The initial COD effluent was 3900 mg/L. Its values were 169 mg/L and 622 mg/L in the presence and absence of 2 g/L PAC, respectively, for the cycle of 5×8 h.

Conclusion: Based on the results of this study and findings of other investogations in this field, it can be concluded that repeated fed-batch bio-reactor has is quite effective for treatment of pretreated leachate.

Abstract

Background and Aims: Population aging is one of the most important economic, social and health challenges of the twenty-first century. The increasing rate of aging in the population will increase the risk of households being confronted with catastrophic health expenditure (CHE). The purpose of this study was to determine the effect of aging on households' CHE in Iran.
Materials and Methods: The data on household income-expenditure were obtained from the annual Iranian Statistical Center reports for the period 2007-2016.  The statistical technique used for data analysis was the regression Logit technique, the software for the final data analysis being the Excel and STATA.
Result: The results showed that increased numbers of elderly people in a household would increase the probability of the household being confronted with CHE. Additionally, the following households were also more likely to be at risk of CHE: urban families; female-headed families; those not owning private homes; those with no income or with an unemployed head; and those in the lower income deciles. Moreover, no insurance coverage would decrease the likelihood of a household to be at risk of CHE.
Conclusion: Considering the findings of this study and the aging of the Iranian population, it is essential that the government, in line with fair financing and reducing the economic burden of health expenditures on households with an elderly member, try, through special support packages for the elderly, to prevent confrontation of such families with catastrophic health expenditures.

Abstract

Background and Aim: Using a proper method to prioritize and develop a basic health services package is a serious challenge. This study was conducted to design a process for developing and revising a basic health services package in Iran.
Materials and Methods: A combined study of quantitative and qualitative methods was used in this study. The qualitative part was conducted through holding 25 in-depth individual interviews and two focus group discussions with experts from the Ministry of Health and Medical Education, Program and Budget Organization and insurance organizations, while the quantitative part was a descriptive cross-sectional study including 277 experts using a valid and reliable questionnaire.  Data analysis of the qualitative and quantitative parts was done using a content analysis and the SPSS-23 software, respectively.                                                                                                                             
Results: The process of developing a basic health services package includes 4 steps, namely, explaining the list of services in the basic health services package, extracting the criteria for including services in the service package, prioritizing the criteria for including services in the package, and comparing services with priority criteria. In the prioritization stage, disease burden criteria, target groups and community needs were found to have the highest means and recognized as the most important criteria.
Conclusion: The process designed in this study for developing and revising a basic health services package provides policymakers with the required scientific evidence by emphasizing the introduction and continuity of services that have higher priorities and the elimination of services that have lower priorities.

Abstract

Background and Aim: Considering the quality of prenatal care services, indentification of the avoidable and inevitable maternal factors and proper promotion of the knowledge and skills of health care service providers is vital for providing suitable preventive and interventional services with the aim of ensuring a successful pregnancy and proper management in the health care system.  This study was conducted to compare the adverse pregnancy outcomes among women with or without a history of Covid-19 in the pregnancy and postpartum stages covered by the Babol University of Medical Sciences, Babol, Iran.
Materials and Methods: This was a descriptive-analytical cross-sectional study including 4124 women, either pregnant or after a recent childbirth (with or without Covid-19) between 20.2.2020  and  29.3.2022, registered in the Sib electronic system (hereinafter Sib) of Babol University of Medical Sciences, Babol, Iran. Using a checklist developed and validated by an expert panel data were collected on the prevalence of maternal and neonatal outcomes in mothers with or without a history of Covid-19 and compared. The data collected were analyzed with SPSS 24 using descriptive tests such as chi-square and Fisher's exact test and STATA version 22, and the adverse outcomes with high frequencies following Covid-19 were determined.
Results: Analysis of the data showed the following to be the most common outcomes among mothers with a history of Covid-19: 1. obstetric complications and maternal disorders/diseases (hypertension, type-2 diabetes mellitus, at least one case of bleeding during pregnancy, premature/delayed birth and premature rupture of the amniotic sac, n= 457, 27.8%); 2. medical and surgical problems (unhealthy pregnancy weight gain and thromboembolic events, n=206, 12.5%). In addition, there was a statistically significant difference between the two groups of pregnant mothers with and without a Covid-19 infection as regards medical and surgical problems (P=0.001) and obstetric complications and maternal diseases (p=0.002). As regards the adverse neonatal outcomes in pregnant mothers with and without a history of Covid-19, there was a statistically significant difference between the two groups in terms of Apgar score at minutes 1 and 5 (p≥0.001) and exclusive breastfeeding (P=0.001) and the presence of Covid-19 symptoms in the baby (p<0.001).
Conclusion: Based on the findings of this study it seems that Covid-19 infection during pregnancy is associated with a higher risk of some of the undesirable maternal and neonatal outcomes.  Therefore, it is essential to plan and implement programs for health education, creation of a sound attitude towards better management, enhancement of the skills and capabilities of the health-care providers and, finally, promotion of the knowledge of service recipients.

Abstract

Background and purpose: The purpose of this research was to  write a critical review of the available service packages in the Mental and Social Health and Addiction Office (MSHAO) of the Ministry of Health and Medical Education in the areas of drugs, alcohol and tobacco, which are currently used in the primary health care system. The content analysis method was used to extract the specialists’ critiques.
Materials and Methods: Initially the MSHAO sent 1362 pages of information in the form of 47 files to researchers to be reviewed. The files were classified, eliminating/removing additional, unrelated and duplicated items; finally 840 pages in 29 files were studied and carefully examined. Then, seven experts in the field of addiction were asked to criticize the packages, using the content analysis method to extract the experts' critiques.
Results: The findings showed that comprehensive information about drugs, alcohol and tobacco addiction is provided to the clients. However, based on a critical review of the existing packages four main critiques were extracted, namely, sources/references, epidemiology, incompatibility of Iranian-Islamic culture, and technical-specialist issues. In addition, criticisms were also extracted in the following areas/services: the number of files and the variety of interventions, editing and writing problems, interventions and discussions with inappropriate in-text sources/citations, some old sources/references (published in 2013 and 2014), old interventions, lack of appropriate compatibility with the culture of the Islamic society of Iran, and direct translation of some parts of the original texts and documents of international meetings/gatherings.
Conclusion: Despite the efforts of the Mental and Social Health and Addiction Office to cover the needs of the majority of people referring to the primary care system, it seems that the high number of interventions and additional information can cause confusion of/for the physician, mental health care provider, family health care provider, general health care provider and other relevant individuals. It is, therefore, suggested that some packages be merged with each other, duplicate items be removed and specific sections be added, some packages be fundamentally revised based on the suggested items, and updated packages be developed with due consideration of  to the culture of the Iranian society.
