Abstract

Background and Aim: Nowadays, community health and different methods for health service delivery are among the most important public health issues in many countries. Most developing nations are trying to establish a health care system capable of meeting their basic needs.Hospitals are the most important service delivery points, and they need skilled manpower and adequate equipment for efficient performance of their medical care and educational functions. Today, the health of the mankind is facing greater hazards and challenges than at any other time in history, and within this context, hospitals are supposed to provide service of the highest standards and with minimum delay. Emergency wards are the sections where these principles apply more than anywhere else. These are rightly regarded as the "heart" of any hospital and a quick, efficient workflow in the emergency room can literally save lives.
Material and Methods: This was a cross–sectional (descriptive/analytic) performed in 2005 to assess the structure, process & performance in the emergency wards of 12 teaching hospitals affiliated to Tehran University of Medical Sciences.
Results: 33.3% of the ward chiefs were women the rest were men they were all more than 30 years of age. The average rate for implementing proper managerial standards was 90.6%. The "manpower index" was 69.1%, and the "facilities & equipment" index was 44/5%. The "directions index" and "activities index" were 66.97% and 93.5%, respectively.
Conclusion: The findings show that there is a positive and significant relationship between standards of management at the emergency department and the facilities and equipment index. Also, there are positive correlations between the emergency department's "manpower and rules" index instructions, activities, and years of service in hospitals.

Abstract

Background and Aim: 2,4-Dichlorophenol (DCP) is a compound  generated in a variety of industrial processes and also by chlorine disinfection of water polluted with phenolic compounds . Dumping of DCP into the environment and water resources is of great concern because of the compound's intense odor and toxicity. DCP is very soluble and resistant to biodegradability so it cannot be removed through conventional water and wastewater treatment processes. On the other hand, pollution prevention (P2) strategies have not been able to resolve the problem.

Material and methods: In this study, one of the AOPS named Fenton was used for DCP degradation. The method is based on simultaneous use of hydrogen peroxide and ferrous sulfate.

Results: Results of treatment with hydrogen peroxide and ferrous ion (as Fenton reagent ingredients) indicate that by oxidation of 50 mg/L DCP at 60 minutes contact time with iron concentration kept at 15 mg/L and various concentrations of H2O2 (50 and 100 mg/L), the efficiency of COD(Chemical Oxygen Demand) reduction would go up from 65% to 80% with higher concentrations of H2O2. We also determined the effect of various concentrations of ferrous ion on DCP oxidation rate. The results showed the prominent role of this ion in DCP treatment: increasing Fe concentration from 5 to 15mg/L produced a 60% reduction in COD occurred in 10 and 60 minutes contact times. Another conclusion was that the oxidation of DCP solutions by Fenton had a major effect on biodegradability so that BOD: COD ratios of these solutions increased significantly after this oxidation.

Conclusion: Pretreatment of waste containing dichlorophenol by the Fenton reagent can enhance the biodegradability of this chemical and perhaps of similar compounds in a short time interval.

Abstract

Background and Aim: Monitoring and evaluation are basic components of any health program. Control charts show clearly the process performance trend longitudinally and help managers and staff to detect general and specific variations and evaluate the process performance correctly. This study was conducted to design and utilize control charts in the primary health care (PHC) system.

Materials and Methods: This study was conducted in two stages. In the first stage, the average weekly, monthly and seasonal delays in receiving health services were compared retrospectively. In the second stage, appropriate control charts were introduced with due consideration of the PHC system.

Results: More than 95% of the weekly average delays were below the upper control limit, while in 100% of the cases the monthly and seasonal averages were under the upper control limit.

Conclusion:  Desirable health outcomes result from appropriate services. It is necessary to monitor health processes coverage and performance with simple and specific indicators. With regard to the health process outputs in the PHC system, it is suggested that the R-X, np and C charts be used in monitoring processes.

The control charts help the service providers to determine and manage process performance by gathering simple, applicable data. Some of the advantages of using these charts are the possibility of longitudinal surveys, simplicity of the calculation methods, and their high applicability.

Abstract

Background and Aim: Nowadays selecting an appropriate technology for storing and disposal of health care wastes is a difficult task. For example, selecting an optimum technology for storing and disposal of hospital wastes is a complex multi-criteria decision-making process, which requires consideration of qualitative and quantitative factors and criteria. This paper presents a new decision-making model for selecting a better method in the management of health care wastes in the City of Tehran.

Materials and Methods: So far selection of technologies for health care wastes storing has been based on inaccurate information or uncertain data, and linguistic variables have been used by decision-makers (experts) to assess the ranks, criteria and options described.In this study a combinational multi-criteria decision-making model has been used which includes the fuzzy analytic network process technique and the fuzzy MULTIMOORA method. This model uses the fuzzy analytic network process to get the related with criteria and sub-criteria of better health care wastes storage technology and uses MULTI MOORA method for evaluation of five available technologies based on the above-mentioned criteria.

Results:  Analysis of the data showed that the "environmental" and "technical" criteria were the most important criteria for selection of the best technology, while for storing health care wastes "hydroclave" is the most appropriate technology in the City of Tehran.

Conclusion: The proposed framework for assessing health care wastes technologies for storing hospital wastes is effective, and the hydroclave technology is to be considered as the best solution and the most suitable option from the environmental, public health and technical perspectives in the City of Tehran.

Abstract

Background and Aim: The present study aimed to determine the stages of changes and cognitive and behavioral processes (Transtheorical model) in the use of nutrients in overweight middle-aged women.
Materials and Methods: The present study was a descriptive-analytic. The research sample consisted of 281 middle-aged women with overweight referring to comprehensive health centers in Isfahan. Personal characteristics, stage of change and behavior and cognitive behavioral processes, and FFQ questionnaire (168-item) were completed by Questioning, and then the information entered the N4 and SPSS 18 software, and information was analyzed using descriptive and analytical tests.
Results: 57.7% of the people were inactive (pre-thinking, thinking, preparation) and 42.3% of them were in active phase. There was a significant difference between the different stages of change and the use of behavior change processes (p=0.001) And Benfrown's test shows the increasing use of cognitive and behavioral processes during the stage of change in use of nutrients (p<0.001).
Conclusion: The difference in the mean of changes processes during the stage of change show the impact of these processes on the nutritional individual's behavior, which is recommended to use stage of change and behavioral change processes for education nutrition behavioral.

Abstract

Background and Aim: One of the complex processes in the Ministry of Health and Medical Education in Iran is the process of registering pharmaceutical supplies. Currently the registration process is a multi-stage process, resulting in parallel services, a waste of time and unnecessary expenses. Therefore, an integrated system will improve the relevant service delivery. The purpose of this study was to identify a set of activities that could be collectively considered as a single service to create an integrated system for registering pharmaceutical supplies in the Ministry of Health and Medical Education, Iran.
Materials and Methods: This was an applied research, beginning with collecting information about the registration processes for 20 different products. In order to identify the services/steps of an integrated system for recording pharmaceutical supplies, first the Gray-Wolf multi-objective optimization (GWO) algorithm was proposed. Then the values of the algorithm parameters were extracted by the goal-based requirements analysis method and the algorithm was implemented. Finally the best services were extracted by the hierarchical analysis process.
Results: Through the proposed algorithm seven services were identified, including product class inquiry, document registration, document review according to license type and product class, licensing, laboratory services, clinical studies and payment service. These services were carefully approved with a precision of 97.3 by the experts of the Ministry of Health. The proposed framework for recording drug requirements was found to be effective and could facilitate the process by up to 90%, reduce the processing time by 80% and reduce the processing costs by 65%.
Conclusion: Creating an integrated system for registering pharmaceutical supplies is one of the important challenges of the Ministry of Health and Medical Education. This can be achieved by identifying services and combining these services to create an integrated system.
