Abstract

Background and Aim: Job enrichment is a method that has been used to increase employee satisfaction and work motivation. The Hackman and Oldham job characteristics model (1976) has served as the foundation for many job enrichment efforts. In particular, a considerable amount of research has been devoted to the study of the job characteristics-job satisfaction relations. Te purpose of this study was to statistically determine the relation between job characteristics and job satisfaction among employees of hospitals affiliated to Tehran University of Medical Sciences (TUMS) in 2007.
Materials and Methods: This study was a cross-sectional one, including 6 TUMS-affiliated hospitals-Emam, Shariati, Baharlo, Ziaeian, Madaen and Azadi. The sample size was 400, consisting of 200 administrative employees (50%), 151 nurses (38%) and 49 physicians (12%). The random sampling and stratified proportional sampling methods were used to select the hospitals and interviewees, respectively. Two questionnaires were developed and used to identify job characteristics and job satisfaction: 1) job diagnostics surveys (JDS) and 2) Hertzberg's job satisfaction questionnaire. The reliability of the questionnaires was ascertained by test-retest using the Pearson correlation coefficient (r = 0.88). Linear-by-linear association and ordinal logistics regression analysis were used for analyzing the data gathered.
Results:The relations between the Motivational Potential Score (MPS) and job characteristics (except for feedback) with job satisfaction were statistically significant (p<0.05). Task identity was the most effective determinant of job satisfaction.
Conclusion: It appears that all the job characteristics (except feedback) are important determinants of job satisfaction of the hospital employees. Although the work environment is in need of restructuring, it is important to note, from a positive perspective, that the job characteristics discussed in this study are readily amenable to change job satisfaction at the organizational level.

Abstract

Background and Aim: Despite the fact that the proportion of youth population is very high in Iran, there is no well designed, comprehensive reproductive health program for the youth. This prompted the health planners to evaluate all the existing centers that deliver reproductive health services to the youth in the country in order to identify the weaknesses and strengths of the services and the centers.

Materials and Methods: A descriptive analytic study was conducted from March 2006 to March 2007, in coordination with the Vice-chancellor for Health of Universities of Medical Sciences, through Offices for Health of the Youth and Schools (OHYS). Five health centers, namely, Tehran, Semnan, Bam, Chabahar, and Fassa Health Centers (the only centers delivering reproductive health services to the youth) were included in the study. They were assessed for structure and physical facilities, job satisfaction of the personnel, satisfaction of the youth consulting them, and the services (including consultative services) delivered. The data were gathered through questionnaires using a checklist and analyzed by the SPSS-13 software.

Results: The health center personnel thought that the quality of services delivered at the centers was low. They also believed that the physical facilities, including equipment in the clinical examination room, furniture, cleanliness of the premises, and entertainment facilities, were not of an acceptable standard. About 64% of the personnel of the health centers personnel expressed job satisfaction. On the other hand, 39% of the young people consulting the centers expressed full, and 54% relative, satisfaction with the services they were receiving the remaining 7% were not content at all. The highest rate of satisfaction was with the psychologist, physician, and midwife, so that 95% of the clients recommended the centers to other young people seeking reproductive health services. Finally, the clients said that in most of the centers paramedical personnel, including nutrition officers (50%) and midwives with a Master's degree (17.5%), were insufficient.

Conclusion: It is suggested that a comprehensive service package be developed to be used in all the health centers, rather than each center deliver services based on the existing facilities and individual preferences. Such a package should be based on the national reproductive health programs (according to the medical education curricula). Intersectoral collaboration (particularly from the Ministry of Education and cultural organizations) is also very essential.

Abstract

Background and Aim: Pressure ulcer is a common problem and a large drain on hospital resources, especially in wards such as ICU where patients stay for a long time. The aim of this study was to explore the likely factors contributing to pressure ulcers in the ICU units of Tehran University of Medical Sciences teaching hospitals, Tehran, Iran.

Materials and Methods: This project was conducted in all the seven ICUs of four teaching hospitals affiliated to Tehran University of Medical Sciences. In the first phase the researcher performed a direct observation of all the 90 patients who were admitted to the ICU units to check the presence of any pressure ulcer. In the second phase, 310 patients discharged from the same ICU units between March 2007 and February 2008 was randomly selected and their medical records reviewed. A structured questionnaire was used to collect the data.

Results: Pressure ulcer occurred more frequently in the elderly women patients who were less active and had less mobility, hospitalized for a long time, or those with fever, diabetes, high blood pressure, paralysis, or respiratory diseases. In addition, pressure ulcer was seen more frequently in the patients hospitalized in the general ICUs, as well as in those with infections and different types of tumors.

Conclusion: Using a standard risk assessment tool and paying attention to the main risk factors of pressure ulcer can be a useful method for identifying the high-risk patients before admission in order to prevent them from developing such ulcers.

Abstract

Background and Aim: Job satisfaction is one of the most important factors in maintenance of a professional person and the job success, increasing the personal efficiency and the quality of services. On the other hand, it is a tool in the hands of managers in order to maintain the staff in their jobs. The aim of this descriptive study was to investigate the satisfaction of physicians and midwives participating in the family physician scheme with the plan.
Materials and Methods:
The study population was made up of 221 physicians and 144 midwifes. The tool was an author made questionnaire certified by experts as a standard, valid and reliable scale via content validity measurements through a pilot study.
Results: The average satisfaction of physicians and midwives was 44.34% while the highest satisfaction was 65% with the location of services and the lowest was 30.6% with the financial issues. In relation to the financial affairs, physicians were more dissatisfied than midwives, so satisfactions were increased by the increase in the amount of salaries.
Conclusion:
Over 4 years of implementation of family physician plan, the physicians and midwives satisfaction was shown to be still low. It is necessary to consider re-programming in the scheme in order to increase the amount of payments and create more motivational factors for physicians as well as to reduce working hours for midwives and establish appropriate levels of payments for them.

Abstract

Background and Aim: Stress during pregnancy can have a lasting effect on the mental health of women after childbirth. Statistics show that about 18% of pregnant women have major or partial depression during pregnancy, but many have not been screened or treated. Lack of treatment can have serious consequences for the mother and her child. The purpose of this study was to determine the mental health status and marital satisfaction of pregnant mothers in Kashan city, Iran in 2015.
Materials and Methods: This was an analytical cross-sectional study. Using the convenience sampling method 202 pregnant women were selected from four urban health centers in Kashan, Iran and entered the study. Data were collected using a Diagnostic Interview, the General Health-28 Questionnaire and the Golombok Rust Inventory of Marital State. Descriptive statistics was used for data analysis, the statistical tests being one-way ANOVA, chi-square test, Pearson correlation test and logistic regression.
Results: The results showed that 26.7% of the pregnant women were suffering from psychiatric disorders, the most and least serious disorders being related to physical activity and depression, respectively. About 80% of the women expressed very good marital satisfaction. Further analysis of the data showed statistically significant associations between depression and age and anxiety, insomnia and duration of marriage, marital satisfaction and delivery type, and mental health and marital satisfaction. Furthermore, marital satisfaction had a negative association with education. (p<0.05).
Conclusion: It seems that it is essential to develop and implement a therapeutic, psychiatric and health care model for women at risk of psychiatric disorders during pregnancy and after childbirth in the health service delivery system in Iran.

Abstract

Background and Aim: The aim of the present study was to investigate the relationship of hope and life satisfaction with students bullying and bullying victimization with the mediating role of school connectedness.                                    
Materials and Methods: This was a cross-sectional study, the statistical population being high school students of Tehran city in the school year 2015-16. A sample of 300 students was selected using the stratified random sampling method. The data collection tools were the Snyder’s hope scale, Diener et al. life satisfaction scale, Waters and Cross School Connectedness scale and the Espelage and Holt Bullying scale. The hypothetical model was analyzed using path analysis with the LISREL8.57 software.  
Results: Analysis of the data showed the following: 1. Hope and life satisfaction had a structural positive direct effect on school connectedness; 2. Life satisfaction had a direct negative effect on bullying; 3. Bullying had a direct effect on bullying victimization; 4. School connectedness had no direct effect on students bullying and bullying victimization. 5. Life satisfaction had an indirect effect on bullying victimization through bullying; 6. Finally, the indirect effects of hope and life satisfaction on bullying and bullying victimization through school connectedness were not confirmed.
Conclusion: In can be concluded that life satisfaction is a facilitating factor for the school connectedness, serving as a protective factor against bullying. Hope and school connectedness may act as protecting factors in the prevention of bullying.

Abstract

Background and Aim: Happiness and benevolence in individual life and satisfaction in marital life are very important from a social point of view. Happiness is one of the most important factors for strengthening and sustainability of marital relationships.
Materials and Methods: A thorough search was done in the following data banks using the Persian and English key words happiness, marital satisfaction and Iran: Magiran, Pubmed, Web of Science, IranDoc, IranMedex and SID. In the initial search, 41 articles were found and studied. After reviewing the full texts of the articles, 8 papers were finally selected for analysis. Data were analyzed using the Revman software, the statistical test being meta-analysis (random effects model).
Results: In the 8 articles (published during the period 2006-2018) which were analyzed, the total number of study participants was 1563. The means of the completed Oxford happiness and Enrich's marital satisfaction questionnaires were 57.3 and 137.2, respectively
Conclusion: Based on the findings, it is concluded that there is a positive correlation between happiness and marital satisfaction of couples in Iran.Therefore, implementing appropriate educational and public health programs is necessary to strengthen the underlying factors of hope and happiness in order to increase the life satisfaction of couples and reduce family conflicts and divorces.

Abstract

Background and Aim: This study aimed to improve the symptoms of children with obsessive-compulsive disorder (OCD), relationships between parents and children, and marital satisfaction using the family-based cognitive-behavioral therapy (CBT).
 Materials and Methods: This was a quasi-experimental pretest-posttest research. The statistical population was all of the 9- to 12-year-old school students suffering from OCD who had consulted the Psychological and Consultation Services Center during academic year 2015-2016 in the city of Tehran. A sample of 28 students selected randomly were assigned to an experimental and a control group. The following questionnaires were completed initially (pre-test) and finally (post-test): 1. Questionnaires completed by the mothers of the children: the Spence Anxiety Scale-Parent Form, the Parent-Child Relationship Questionnaire and the Afrooz Marital Satisfaction Questionnaire (short form); 2. The questionnaires completed by the children: the Spence Children's Anxiety Scale. The intervention program was designed based on CBT and included 22 sessions, eight group sessions for mothers, eight individual play therapy sessions for children, and two specific sessions for each family.
Results: Data analysis using multivariate analysis of covariance showed that family-based cognitive-behavioral therapy was effective in improving marital satisfaction and parent-child interaction as well as reducing child anxiety (p <0.01).
Conclusion: Based on the findings, it can be concluded that family-based cognitive-behavioral therapy intervention may lead to improved relationship between parents and children with obsessive-compulsive disorder and marital satisfaction, as well as reduced anxiety of these children and their parents.

Abstract

Background and Aim: One of the dimensions of health is mental health that could be affected by the quality of occupational life. The purpose of this study was to determine the effect of job satisfaction on teachers' mental health with due consideration of the moderating role of gender and the subjects taught.
Materials and Methods: This was a quantitative survey using a questionnaire including a sample of 373 teachers, selected by multistage random sampling, from among all (n = 1500) the secondary school first- and second-grade teachers in Najafabad city, Isfahan Province, Iran. Data analysis was done using the software AMOS and SPSS.
Results: The confirmatory factor analysis of the items showed that these items could explain 63.25% of the variance of the mental health construct. In addition, the standard effect of job satisfaction on mental health was found to be 0.37 (p <0.001), explaining 13% of the mental health construct variance. Further analysis of the data showed that the teachers’ mean mental health construct (66.64) and their mean of job satisfaction (73.90) were lower and higher than the means of the respective spectra (p <0.001), respectively.
Conclusion: Job satisfaction, which has been proven to affect the teachers’ mental health, can improve their personal and social life and make them more efficient in education. In this regard, gender and the type of subject taught can have moderating effects on the relationship between job satisfaction and mental health of the teachers. Based on the beta-values (0.55 for the women group and 0.68 for the experimental science teachers group), the gender female and teaching experimental science can have a stronger effect on the teachers’ mental health than gender male and teaching subjects other than experimental science.

Abstract

Background and Aim: Breast cancer is one of the most common cancers in Iran. The aim of this study was to assess the effectiveness of existential-humanistic therapy in self-acceptance and marital satisfaction in women with breast cancer.
Materials and Methods: This was a quasi-experimental study with a pre-test, post-test and follow-up design with a control group. The statistical population was the women with breast cancer referring to Shahid-Rahimi Hospital in Khorramabad, Iran in 2020, from among whom two groups (experimental and control) of 15 women each were randomly selected and asked to complete the self-acceptance (USAQ) and marital satisfaction (ENRICH) questionnaires. Then the experimental group attended eight 90-minute intervention sessions. After the intervention, both groups took a post-test and followed up further after two more months. The data were analyzed using the repeated measures ANOVA test.
Results: The repeated measures ANOVA test showed that the interaction between group and time was statistically significant. Therefore, the self-acceptance and marital satisfaction variables increased significantly (F=5/77, P<0/01 and F=13/97, P<0/01, respectively).
Conclusion: According to the results of this study, existential-humanistic therapy has a significant effect on self-acceptance and marital satisfaction in breast cancer patients. It is, therefore, recommended to pay attention, in addition to the physical problems of such patients, also to their psychological problems and, along with pharmacotherapy consider psychotherapy, including existential-humanistic therapy as well.

Abstract

Background and Aim: The District Health Network is based on the principles of equity, appropriate technology, inter-sectoral collaboration and public participation. Measuring patient satisfaction is one of the basic criteria in determining the quality of care services. The purpose of this study was to compare the level of satisfaction of service recipients between government and non-government health posts in the south of Tehran in 2021.
Materials and Methods: This was a cross-sectional, descriptive and analytical study. A two-part researcher-developed questionnaire was used to collect data. The first part included items related to demographic characteristics of the clients, insurance status, geographical location and type of health post, appointment system and frequency of visits, and the second part contained questions on the type and level of satisfaction with the services provided.
Results: In the governmental health posts, the clients were mostly satisfied with, in a decreasing order, vaccination services, post-vaccination education, waiting time and physical space. In the outsourced posts, post-vaccination care education, under-one year old child care, vaccination and a sense of responsibility obtained, in an increasing order, the highest scores of satisfaction. Further analysis of the data showed the clients’ least satisfaction with youth and middle-aged care in governmental posts and youth care in outsourced posts.
Conclusion: Continuous and frequent measurement of satisfaction and developing timely interventions based on the results obtained will help greatly promotion of the quality of health services.
