Abstract

In this descriptive study, we compared the amount of public spending on health researchandtheprocessforallocationofresearchfundsbetweenIranandseveraldeveloped countries.
The study made it clear that the State&aposs total per capita spending on research and the percentage of the total government budget and the Gross Domestic Product spent on research were considerably lower than in most developed nations. The administrative process for obtaining funds was long and tedious, and private sector spending on health research was negligible (in contrast to Western countries, where the most research is funded by the private sector). The results of this study could have important implications for health care planning and research administration in Iran.

Abstract

Background and Aim: In the past few years publications in the field of medicine have increased considerably in Iran and knowledge production in this country shows the greatest growth rate in the entire Middle East region. Considering the fact that 12% of all faculty members in the health and medical sector in Iran are employed by Tehran University of Medical Sciences, this university is regarded as the country's leading research institution. This study is focused on the behavior of the researchers in this university in terms of collaboration in research and factors that affect such behavior.

Materials and Methods: Our samples included all research projects in Tehran University of Medical Sciences that had obtained grants from inside and outside the university in 2004, had registered their contracts, had been finished by the time of this study (October 2006- March 2007) and whose final reports had been submitted (315 projects). The data collecting forms were filled for 301 files. The researcher's questionnaire was sent to the principal investigators of the projects and eventually 208 questionnaires were collected. We used the SPSS software package and the usual statistical tests (frequency ratio, chi square, analysis of variance and multiple linear regression with the Enter method) to assess the factors affecting the participation scores of individuals. Based on the data gathered from the forms, we used the UNICET software to analyze the 'cooperation network' between project directors and their principal co-workers and the relationship between different sections of the university.

Results: Only in 2.2 percent of the projects whose contracts were arranged in 1383 there has been some cooperation between the project director/principal coworkers and non-academic organizations. This ratio reaches 2.5% for collaboration with other universities. Among all projects, health system research programs show a better status of collaboration. Linear regression analysis showed that another important factor affecting the participation score is the "method for choosing the research topic". University-affiliated research centers only collaborate with the Schools of Medicine and Public Health, while in the School of Rehabilitation and Physical Medicine all projects are defined within the School's own structure, with virtually no collaboration with any other sections of the university.

Conclusion: The present study showed that research programs in this university include little collaboration with organizations outside the university or with other universities. Part of this is related to the current structure of the research sector in the country, which is largely financed by the government with hardly any participation by the private sector. Current potentials within the University make it possible to embark upon joint research initiatives with other academic institutions. As the possible effect of intervention on health system research has been shown, policies can be implemented to promote effective collaborative research at university and national level.

Abstract

Background and Aim: Puberty is a critical period in a woman's life. An important step in strategies designed to promote family and community health is determination of girls' health priorities and needs assessment at puberty. This study aims to illustrate the efficiency of need assessment approach in exploring and comparing the viewpoints of mothers, teachers and adolescent girls on girls' pubertal health in the context of the school. The findings would help plan better programs aiming at changing the heatlh behaviour of girls.
Materials and Methods: In this qualitative study 12 guidance school girls, 12 mothers and 7 teachers from District 6, Tehran were selected to participate in focus group discussions to prepare a semi-structured questionnaire. The content analysis method was used to analyze the results of discussions with the participants and complete the final questionnaire.
Results: The findings reveal the efficiency of the need assessment approach as follows: a wide discrepancy in viewpoints of the three study groups was identified in terms of pubertal health priorities, health needs of the girls, and pubertal health educational priorities. In addition, the results show a lack of a notion of comprehensive sex health education in the school.
Conclusion: The findings indicate that the need assessment approach is a useful way to plan and implement appropriate health education programs for adolescent girls.

Abstract

Background and Aim: Priority setting is one of the most important issues in health research systems. No national health system can afford to finance all research projects proposed by researchers, especially in developing countries. Therefore, we decided to set the main national health research priorities in Iran using the Essential National Health Research (ENHR) method.

Materials and Methods: All of the Iranian universities of medical sciences and other stakeholders collaborated in this study. The methodology for research priority setting was based on needs assessment and ENHR. 

Results: The total number of research priorities gathered from universities of medical sciences was 6723. The proportions of topics related to basic science, applied and development subjects were 17%, 78% and 5%, respectively. Further analysis of the data showed that almost half (46.3%) of the research priorities were descriptive, 36.0% analytical, and 17.6% interventional. The research priorities were divided into 9 main areas, namely, communicable diseases, noncommunicable diseases, health system research, pharmaceutical sciences and industrial pharmacy, basic science, traditional and herbal medicine, nutrition, environmental health, and dentistry.

Conclusion: Up to now the common procedure for research priority setting has been, with a top-to-bottom approach, managed by a limited number of researchers and experts, while in the method presented in this paper a bottom-to-top approach is used, which is more effective.

Abstract

Background and aim: Oral and dental diseases limit an individual&aposs activities at the school, work place and home, leading to a loss of millions of work and school hours all over the world. The period of adolescence is critical as regards shaping of healthy behaviors related to oral and dental diseases. The purpose of this study was to explore and describe school pupils&apos viewpoints on factors influencing development of dental caries.  

Materials and Methods: A qualitative study with a content analysis approach was conducted to collect the perspectives of 18 Iranian students in 8 guidance schools selected by purposive sampling. Structured and semi-structured interviews were held to collect data. An informed consent was obtained and the interviews were recorded and transcribed. The data was analyzed through an inductive approach of content analysis. In order to support the validity and rigor of the data, different criteria such as acceptability, confirmability and transferability were utilized.

Results: Data analysis could identify four major factors influencing dental caries development: "family interactions in dental health", "role of school in caring for teeth", "role of dentists in caring for teeth" and "effect of education in caring for teeth". The factors influencing caring for teeth and mouth had a greater significance.

Conclusion: The students&apos experiences, as observed in this study, can help in developing mutual trust between students and dentists, delivering better services in schools by relevant organizations, and designing more effective oral health interventions, with due consideration of vital role of the parents.

Abstract

  Background and Aim: Current traditional research methods for solving social problems were challenged due to limited participation of community . The health companion project was carried out with the aim of determining effective model of community participation for solving health problems in Boyerahmad and Dena township. .

  Materials and Methods : This study is a community based participatory research performed in Kohgiloyeh and Boyerahmad province 2005-2009.The study population was the entire rural and urban households of Boyerahmad and Dena county.The strategic committee of project including academic researchers, managers of health and health related sectors was formed and this committee selected six regions for research activities. In each region local directing group "health companion " was established with participation of stakeholders: academic researchers, local leaders, health providers and public representatives to guide all aspects of project. The members of the group empowered and enabled by attending training workshops for need assessment, priority setting, research methodology that through these activities the health problem priorities of the regions were recognized, the research area determined and intervention programs were designed and implemented according to these research area.

  Results: The most important achievement of this project is the methods of activities to reaching goals. Implementation of six participatory interventional proposal for solving health problems and needs are the other achievement. The frequency and means of health problems and their risk factors significantly reduced after the completion of intervention program in each region.

  Conclusion: Establishing and activities of health companion groups followed model of Planned Approach to Community Health (PATCH) that help community to form health promotion team, collecting and organizing of data, choosing health priorities, developing a comprehensive intervention plan and evaluation .

Abstract

  Background and Aim: Women perceive menopause differently. Beside biological changes, women experience menopause based on the cultural scenarios. The qualitative research presented here is about how Iranian women perceive menopause within cultural scenarios.

  Materials and Methods : Data was collected during 4 focus group sessions and 6 individual in-depth interviews of women aged 40 years and older. Informants were selected from the Tehran University of Medical Sciences. The sessions were facilitated by using a semi-structured inventory focus group. Content analysis was adopted using combined free and analytical coding to reduce data, to extract meanings, to categorize domains and to obtain themes .

  Results: We categorized our findings into four main domains: gender role, the changes in socialization, social interactions and supporting resources. Descriptions of the later domain are going to be reported in subsequent manuscript.

  Conclusion: The women's understandings of menopause are shaped based on the specific cultural scenarios. Menopause recognized as a factor which changes women’s life line. Meanings as 'Loosing femininity' and 'changes in women's roles in the community' describe women's gender role, social interactions and changes in the socialization throughout the menopause years. The women’s misconceptions, accumulated myths, and ignorance about femininity were evidenced based on narratives. The majority of women raised ‘role change’ as an important determinant in social interactions. Gender role changes as well as women's socialization patterns have forced women to accept loosing femininity as a reality. Developing comprehensive and culturally sensitive health services to address women's needs in the climacteric years is suggested .

Abstract

  Background: Sexually transmitted infections are one of the most prevalent infections all over the world that impose significant morbidity on people. There are an increasing number of sexually transmitted infections in Islamic Republic of Iran during recent years. So developing and implementing programs for STIs prevention and control is considered as a health priority. While the role of gender based power in sexual relationships has in recent years been acknowledged, the understanding has largely lacked practical considerations in the STIs/HIV/AIDS prevention fields. This study aims to explore gender sensitive STIs/HIV/AIDS prevention services concepts and dimensions.

  Method : This study employed content analyzes research design. Semi-structured in-depth interviews were conducted with 37 expert reproductive health managers and providers that selected purposively and then continued by snow ball method. Data collected by using semi-structured interview guide. The interviews were transcribed and typed. Data was analyzed according to content analysis.

  Results: 2 categories emerged as the result of data content analysis: 1) gender sensitive structure including employers, facilities and management dimensions 2) gender sensitive process including care and educational processes.

  Conclusion: providing gender sensitive STIs/HIV/AIDS prevention services need gender sensitive management, facilities and providers and gender sensitive care and educational design.

Abstract

  Background and Aim: Health-risk behaviors are socially constructed and influenced by the given culture. Therefore, the health-risk behaviors are prioritized differently in diverse cultures. In this study, we aimed to explore the priority of health risk behaviors among high school students in Tehran.

  Materials and Methods : In this qualitative study using rapid assessment approach, we interviewed 130 individuals who voluntarily participated in the focus group discussions (FGDs, n=17). They were students (n=9 FGDs), the parents (n=5 FGDs), and the teachers (n=3 FGDs). Four of the principals were individually interviewed. Interviews focused on (1) perceptions about and description of healthy or unhealthy behaviors (2) priority of health risk behaviors in adolescents (3) perceptions and beliefs about the formation of the risk behaviors among adolescents. All sessions were embarked on the public schools located in the area five in West Tehran. After obtaining a written consent form from the participants, primary investigators and three of the trained facilitators (male and female) conducted the sessions. We employed content analysis to extract the major themes and sub-themes. The rigor of the data was obtained.

  Results : Addiction, risky sexual behavior, and social misconduct ion (aggression) were identified as the main concerns for the boys. Risky sexual behavior, early marriage, eating unhealthy habits, physical motionless, and fear of the future (job, education, marriage) were identified as the main health-risk behaviors for the girls.

  Conclusion : According to our findings, health risk behaviors are classified differently for boys and girls. However, sexual risk behaviors are common findings for both groups. The adolescents' risky behaviors are formed throughout their socialization and seriously influenced by the given culture. Understanding of gender schema was identified as an important factor in the way participants do priority health-risk behaviors of young people. Comprehensive and holistic health education and promotion programs targeting adolescents as well as training programs for parents are suggested.

Abstract

Background and Aim: Much emphasis is being put on participation of the community and parents in early child development (ECD). This article discusses the lessons learnt from the MAZDAK project, which attempts to involve the community in the Monitoring and Promoting Children’s Development Program.

Materials and Methods This was a 9-month community-based participatory research (CBPR) project of the Health System Research (HSR) type. The target population were the families residing in western Eivanak, including 300 children. All the phases of the project were conducted with participation of groups of 10 local volunteer women (in Persian singular Rezakar, plural Rezakaran), each group covering 30 children. At the end of the period challenges and limitations of the community participation were discussed in 2 focus-group discussions (FGDs) and a telephone survey with the participation of 50 of the parents.

Results: Community participation could be seen at three levels: 1. Integrating and linking the existing services and programs in community organizations; 2. Creating an interface organization of people in order to facilitate involvement of community members (Rezakaran); 3. Involving the parents themselves in the process of monitoring and promoting child development. Barriers to more active participation of parents were as follows: mothers’ occupation; number of children; father’s or other family members’ disagreement of the; holidays and bad weather; large numbers of individuals under coverage of each Rezakar which made her deeper understanding and communication with the mothers difficult; irregular follow-ups; age and educational differences between the Rezakaran and those under their coverage; frequent changing of Rezakaran.

Conclusion: Building trust, engaging and empowering people and governmental and non-governmental organizations is the greatest challenge in such a program. Experience shows that it is possible to increase the chance of success by selecting people’s favorite programs and priorities, using simple and friendly tools (ASQ Album), use of technologies to facilitate communication, selection of volunteers with due consideration of social conditions and, finally, increasing the motivation of volunteers for sustainability of activities.

Abstract

Background and Aim: Family female heads are one of the most vulnerable groups in the society and empowering them is essential. This research was a systematic study to design and develop a model of educational needs of female heads in the female-headed households.
Materials and Methods: This study was a qualitative applied research of a research synthetic type conducted using content-analysis and meta-analysis. The statistical population included all the relevant works published in Persian or English between 2012 and 2021 available in four reliable databases, namely, ERIC, Science Direct, PubMed and Google Scholar. Using the keywords "education of women-headed households, women's empowerment, women-headed households, and skills of women-headed households" and based on the inclusion criteria, initially 140 scientific articles were identified and finally, based on the exclusion criteria, 47 articles were selected for the final analysis. The Software MAXQDA 18 was used for the qualitative analysis of the articles.
Results: Analysis of the data obtained showed the educational needs of women-headed households as the main theme to consist of four subthemes including "knowledge and skills of desirable sexual orientation", "knowledge and skills of raising children", "knowledge and skills of occupation" and "individual and social development and empowerment”. Based on meta-analysis out of the four main themes 22 themes were extracted and a model was designed for educating female-headed households.  
Conclusion: Considering that the educational needs of female-headed households have been identified, it is recommended that educational courses be designed in accordance with their educational needs.
